ADDENDUM TO EXTRAJUDICIAL SETTLEMENT OF ESTATE

WITH WAIVER OF RIGHTS

KNOW ALL MEN BY THESE PRESENT:

This Addendum to Extrjudicial Settlement of Estate with Waiver of


Rights is executed by and between:

ROMEO L. ROMBAOA, of legal age, widower, Filipino citizen and a


resident of Barangay Cayanga, Paniqui, Tarlac;

BELINA R. ALMACHAR, of legal age, married, Filipino citizen and a


resident of Barangay Cayanga, Paniqui, Tarlac;

SYLVIA C. ROMBAOA, of legal age, single, Filipino citizen and a


resident of Barangay Cayanga, Paniqui, Tarlac;

ROMEO L. ROMBAOA JR., of legal age, married, Filipino citizen and


a resident of Barangay Cayanga, Paniqui, Tarlac;

ANA LEA C. ROMBAOA, of legal age, single, Filipino citizen and a


resident of Barangay Cayanga, Paniqui, Tarlac; and

ANA LEA C. ROMBAOA, of legal age, single, Filipino citizen and a


resident of Barangay Cayanga, Paniqui, Tarlac; and

EDWARD NIÑO G. ROMBAOA and LORRAINE G. ROMBAOA, both


of legal age, Filipino citizens and residents of Barangay Cayanga, Paniqui,
Tarlac, representing the deceased RODEL C. ROMBAOA;

WITNESSETH:

That the above-named parties are the heirs of the late ANITA CORTEZ
ROMBAOA who died intestate on October 06, 2012, a copy of her Certificate
of Death is hereto-attached and marked as ANNEX “A”;

That we have extra-judicially settled the estate left by the late ANITA
CORTEZ ROMBAOA, who died without any Last Will or Testament, and
without any outstanding debts or accounts on December 11, 2017 before
Notary Public of Atty. Bienvenido M. Perez under Doc. No. 288, Page No.
58, Book No. 32, Series of 2017, is hereto-attached as ANNEX “B”;

That therein what was settled is Transfer Certificate of Title (TCT) No.
220494 and Transfer Certificate of Title (TCT) No. 102525;

That there are still properties left by the above-said deceased, which
particularly described as follows:
TRANSFER CERTIFICATE OF TITLE NO. 319103

“A parcel of land Lot 11 of the consolidation and


subd. plan, Pcs-03-000897, being a portion of
consolidated lots 4397-C and 7397-D (LRC) Psd-1294,
LRC REC. NO.), situated in the Barrio of Apulid,
Municipality of Paniqui, Province of Tarlac, xxx
containing an area of THREE THOUSAND FIVE
HUNDRED FIFTY TWO (3,552) SQUARE METERS,
more or less.

TRANSFER CERTIFICATE OF TITLE NO. 319103

“A parcel of land Lot 4371 of the subdivision


plan, Psd-03-068529, being a portion of consolidated
lots 4371-H LRC Psd-4087, LRC REC. NO.), situated
in the Barrio of Apulid, Municipality of Paniqui, Province
of Tarlac, xxx containing an area of THREE
THOUSAND FIVE HUNDRED FIFTY TWO (12,000)
SQUARE METERS, more or less.

That pursuant to Section 1 of Rule 74 of the Revised Rules of the Court


of the Philippines, we hereby adjudicate the above described properties
equally in our favor.

this page on which this Acknowledgment is written, has been signed by the parties and
their two (2) instrumental witnesses on the left margin of each of the first and third pages
and on the top of each of their typewritten names of the second page.
