process improvement.
Root cause analysis is a collective
term that describes a wide range of
approaches, tools, and techniques
used to uncover causes of problems.
Benefits of attending
Root Cause Analysis (RCA) gets to the bottom of an
issue so that action can be taken to fix it
permanently. By systematically peeling away layers
of causes and effects, RCA identifies breakdowns in
processes and systems that contributed to the
event. Done correctly, RCA will improve safety,
reduce future incidents – and ultimately lower your
total cost of risk.
Participant will learn the RCA methodology with
series of group work, role-play and simulation on
real case RCAs.
This session will help the participants to
1. Instigate and determine why an event of
occurred in a given circumstance
2. Collect relevant data required data for the Root
Cause Analysis
3. Organize and analyze the information gathered
during investigation and identify gaps and
deficiencies
4. Map the root cause to identify the underlying
reason for each factor
5. Develop effective counter measure and
recommendations
6. Make Action plan, execute and monitor
progress of implementation
