Instructions: Please fill out the table below with your event preferences.
Parents
• Please indicate at least 5 to 10 events that you would be willing to coach in the “P” column. Number them according to your
preference, with “1” being your first preference. If you do not have a preference, indicate “1” next to all of the events that
you would be willing to coach. You will only be assigned to one event.
• Read and fill in page two of the form and sign.
• Submit the application with payment indicated below.
Students
• Please indicate the number of events in which you would like to participate. Only students with 3-4 events will be eligible
for the State Competition Team. We recommend that sixth grade students compete in one or two events.
• Follow the Science Olympiad link on MVMS’ web site for descriptions of events.
• Rank the events according to your preference in the “S” column of the table, with “1” being your first preference.
• Please rank all of the events (1through 23) even if you do not want to participate in them, so that we know which ones are
your least favorites.

Note: Selecting students and coaches for events is a complex process involving student preferences, team balance, team
competitiveness and event logistics. The selection committee makes every effort to give students and coaches their preferences, but
cannot make guarantees. When a parent coaches an event, the student will be given preference for that event.

 Students: Please list any prior Science Olympiad or Science Field Day events (include how you placed in
competition). Also indicate any relevant hobbies or activities.

I give my child permission to participate in the Science Olympiad practices, to attend event Workshops, attend the Science Olympiad
Regional Competition at Rancho Bernardo High School on 6 Feb 2010, and, if selected, to attend the State Competition in April/May
2010 (Fri. night & Sat. day.) in the Los Angeles area. All transportation to be provided by parents.

I agree to be responsible for any damage done by my child, accidentally or purposefully, during any competition or prep/study.

I understand that parent volunteering by at least one parent is required for my child to be a team member. My child and I
both understand that participation in practice sessions is critical to ensuring success at the Regional and State Competitions.

I may be coaching or co-coaching. I understand that MVMS normally qualifies for the State competition and that selected 3 or 4 event
students and all coaches are expected to continue practices until the State competition in April or May.

Fees: One Event: $50 (includes regional registration expenses, competition expenses and t-shirts)
