Penelitian Klinikoepidemiologik pada Tuberkulosis Kutis di Rumah Sakit Pendidikan
Tersier, Andhra Pradesh, India

Abstrak

Latar Belakang: Tuberkulosis kutis merupakan 1,5% dari tuberkulosis ekstra paru dan
penyakit ini terus menjadi salah satu tantangan karena memiliki tampilan klinis yang berbeda.
Penelitian ini dilakukan untuk mendokumentasikan tipe tuberkulosis kutis yang paling
umum, tampilan klinis yang atipikal (bila ada) dan responnya terhadap terapi singkat
(DOTS).

Metode: Semua pasien dengan TB kutis yang diduga secara klinis dan datang ke bagian
rawat jalan RSUP Dr. Dermatologi di rumah sakit kami dari Oktober 2012 hingga April 2016
dilibatkan dalam penelitian ini. Riwayat lengkap penyakit dan pemeriksaan menyeluruh
secara umum, sistemik dan kulit dilakukan bersamaan dengan dokumentasi rincian
demografi. Pemeriksaan darah rutin, tes biopsi dan mantoux juga dilakukan. Kasus yang
didiagnosis dan diobati dengan DOTS.

Hasil: Sebanyak 25 kasus tuberkulosis kutis dimasukkan dalam penelitian. Jenis tuberkulosis
kutis yang paling umum adalah lupus vulgaris. Gambaran klinis atipikal yang dicatat selama
penelitian adalah multifokal lupus vulgaris (LV), ko-eksistensi tubekulosis kutis verukosa
dan LV, Tuberkulosis kutis verukosa pada bibir bawah, erythema induratum of bazin
dengan tampilan plak anular dalam satu kasus dan eritema nodosum pada kasus lain. DOTS
efektif pada sebagian besar pasien.

Kesimpulan: Tuberkulosis kutis memiliki banyak tampilan klinis. Kecurigaan klinis yang
tinggi diperlukan dalam tampilan klinis yang jarang. Ko-eksistensi dari dua atau lebih pola
morfologi dapat terjadi. Kasus yang meragukan, 5-6 minggu diberikan terapi. Dosis yang
adekuat sangat penting untuk respon yang baik. Obat lini kedua harus dipertimbangkan dalam
kasus kegagalan / resistensi klinis.

PENDAHULUAN

Tuberkulosis sudah setua usia manusia yang ada, dengan bukti dari penyakit tersebut
yang telah ditemukan pada mumi Peruvian dan pada kerangka manusia yang berusia 300 SM.
Secara global, 9,6 juta kasus-kasus tuberkulosis baru ditemukan pada tahun 2014 dan 1,3 juta
meninggal.1 Tuberkulosis biasanya dipertimbangkan sebagai penyakit yang mengenai rakyat
dengan kemiskinan dimana 94% kasus kasus yang muncul terjadi di negara-negara dengan
status sosial ekonomi rendah. Meskipun insidennya telah turun hingga 0,1% bahkan di negara
berkembang, penyakit ini tetap terjadi. Salah satu yang luar biasa karena ko-infeksi dari
human immunodeficiency virus (HIV), resistensi obat dan presentasi atipikal.2,3. Penelitian
ini dilakukan untuk mendokumentasikan tipe tuberkulosis kutis yang paling umum, tampilan
klinis yang atipikal jika ada dan respon terhadap terapi singkat (DOTS).

METODE

Semua pasien dengan TB kutis yang diduga secara klinis dan datang ke bagian rawat
jalan RSUP Dr. Dermatologi di rumah sakit kami dari Oktober 2012 hingga April 2016
dilibatkan dalam penelitian ini. Riwayat lengkap penyakit dan pemeriksaan menyeluruh
secara umum, sistemik dan kulit dilakukan bersamaan dengan dokumentasi rincian
demografi.pemeriksaan darah rutin, enzyme linked immunosorbent assay (ELISA) untuk HIV,
Rontgen Toraks, Tes Mantoux dan biopsi dilakukan pada semua kasus. Pemeriksaan sputum
untuk Basil Tahan Asam (BTA), sitologi aspirasi jarum halus kelenjar getah bening dan tes
radiologi lainnya dilakukan dalam kasus yang relevan. Kasus yang terdiagnosis diberi DOTS
untuk jangka waktu 6 bulan dan respon dinilai pada 6 minggu dan pada akhir terapi,jika
terdapat efek samping juga dicatat selama masa pengobatan.

HASIL

Sebanyak 25 kasus tuberkulosis kutis dimasukkan dalam penelitian. Rasio laki-laki


dan perempuan dalam penelitian adalah 1,5: 1. Kelompok usia populasi penelitian berkisar 5-
40 tahun dengan usia rata-rata adalah 25 tahun 6 bulan. Lokasi yang paling sering terlibat
adalah ekstremitas bawah yang terlihat pada 13 kasus [52%]. Jenis klinis yang paling umum
dari tuberkulosis kutis adalah lupus vulgaris yang terlihat pada 11 kasus (44%) dan yang
paling umum adalah erythema induratum of bazin yang terlihat pada 2 kasus (8%), rincian
diberikan dalam Tabel 1.

Dari pasien tb kutis, yang juga terkena HIV ditemukan pada 3 kasus (12%). Presentasi
atipikal yang dicatat selama penelitian adalah multifokal lupus vulgaris (LV), ko-eksistensi
tuberkulosis kutis verukosa dan LV, tuberkulosis kutis verukosa pada bibir bawah, TVC pada
bibir bawah, erythema induratum of bazin (EIB) yang muncul dengan tampilan plak anular
dalam satu kasus dan erythema nodosum dalam kasus lain seperti yang ditunjukkan pada
Gambar 1-5. Mantoux positif dalam 84,2% kasus dan histopatologi khas terlihat pada 90%
kasus. Tabel 2 menunjukkan rincian penyelidikan laboratorium dari populasi penelitian.
DOTS efektif dalam semua kasus kecuali pada 2 (8%) pasien dan truncal acne diamati pada
2 (8%) pasien seperti yang ditunjukkan pada Gambar 6 dan 7.

DISKUSI

Pada penelitian ini, tuberkulosis kutis menunjukkan insiden yang lebih tinggi pada
laki-laki, sama dengan mayoritas hasil studi di India. Hal ini dapat dihubungkan dengan
resiko cedera yang lebih tinggi pada laki-laki, dimana pasien yang dilibatkan dalam
penelitian kali ini berprofesi sebagai pekerja kasar. Sebagian besar pasien berada pada usia
dekade kedua, sama dengan hasil studi di India lainnya. Jenis tuberkulosis kutis yang paling
umum dalam penelitian ini adalah lupus vulgaris [44%], sama dengan beberapa studi lain.
Namun beberapa studi di India lainnya ditemukan scrofuloderma sebagai tipe yang paling
umum. Lihat Tabel 3.
` Pada penelitian ini ditemukan bahwa ekstremitas bawah merupakan lokasi tersering
kejadian TB kutis. Ditemukannya insiden tinggi TB kutis pada ekstremitas bawah di India
bisa dijelaskan dengan re-inokulasi basil tuberkulosis melalui trauma ringan, terutama saat
jongkok.

TB kutis, seperti yang dikutip oleh Pillsbury, Shelly dan Kligman sebagai “di kulit
tuberkulosis muncul dalam berbagai bentuk yang menakjubkan ”. Sejalan dengan itu kami
mengamati berbagai gambaran atipikal selama penelitian ini. Lupus vulgaris multifokal
terlihat pada laki-laki berusia 40 tahun, namun laporan mengenai penyakit multifokal pada
literatur sangat sedikit. Kejadian multifokal paling banyak terjadi pada pasien yang tidak di
vaksinasi dan malnutrisi serta umumnya mantoux test nya negatif. Ada beberapa laporan
tentang ko-eksistensi dari tuberkulosis yang berbeda pada orang yang sama dengan
kombinasi TVC dan scrofuloderma paling sering dilaporkan,termasuk pada penelitian ini.
Studi imunohistologik terhadap granuloma pada TB kutis menunujukkan perubahan spektrum
yang ditunjukkan dengan rasio CD4 +/- CD8, contohnya pada LV dengan imunitas tinggi,
TVC dengan imunitas sedang, dan SFD dengan imunitas rendah. Ada beberapa tipe TB kutis
yang muncul tergantung pada imunitas seseorang. Kemungkinan alasan lainnya adalah TVC
dapat berkembang dari auto-innoculation bacilli dari TB kutis yang berdekatan yaitu SFD.
Pada penelitian kami ditemukan kasus TVC pada lokasi yang tidak umum yaitu di bibir
bawah pada seorang wanita berusia 50 tahun. Beberapa lokasi yang tidak biasa juga
dilaporkan terjadi di jari, di atas keloid, dll.
EIB secara klinis bisa menyerupai eritema nodusum pada suatu kasus. Observasi lain
oleh Maharaja dkk dalam studinya mengenai gambaran kliniskohistopatologis pada nodul
lunak erimatosus umumnya terjadi di ekstremitas, dimana terjadi perubahan histologis pada
eritema nodusum dalam 8/30 kasus dan 3 kasus diantaranya mengarah ke EIB. Presentasi
atipikal EIB lainnya berupa plak anular besar di bagian atas paha pada pasien berumur 25
tahun. Jadi EIB tidak selalu didefinisikan dengan gambaran klasik (nodul ulseratif di tungkai
bawah), namun tetap gambaran histopatologi yang akan membantu menegakkan diagnosis
definitif dan differensial.

` Mantoux test positif terlihat pada 84,2% kasus, hasil ini sebanding dengan studi yang
dilakukan Binod kumar dkk, hasil yang berbeda terlihat dalam penelitian lain, rincian dapat
dilihat pada Tabel 4.

Perubahan histopatologik tipikal ditemukan pada 90% kasus yang juga ditemukan
pada penelitian India lain. Durasi pemberian obat anti tuberkulosis (OAT) untuk tuberkulosis
kutis adalah antara 6-12 bulan pada beberapa penelitian lainnya. Kami mengimplementasikan
terapi OAT yang diberikan selama 6 bulan. Pada penelitian oleh Raghu Rama dkk tentang
terapi DOTS di pasien tuberkulosis kutis, efikasinya hampir sebanding dengan kemoterapi
jangka pendek harian, dengan manfaat lainnya yaitu paparan terhadap obat yang lebih sedikit,
pemberian obat dibawah pengawasan dan pengeluaran untuk transpor menjadi lebih sedikit.
Raghu menemukan bahwa tidak ada kegagalan pengobatan atau efek samping yang
signifikan dengan penggunaan terapi DOTS pada penelitiannya. Namun, pada penelitian ini,
kami menemukan adanya kegagalan pengobatan pada 8% kasus dan ditemukannya efek
samping minor seperti jerawat dipunggung pada 8% kasus, secara detail dapat dilihat di Tabel
5.
Lama percobaan pengobatan di pasien yang dicurigai tuberkulosis kutis adalah selama
5-6 minggu, kecuali pada pasien dengan tuberkulid dan pasien tidak yang memperlihatkan
gejala klinis yang signifikan sebelum pengobatan. Jika setelah jangka waktu tersebut pasien
tidak menunjukkan perubahan signifikan, maka diagnosisnya perlu dikaji ulang kembali.
Namun dengan adanya tuberkulosis yang multi drug resistant (MDR), pendekatan ini tidak
dibernarkan. Tuberkulosis kutis yang MDR harus selalu dipikirkan pada pengobatan pasien
yang tidak memperihatkan respon signfikan setelah pemberian OAT lini pertama atau pada
pasien yang klinisnya memburuk bahkan setelah pemberian OAT. Diagnosis definitif dari
tuberkulosis MDR sulit dilakukan akibat sukarnya isolasi dan tes diagnositik molekuler yang
sensitivitasnya masih rendah. Sehingga, selalu dibenarkan untuk dilakukannya percobaan
pemberian OAT lini kedua sebelum menganggap pasien tidak responsif terhadap terapi. Kami
memiliki satu pasien laki-laki berumur 40 tahun yang bertubuh sehat (tinggi 6 kaki, berat 120
kg) yang dikonfirmasi dengan biopsi menderita skrofuloderma, yang tidak responsif dengan
pemberian OAT rutin bahkan setelah 5 minggu terapi, sembari berpikir mengenai
kemungkinan resistensi obat sebelum memulai pengobatan OAT line kedua, kami
memberikan OAT lini pertama yang disesuaikan dengan berat badannya. Dalam 2 minggu
setelah penambahan dosis, pasien langsung memberikan respon, seperti yang dijabarkan di
gambar 8. Oleh karena ittu, pendosisan obat yang adekuat, terutama berdasarkan berat badan
pasien, juga sangat penting sebelum memikirkan kemungkinan diagnosis lain atau adanya
resistensi obat.
Kesimpulanya, tuberkulosis kutis memiliki berbagai sisi. Kecurigaan klinis yang
tinggi dibutuhkan pada presentasi yang jarang ditemukan. Pola morfologis yang tumpang
tindih atau lebih dari 2 dapat terjadi. Pada kasus yang meragukan, percobaan terapi selama 5-
6 mingggu sangatlah mebantu. Dosis adekuat juga diperlukann untuk didapatkannya respon
yang baik. OAT lini kedua dapat dipertimbangkan pada kasus yang mengalami kegagalan
pengobatan atau resistensi obat.

A clinicoepidemiological study of cutaneous tuberculosis in a tertiary


care teaching hospital in Andhra Pradesh, India

ABSTRACT

Background: Cutaneous tuberculosis constitutes 1.5% of extra pulmonary tuberculosis and the disease continues to
be a challenging one because of its multifaceted presentation. The present study was done to document the most
common type of cutaneous tuberculosis, atypical presentations if any and response to directly observed therapy short
course (DOTS).
Methods: All patients with clinically suspected cutaneous tuberculosis attending outpatient department of
dermatology in our hospital from October 2012 to April 2016 were included in the study. A detailed history of
presenting illness and thorough general, systemic and cutaneous examination was carried out along with
documentation of demographic details. Routine blood invetigations, biopsy and mantoux test were done. Diagnosed
cases were treated with DOTS.
Results: A total of 25 cases of cutaneous tuberculosis were included in the study. Most common type of cutaneous
tuberculosis was lupus vulgaris. Atypical presentations noted during the study were multifocal lupus vulgaris (LV),
co-existence of tuberculosis verrucosa cutis (TVC) and LV, TVC of lower lip, erythema induratum of bazin
presenting as annular plaque in one case and as erythema nodosum in another case. DOTS were effective in majority
of the patients.
Conclusions: Cutaneous tuberculosis is multifaceted. High clinical suspicion is necessary in rare presentations.
Coexistence of two or more morphological patterns can occur. Doubtful cases, 5-6weeks of therapeutic trail helps.
Adequate dose is essential for good response. Second line drugs are to be considered in case of failure /clinical
resistance.

RESULTS

A total of 25 cases of cutaneous tuberculosis were


included in the study. Male to female ratio in our study
was 1.5:1. Age group of the study population ranged
from 5-40 years with the mean age being 25 years 6
months. Most commonly involved site was the lower
limb which was seen in 13 cases [52%]. The most
common clinical type of cutaneous tuberculosis was
lupus vulgaris seen in 11 cases (44%) and the least
common was erythema induratum of bazin seen in 2

DISCUSSION

Cutaneous tuberculosis showed a higher incidence in men


in our study similar to majority of the Indian studies.4-6
This may be attributed to higher risk of sustaining injury
in men, as many of our patients were involved in heavy
manual work. Most of the patients were in their second
decade, similar to that observed in other Indian studies.6,7
The most common clinical type of cutaneous tuberculosis
in our study was lupus vulgaris [44%], this was similar to
few studies.5,6,8 However few other Indian studies found
scrofuloderma as the most common type and Table 3
shows the details.7,9,10

it was also noticed by few authors.6,8 High incidence of
cutaneous tuberculosis on the legs in Indians can be
explained by the re-inoculation of tuberculosis bacilli
through minor trauma, especially during squatting.

Cutaneous tuberculosis was earlier quoted by Pillsbury,


Shelly and Kligman as “in the skin tuberculosis presents
itself in an astonishing variety of forms”.11

was seen in a 40 year old male, there are very few reports
of multifocal disease in the literature.12,13 Multifocal

test. There are few reports of co-existence of different
tuberculosis in the same person with combination of TVC
and scrofuloderma [SFD] being most commonly reported
including ours.14,15 Immunohistological study of
granuloma in cutaneous tuberculosis have shown a
spectrum of changes as evidenced by CD4±CD8 ratio, i.e
LV with strong immunity, TVC with intermediate
immunity and SFD with low-level immunity.16
Depending up on the level of immunity in a person over a
period of time, may be different types of cutaneous
tuberculosis are expressed. Another probable reason is,
TVC may develop from auto-innoculation of bacilli from
adjacent cutaneous tuberculosis that is SFD. We found
