TITLE PAGE
The title page is the first page of the student portfolio. It includes the title, the
submissionstatement, grade level, academic track and strand, full name of the author and the
month and yearof expected graduation.

Letter to the Readers
It is a one-page letter addressed to the readers informing them of what to expect from the portfolio.
The name of the author should be printed in capital on the right side of the page fourspaces after
the last line of the last paragraph.
Acknowledgement
The acknowledgement is a one-page expression of appreciation for the assistance given tothe
student in the course of her / his work immersion. The name of the author should be printed
incapital on the right side of the page four spaces after the last line of the last paragraph.
The table of contents lists the chapter titles, the first and second level headings and the pages
where they are located. The titles are placed near the left margin and the page number nearthe
right margin. The title and page numbers are separated by light dot lines. Example format

Introduction
The introduction contains a brief background regarding the work immersion in the K
to 12Curriculum and its importance, application or impact to the grade 12 students.1.
The student will identify the office to where he / she was assigned to be deployed.
b. Describe the organizational structure of the office to where the student was assigned for her / his
work immersion
