OAC 5101:2-12-04(B)2)(e) states that a fire inspection and approval needs to be obtained at least once annually within each twelve months from the date of the last fre epproval report, VIOLATION: 4. As documented in the 06/22/2017 inspection report, ODIFS staff determined thatthe center had not been inspected and approved within 12 months from the date of the last fire approval, as required. 2. QAC5101:2-12-09(C)2}' states that criminal record checks are required prior to the first, ‘day of employment for employees and childcare staff members and the results shall be reviewed by the administrator prio to the employee or child care staf member being let alone with children, ‘VIOLATION: a.
