AFFIDAVIT OF WITNESS
(GLORIA DANO)

I, GLORIA DAÑO, Filipino, of legal age, married and a resident of Maribago,

Lapu-Lapu City, after having been duly sworn to in accordance with law, do hereby
depose and state, THAT:

1. I am presently employed in the Office of the City Mayor of Lapu-Lapu since

2001;
2. In 2004 I was assigned as head of the administrative division of the said
Office;
3. One of our duties in the administrative division is the processing of vouchers
for payments to the City’s suppliers;
4. In 2004, the administrative division processed the voucher for the purchase of
the 470 computer units for distribution to the different public high schools of
the City;
5. As part of the processing of the vouchers, the office will have to scrutinize the
supporting documents such as the ALOBS, Abstract of Bids, Notice of Award,
Purchase Order and Delivery receipts;
6. Once all the supporting documents are in order, the voucher will be
processed to the requisitioning office, to the accounting, to the budget office
and to the treasury office before the same will be returned to the office of the
mayor for the signature of the mayor;
7. The voucher for the computer project was properly processed since the same
contained all the necessary signatures for the drawing of the check for
payment;
8. There was no incidence were payment was made to the supplier without the
completed voucher;
9. I hereby execute the same on my own personal knowledge and information.
