The Discipline
Office is in charge in monitoring the mobilization and discipline of students and
conducts investigation of students who violates school rules. They formulate and
implement policies, rules, and regulations for student conduct.
In this paper, we will be focusing on the Category 1 offenses that students usually
violate. Category 1 offenses has 10 discipline infractions. First to second offenses will be
warning to students; 3rd offense, informing/summoning parents/guardians; on the 4th
offense the sanction will be 1-3 days suspension; and on the 5th offense will be
automatically considered as a Category 2 offense.
The Discipline Officers have been doing a D.O. policies re-orientation every 1st semester
each year to remind students of the policies and the new offenses if there are any.
However, students still violate these offenses specifically category 1 offenses.
Therefore, we would like to conduct a report on why students keep on violating the said
offenses. The recommendations or project proposal for decreasing the number of
student’s violation will be made by the Discipline Office base on the result of this report.

The purpose of this report is to:

 Know the reasons why students keep on violating category 1 offenses.
 Analyze the students’ insight about the Discipline Office.
 Identify the most frequent offense the students violate.
 Decrease the number of violators in Category 1 offense.
