JUDICIAL AFFIDAVIT

I, APRIL JEWEL RAÑA, of legal age, Filipino, single, Branch Head of

Metrobank Bacolod Capitol Branch, and a resident of Bacolod City,
Philippines, after having been sworn in accordance with law, hereby depose
and state that:

I am submitting this Judicial Affidavit as a WITNESS for the Plaintiff in

the case entitled Metropolitan Bank & Trust Co., Plaintiff versus JJM
Corporation, et. al., Defendants, before the Regional Trial Court, Branch 42,
Bacolod City, and pursuant to A.M. No. 12-8-8-SC (Judicial Affidavit Rule) of
the Supreme Court.

The name of the lawyer who conducted the or supervised my

examination is Atty. Pacifico M. Maghari III, with office address located at
590 Ylac St., Villamonte, Bacolod City. Such examination was conducted in
the same office address on 1 August, 2013.

The purpose of my testimony is to prove that I have been employed

with Metrobank for the last 13 years and that I personally know Ma. Regina
L. Cordova who was the former Branch Head of Metrobank Bacolod 6 th St.
Branch as well as Atty. Cynthia G. Ruiz, one of the legal officers of the
Metrobank Head Office in Makati City and that Ms. Cordova was duly
authorized to file the instant case for collection of sum of money against
defendants JJM Corporation, Claudia Jalandoni, Andres Jamora and Jennifer
Jalandoni as evidenced by a Special Power of Attorney which I will identify.

I answered the questions asked herein fully conscious that I do so

under oath and that I may face criminal liability for false testimony or
perjury, to wit:

Q1: Ms. Witness, what is your relation to the plaintiff Metrobank?

A1: Metrobank has been my employer for the last 13 years and I am
currently the Branch Head of Metrobank Bacolod Capitol Branch.

Q2: Do you know the defendants in this case?

A2: Yes, defendants JJM Corporation, Claudia Jalandoni, Andres Jamora

and Jennifer Jalandoni secured a loan from Solidbank which they were
unable to settle and thus, this present case for collection of sum of money
was filed against them before this Honorable Court.

Q3: You said the defendants secured a loan from Solidbank, what is the
relation of said bank to Metrobank your employer?

A3: Solidbank was acquired by Metrobank sometime in the year 2000

and thus, all business affairs of Solidbank were subsequently continued by
Metrobank.

Q4: You said a case for collection of sum of money was filed against JJM
Corporation, Claudia Jalandoni, Andres Jamora and Jennifer Jalandoni, who
specifically filed this case?

A4: Upon instructions of the Metrobank Head Office, our legal counsel
Atty. King and associates filed the case against said defendants and the
complaint was duly verified by Ma. Regina L. Cordova, Branch Head of
Metrobank Bacolod 6th Street Branch by virtue of a Special Power of
Attorney duly notarized by Atty. Cynthia G. Ruiz, one of the legal officers of
Metrobank Head Office.

Q5: Showing to you a Special Power of Attorney dated September 6, 2002

signed by a certain Rufo Venus, Jr. and notarized Atty. Cynthia Ruiz, Notary
Public are you familiar with this document?

A5: Yes, this is the Special Power of Attorney I am referring to which our
legal counsel used in order to file the case against the herein defendants
which complaint for sum of money was duly verified by Ma. Regina L.
Cordova.

Q6: The identified Special Power of Attorney is hereby marked as Exhibit

“G” forming integral part of your judicial affidavit. Do you know Ma. Regina
L. Cordova?

A6: Yes, she was my colleague in Metrobank Bacolod Area for several
years and at the time she was Branch Head of Metrobank Bacolod 6 th
Branch and I was the Branch Head of Metrobank Bacolod Capitol Branch.

Q7: How about Atty. Cynthia Ruiz, the Notary Public who notarized the
Special Power of Attorney you have identified?

A7: Yes, she is one of the legal officers of Metrobank Head Office and I
had numerous transactions in my branch which she was the one who either
sent my correspondence or notarized the documents itself coming from
head office, thus I am familiar with her as well as her signature which is the
same signature found on the Special Power of Attorney I have identified and
which she likewise certified as a true copy thereof.

Q8: What is now the status of the loan of herein defendants?

A8: The case records of this loan are already under my branch and as per
record, said loan remains unsettled as of to date.

I am the legal counsel who conducted the foregoing examination of
witness APRIL JEWEL RAÑA as contained in this Judicial Affidavit;
