ACCOMPLISHMENT REPORT IN SCIENCE AND HEALTH


S.Y. 2018-2019

I. INTRODUCTION:
Education is a process, beginning at birth, of developing intellectual capacity,
manual skill and social awareness, especially by interaction. The term refers to the
process of imparting literacy and generally accepted body of knowledge.

Accomplishment report designed for the assessment of the achievement done by


the teacher. This report provides the activities on how the teacher accomplishes the
tasks of developing and achieving their goals within the school year. This report is
suggested because it recognizes the creativity and responsibilities of the teacher in
performing their duties in the best way as a teacher can.

II. GOALS AND OBJECTIVES:


Educations goal is to provide knowledge in the development of young children, to
help achieve a high standard of morality and acquire broad areas of high education so as
to become
productive human, and being ready to respond to the needs of a country.

III. ACHIEVEMENT/ACCOMPLISHMENTS
A. Pupil Development:
1. Administered Diagnostic Tests, Chapter tests and Periodical Tests to
evaluate the learning proficiency of pupils.
2. Participated in Science Club/YES-O
3. Registered 10% of the pupil’s enrollment in the District Science Club/YES-
O
4. 100% of the pupils participated in School Science Month Celebration on
September 2015.
5. Participated in the District Science Fair- September 26, 2018

D. Physical Facilities:
1. Improved Physical Facilities for effective Science instructions provided a
project
display in every classroom.
2. Updated bulletin boards which are congruent to the lessons in Science.
3. Maintains the cleanliness and beautification of the Science Park.

V. PROBLEMS MET:
1. Poor study habits of the pupils
2. Failure of pupils to do homework and projects.
3. Some pupils often come to school unprepared for the lessons.

VI. SOLUTION AND RECOMMENDATION:


Parents should be informed about the study habits of their children and
encourage them to give priority
to their children’s welfare.

VII. CONCLUDING STATEMENT:


A school that aims high should take all the risks and difficulty to meet its goal.
Teachers are like cows. When
a cow is well nourished, it becomes a good milking cow that gives out the best milk.
When a cow is poorly nourished
it becomes dry and milkless. As heads of institution play the most significant role of
nourishing their teachers with
positive values, inspiring works/activities worth emulating and the best qualities of
leaders. Teacher do what their
heads do as far the attainment of a good, the head and the subordinates must work
hand in hand to progress.
