Exceptional Characteristics: The student is classified as having an intellectual disability,


namely Down Syndrome.

PART B: PRESENT LEVEL OF EDUCATION PERFORMANCE

I. Basic Cognitive Skills/Daily Living Skills

In regards to basic cognitive skills and daily living skills, the student is making progress
with following her daily routine, as well as, transitioning and using the bathroom
throughout the day. The student is also making progress expressing thoughts and
emotions, as well as, interacting with peers. This student is making progress showing
appropriate behavior.

II. Reading

In regards to reading, the student has begun to memorize fundations chant and
enjoys to share her knowledge of skills with other students. The student loves looking at
book and being read to. The student is able to memorize books and read some words on
pages of highly preferred books. The student is making progress with pointing to words
on the page and using picture clues to help identify unfamiliar words. The student is able
to build words using word families, such as ‘at’, ‘ug’, ‘en’, ‘ig’. The student is able to
successfully identify the beginning letter sound to change words in the word family and
match those words to corresponding pictures. The student is able to identify her sight
words receptively, but still has difficulty identifying words expressively. The student is
able to answer occasional “wh” questions.

III. Mathematics

In regards to mathematics, the student is able to count to 20 independently and is


currently working on counting to 30. Occasionally, the student needs support with
numbers in the teens. The student is making progress identifying her numbers. The
student is able to trace numbers independently.
IV. Writing

In regards to writing, the student can write a shortened version of their name
(nickname). The student can generate ideas write about on a chosen topic and is making
progress with letter-sound correspondence. The student is making progress forming
letters correctly and is bale to draw basic shapes. The students is making progress
copying printed material during writing assignments.

PART C: GOALS AND OBJECTIVES

Goal #1: When given a social or academic situation, the student will not get angry or
aggressive when embarrassed or frustrated.

Goal #2: The student will demonstrate the visual and fine motor skills of prewriting with
minimal assistance from a teacher.

Goal #3: Given a small group structured setting, Scott will transition from one activity to
another quietly (e.g., no yelling, screaming) and with hands and feet to self (i.e., without
kicking, hitting)

Goal #4: When given a grade level text, the student will read and demonstrate
comprehension of grade-appropriate literary texts.
