OAC 5101:2-12-04(B)2)(e) states that a fire inspection and approval needs to be obtained at least once annually within each twelve months from the date of the last fre epproval report, VIOLATION: 4. As documented in the 06/22/2017 inspection report, ODIFS staff determined thatthe center had not been inspected and approved within 12 months from the date of the last fire approval, as required. 2. QAC5101:2-12-09(C)2}' states that criminal record checks are required prior to the first, ‘day of employment for employees and childcare staff members and the results shall be reviewed by the administrator prio to the employee or child care staf member being let alone with children, ‘VIOLATION: a. As documented in the 06/22/2017 inspection eport, ODIFS staff determined that background checks were not requested for childcare staff member Sakeya who had sole responsibility forthe Pre-K group of children and did not have BCI or FBI criminal record checks results on Bile, as required 3, QAC5101:2-12-09(CY) states that criminal records checks are required every five years from the date of the mest recent background check. QAC 5101:2-12-10(A)(D(a) states that a center shall have atleast one child care stat ‘member on-site during all hours of operation who is currently trained in child abuse recognition and prevention, VIOLATION: ‘As documented inthe 03/26/2018, 03/27/2018 and 04/03/2018 complaint inspection, ODJFS staff determined that at least one child care staff member with currently valid training in child abuse recognition and prevention was not present from 5:30 PM to 6:30 PM, 8. As documented in the 03/26/2018, 03/27/2018 and 04/03/2018 complaint inspection, ODJFS staff determined thatthe center's administrator, Althea Caver, bad not completed the ODJFS prescribed one-hour child abuse overview. QAC $101;2-12-18(AY(1) states that the center shall ensure that required child care staff ‘members/child ratios and maximum group size limitations are maintained a allies in accordance with appendix A to this rule, VIOLATION: a. As documented in the 03/23/2017 inspection report, ODJFS staff determined a ratio ‘of one childcare staff member for 15 children inthe preschool and school-aged combined group. The required ratio for this group is 1:12. 'b. As documented in the 03/26/2018, 03/27/2018 and 04/03/2018 complaint inspection, ODIFS staff detemmined & ratio of one child care staf member for 10 children inthe infant, toddler and preschool combined group anda ratio of one childcare stall ‘member for 19 children inthe school-age group. The required ratio for these groups are 1:5 or 2:12 and 1:18, respectively 10, OAC 5101:2-12-18(A)(3) states that even if only one childcare staff member is needed to be inthe balding to meet ratio, there shall be at least one other employee or childcare staff ‘member at the center. VIOLATION: a. As documented in the 03/13/2017 complaint inspection, ODJFS staff determined that the center did not have a second employee or childcare staff member present, as require. '. OAC $101:2-12-19(A}(1)82) states that no child shall be left unsupervised Supervision means child care staff members have knowledge ofa child's needs, sccountability for thei care a all times and that ell children in care ae within sight and hearing of child care sta members at all times. Supervision includes awareness of and responsibilty forthe activity of each child and being physically present and near enough to intervene if needed, Staff must be able to see and hear children without the use of mechanical devices such as baby monitors video cameras or walkie talkies.
