BIRTH HISTORY

Prenatal History
Age of mother during pregnancy was 26. During pregnancy, mother was neither
hypertensive nor diabetic. There were no bleeding or spotting and other illnesses noted.
However, She was diagnosed with urinary tract infection halfway through the 6th month of
pregnancy based on routine urine test and was treated with antibiotics. The mother did not
smoke, drink, or use illicit drugs during the pregnancy.

Natal History
Patient was born at full term from a G2P1 mother through spontaneous vaginal
delivery without any complication at a clinic in Macabebe. Delivery was performed by a
midwife. Labor took about 5 hours. The patient weighted 3.5kg at birth.

Neonatal history:
The mother cannot recall the patient’s APGAR score but the mother relayed that she
has good cry, resuscitative effort was not required, and no cyanosis, jaundice, or pallor
noted. Both the mother and the patient were discharged 24 hours after the delivery with no
complications.

Feeding history
The patient was breast fed until 2 years old and formula milk was introduced
thereafter, upon cessation of breast feeding. Vegetable and fruit puree was introduced at 5
months and solid food at 1 year and 3 months. Her diet consists of the usual adult food such
as rice, chicken, vegetables, and fruits. The patient was particularly fond of eating junk food
and carbonated sweet drinks. The mother claims that her child has a good appetite, with no
food intolerance.
