This is to certify that Mumtaz Ahmad S/o MD Basir was working in the physiotherapy Clinic
from 2nd May 2014 to 30th June 2015.

During this period he gained experience to evaluate, assess, plan and execute physiotherapeutic
modalities to patients (adult and pediatric) suffering from orthopedic, neurological, medical and surgical
problems. Present in the meeting during clinical discussions with special cases. Participated and
managing the camp during world physiotherapy day in the clinic. He was paid Rs. 10,000/per month for
his duties.

He is an agile, dynamic and enthusiastic worker. His humane approach towards patients helped him in
developing an excellent rapport with the patient.
