2015:
 For Grade 11-12, learners who fail a unit/set of
competencies must be immediately given remedial
classes.
 They should pass the summative assessments during
remediation to avoid a failing grade in a learning
area/subject. This will prevent students from having
back subjects in SHS.
 However, if the learner still fails remedial classes, he/she
must retake the subject/s failed during the summer or
as a back subject.
• Summative Assessments are also given during
remedial classes. These are recorded, computed,
weighted, and transmuted in the same way as the
Quarterly Grade. The equivalent of the Final Grade
for Remedial classes is the REMEDIAL CLASS MARK
(RCM).
• The Final Grade at the end of the school year and
the Remedial Class Mark are averaged. This results
in the RECOMPUTED FINAL GRADE.
•If the Recomputed Final Grade is 75
or higher, the student is promoted to
the next grade level. However,
students will be retained in the grade
level if their Recomputed Final
Grade is below 75.
•The teacher of the remedial class
issues the Certificate of Recomputed
Final Grade which is noted by the
school principal. This is submitted to
the division office and must be
attached to both SF 10-SHS and SF5A-
SHS.

• -Reference guide of SHS subjects, including approved
subject substitutions and other subjects. It is not
necessary to print this and attach it to the form.
• -Advisers can mark the checkboxes of subjects that the
learner has passed. In the electronic copy, the
checkbox is automatically marked when a learner
passed the subject.
• -This can aid in enrolment and in making sure that all
required subjects are passed by the learner.
AFTER ACCOMPLISHING THE
INDIVIDUAL ELECTRONIC

STUDENTS:
For the reading of forms of Grade 11, the electronic copy shall be printed,
to be attached to the other forms: PHOTOCOPY OF SF10-JHS, Birth
Certificate, etc., for validation.

After the reading of forms, the hard copy of the SF10-SHS attached to the
other forms (SF10-JHS, Birth Certificate, etc.) shall be forwarded to the
school registrar. On the other hand, the individual electronic file shall be
saved by the adviser, to be forwarded to the school registrar (saved via
flash drive and/or CD). Optional: Another copy of the file shall be sent to
the school official email account. (FOR BACK UP)

For the next school year, the hard copy of the SF10-SHS with the other
forms (SF10-JHS, Birth Certificate, etc.) and the electronic file shall be
given to the new adviser (Grade 12).

For the reading of forms of Grade 12, the completed and updated hard
copy shall now be attached to the forms.

After the reading of forms of Grade 12, the hard copy of the SF10-SHS
attached to the other forms (SF10-JHS, Birth Certificate, etc.) shall be
forwarded to the school registrar. On the other hand, the individual
electronic file shall be saved by the adviser, to be forwarded to the school
registrar (saved via flush drive and/or CD). Another copy of the file shall
be sent to the school official email account (for BACK CP).
