You are hereby advised to submit to the Administrative Di . this Office, not later than October 31, 2016, applications for reclassification/conversiog of teacher positions to T-l1, T-IIl, MT-I, MT-ll and Administrator positions (Principal/Head Teacher) together with the supporting documents (1 copy exch) and ight (8) copies of Plantilla Allocation List (PAL) with colatilla in each page 2. It is emphasized that only those applicants who meet the existing criteria based on the PenFd Organization and StalTing Standards and Qualification Standards and applicants for School Principal/Head Teacher Positions based on DepEd Order No. 97, ¢, 2011 dated December 16, 2011 should be included in the reclassification requests- 3. Attached are list of required documents and guidelines, for information and guidance epartment of Ed:cation Incls: as stated ROAS “LEASED List of documents required for the following requests: (Except for the eight (8) copies of the Plantilla Allocation List, all other documents should be submitted in one copy each.) \ Request for Funding and Issuance of NOSCA for Reclassification of Teaching Position due to Approved Equivalent Record Forms (ERFS). 1, Letter request of the School Principal/Head in the case of secondary schools, and Schools Division Superintendent in the case of elementary school; 2. Eight (8) copies of the Plantilla Allocation List with collatila in each page, as follows: “hereby assume full responsibility for the veracity and accuracy on the evaluation of the above reclassification of positions in accordance with existing DepEd guidelines, and the authenticity of the supporting documents as submitted by the applicants. We hereby release the Department of Budget and Management from any liability, whether civil or criminal as a result of any biases, negligence or fictitious documents used as basis, in the reclassification of the above positions which may result to complaints of any aggrieved party in the future.” (see note 1) Schools Division Superintendent 3. Endorsement by the Schools Division Superintendent and Regional Director of the abovementioned request in the case of secondary schools and endorsement by the Regional Director in the case of elementary school; 4. Certification on the availabilty of funds for the purpose by the Budget Officer of the Schools Division Office, if the lump-sum for the purpose has already been allocated by Schools Division or if not, certification by the Regional Accountant or Regional Budget Officer, whoever is assigned to control the lump-sum appropriation in the Regional Office; and 5. Properly evaluated/reviewed/approved Equivalent Record Forms (ERFs) of the incumbent of the position requested to be reclassified, Note: 1. Previously, we required the submission of the authenticated Certificate of Authentication and Verification (CAV) issued by the Commission on Higher Education whether or not the incumbent graduated from whatever Master Degree and certifying the authenticity of the copies of diploma and Transcript of Records because there are fake documents that were previously inadvertently honored by DepEd. With this colatilla, which we required to be included in the PAL, we no longer required the submission of such CAV since the responsibility to determine the authenticity of the documents submitted by the incumbent to be reclassified thru approved ERFs is already the sole responsibility of the Schools Division Processing of Request for Funding and Issuance of NOSCA for Conversion of Teaching Position to Master Teacher (MT) Position or reclassification of MT to a higher level of MT ion. 1. Letter request of the School Principal/Head in the case of secondary schools, and Schools Division Superintendent in the case of elementary school; 2. Endorsement by the Schools Division Superintendent and Regional Director of the abovementioned request in the case of secondary schools and endorsement by the Regional Director in the case of elementary schools; 4. Certification on the availabilty of funds for the purpose by the Budget Officer of the Schools, Division Office, if the lump-sum for the purpose has already been allocated by Schools Division or if not, certification by the Regional Accountant or Regional Budget Officer, whoever is assigned to control the lump-sum appropriation in the Regional Office; and 5. For Secondary Schools: 1. List of the name of Teachers (with corresponding item nos. per PSIPOP) by Subject Area (major/minor subject per transcript of records) to be certified by the School Principal/School Head. To be used as basis of the Schools Division Office/Regional Office in evaluating whether or not the particular subject area of secondary school is qualified to have a Master Teacher position. Transcript of records to be attached to the request as agreed during the RAMT consultation- dialogue. 2. List of existing Master Teacher positions per subject area or Certification that there is no existing Master Teacher position in the school, in case there is none. For Elementary Schools: 1. Summary List of the Number of Teachers per District to be certified by the Schools Division Superintendent which should not exceed the total number of teaching positions in the latest PSIPOP or our GMIS record. 2. Summary List of the Number of Master Teachers per District to be certified by the Schools Division Superintendent. 6. Evaluation sheet for ranking Master Teacher in prescribed form properly ‘evaluated/reviewed by the Division Evaluation Committee/Selection and Promotion Board and approved by the Schools Division Superintendent. 7. In case of lone applicant, waiver regarding non-interest in applying for reclassification of position to Master Teacher of all teachers listed under the concerned subject area. 8. In case of reclassification from Master Teacher position to a higher level of Master Teacher position (say, MT | to MT 11 or MT Il to MT il), any document that would support the educational qualification and length of service of the incumbent to the existing MT position of the incumbent. M. Processing of Request for Funding and Issuance of NOSCA for Reclassification from Teacher position toHead Teacher (HT)position or HT position toSchoo! Principal position as school head/school in-charge of both Elementary and Secondary School. 1. Letter request of the Schools Division Superintendent; 2. Eight (8) copies of the Plantilla Allocation List with colatilla in each page, as follows: “I hereby assume full responsibility for the veracity and accuracy on the evaluation of the above reclassification of positions in accordance with existing DepEd guidelines, and the authenticity of the supporting documents as submitted by the applicants. We hereby release the Department of Budget and Management from any liability, whether civil or criminal as a result of any biases, negligence or fictitious documents used as basis in the reclassification of the above positions which may result to complaints of any aggrieved party in the future.” (see note 1) Schools Division Superintendent 3. Endorsement by the Regional Director of the abovementioned request; 4. Certification on the availability of funds for the purpose by the Budget Officer of the Schools Division Office, if the lump-sum for the purpose has already been allocated by Schools Division or if not, certification by the Regional Accountant or Regional Budget Officer, whoever is assigned to control the lump-sum appropriation in the Regional Office; and 5. Certification that there is no existing Head Teacher or Principal item for the school; 6. Position Description Forms; 7. Organizational Chart; 8. Justification of the reclassified position; 9. In case of reclassification from Head Teacher position to a higher level of Head Teacher position (say, HT | to HT ll or HT Il to HT Il) or from School Principal to a higher level of ‘School Principal position, any document that would support the educational qualification (CAV/ and length of service of the incumbent to the existing HT/ SP position 10. Service Record 11. Performance rating for 3 consecutive years or 2 years if ratings are both “Outstanding” W. Processing of Request for Funding and Issuance of NOSCA for Reclassification of Position from Teacher position to Head Teacher (HT) position for supervising at least 6 teachers under his/her subject area for Secondary School only. Letter request of the Schoo! Principal/Head in the case of secondary schools; Eight (8) copies of the Plantilla Allocation List with colatilla in each page, as follows: “Ihereby assume full responsibility for the veracity and accuracy on the evaluation of the above reclassification of positions in accordance with existing DepEd guidelines, and the authenticity of the supporting documents as submitted by the applicants. We hereby release the Department of Budget and Management from any liability, whether Civil or criminal as a result of any biases, negligence or fictitious documents used as basis in the rectassification of the above positions which may result to complaints of any aggrieved party in the future.” (see note 1) ‘Schools Division Superintendent Endorsement by the Schools Division Superintendent and Regional Director of the abovementioned request; Certification on the availability of funds for the purpose by the Budget Officer of the Schools Division Office, if the lump-sum for the purpose has already been allocated by Schools Division or if not, certification by the Regional Accountant or Regional Budget Officer, whoever is assigned to control the lump-sum appropriation in the Regional Office; List of the name of Teachers (with corresponding item nos. per PSIPOP) by Subject Area (major/minor subject per transcript of records) to be certified by the School Principal/School Head. To be used as basis of the Schools Division Office/Regional Office in evaluating whether or not the particular subject area of secondary school qualified to have a Head Teacher position. Transcript of records to be attached to the request as agreed during the RAMT/FDU consultation-dialogue. List of the name of existing Head Teacher positions by subject area or Certification that there is no existing Head Teacher position in the school, if there is none. This is being required since only one HT position is allowed per subject area. In case of reclassification from Head Teacher position to a higher level of Head Teacher Position (say, HT | to HT Il or HT II to HT Ill), any document that would support the education qualification (CAV/ and length of service of the incumbent to the existing HT position Service Record Performance rating for 3 consecutive years or 2 years if ratings are both “Outstanding”.
