1.0 Purpose
The purpose of this procedure is to outline the controlled preparation, maintenance, and distribution of
the company's functional organizational chart.

2.0 Scope
This procedure applies to the controlled Functional Organizational Chart of [Company Name], as
prepared by Human Resources.

3.0 Definition Of Terms

3.1 Functional Organizational Chart


A document, consisting of the graphical depiction of the functions of personnel in all company
departments and their lines of direct reporting responsibility, as prepared under the authority of
Administration and as controlled by Human Resources

3.2 Reference Copy


A copy of the Functional Organizational Chart is released for distribution to all Managers for
reference purposes only. In the event of changes to the Functional Organizational Chart, Human
Resources will issue revised charts on the first day of the month proceeding the month in which a
change(s) has occurred.

4.0 Responsibilities and Requirements

4.1 Human Resources


The Human Resources section is responsible for the preparation and maintenance of an accurate
Functional Organizational Chart. A Human Resources section representative confers with
Department Managers regarding any and all changes to departmental structure that alters the
functional responsibilities being performed within their department as depicted on the Functional
Organizational Chart.

4.2 Manager, Administration


The Chief Executive Officer is responsible for the authorization of the controlled Functional
Organizational chart and the distribution of Reference Copies.

6.0 Procedure

6.1 Functional Organizational Chart Maintenance


A Human Resources section representative maintains the controlled Functional Organizational
Chart by updating the chart as changes occur.

6.2 Functional Organizational Chart Revisions


A Human Resources section representative documents all changes to the functional structure
caused through hiring, reorganization, or other means. The Human Resources representative
documents the changes on the controlled Functional Organizational chart the changes and notes
the date the changes become effective. Human Resources notify the QA/RC manager of any
changes as they occur. The Human Resources representative will confirm with the Department
Manager that all documented changes on the Functional Organizational Chart are accurate,
correct any inaccuracies, and prepare a new copy of the chart to be authorized by signature,
marked with an effective date, and assigned a revision number by the Chief Executive Officer on
the last working day of each month in a change has occurred. The new revision becomes the
controlled Functional Organizational chart.

6.3 Reference Copy Distribution


The Chief Executive Officer prepares Reference Copies of the Functional Organizational Chart
reflecting any changes that occur in the functional structure. These Charts are distributed to all
Managers on the first working day of each month following a month in which a change has
occurred. If there are no changes, the previously issued chart is still valid. Human Resources is
responsible for distributing a reference copy of the valid Functional Organizational Chart to new
Managers.

6.3.1 Functional Organizational Chart Control


A Human Resources Representative maintains the revised, controlled Functional
Organizational Chart.
