06. Soft Copy of the complete instruction is available in `OFB COMNET’. Hard copy of the
instruction is being made and will be circulated to all concerned in due course of time. In the
meantime, hard copies of the whole document may be made at the units and availability of the
same may be ensured at all relevant places/offices. The contents of this letter may be brought to
the notice of Local Accounts Office and all other establishments, related with OFMR, in the same
station.

Sir,
02. The Ordnance Factory Medical Regulations (OFMR) will be applicable to all
the existing Ordnance and Ordnance Equipment Factories as well as factories/units to
be established in future. Wherever, in the OFMR, it is mentioned that Ordnance
Factory Board will decide/issue instructions/guidelines, it implies that the instructions/
guidelines will have to be issued with the approval of the Ordnance Factory Board and
such instructions/guidelines should be in conformity with the requirement of the
Organization and keeping pace with the changing scenario.
03. These orders will take effect from the date of circulation of this letter by the
Ordnance Factory Board.
04. The employees who are desirous of exercising fresh option for inclusion into
OFMR or exclusion from OFMR may be allowed to do so within a period of three
months from the date of circulation of this letter by the Ordnance Factory Board
(Chapter-I, Para-3 (i & ii refer).
This issues with the concurrence of Ministry of Defence (Finance) vide their U. O.
No. 981/IF/DP.I/2008 dated 25-9-2008.

CHAPTER II: OCCUPATIONAL HEALTH SERVICES
They will receive occupation related health care as per the provisions
for health care to entitled employee under OFMR.
For illness and injuries related to their Factory occupation, apart from
provisions under health care of entitled employees, the employee may
also be referred to any private Hospital recognised under either CGHS
or CS (MA) Rules for treatment, if the Medical Officer-in-charge of the
Factory Hospital deems such referral to be essential in the best
interest of the patient’s health. Such referrals to private Hospitals
outside the state will require the sanction of Director of Health
Services, Ordnance Factory Board, on the recommendation of Medical
Officer-in-charge of the Factory Hospital.

The entitled employees and their entitled family members will be given
treatment for special diseases like Cancer, Diabetes, Poliomyelitis,
Cerebral Palsy & Spastics, Tubercular Diseases (Indoor treatment),
mental diseases, Leprosy, Thalassaemia Major etc. to the extent
facilities are available at the Factory Hospital.

CHAPTER IV: MEDICAL FACILITIES FOR PERSONNEL OTHER
THAN ENTITLED EMPLOYEES AND THEIR ENTITLED FAMILY
MEMBERS

In case of admission, Hospital stoppage charges will be levied as for
entitled employees. If they cannot be treated in the Factory Hospital
they may be transferred to the nearest Military Hospital or admitted to
Civil Hospital/private Hospital in accordance with the provisions of
Regulations for Medical Services of Armed Forces.

In case of injuries related to their work at the Factory, they will be
entitled to free treatment at the Factory Hospital/Health Clinic to the
extent facilities are available.

CHAPTER V: MISCELLANEOUS

For non-entitled patients:

C. Breast-fed babies admitted to Hospitals as patients for treatment
will be charged Hospital stoppages at rates prescribed for children
below 12 years. However, when a breast-fed baby, who can not be
separated from the mother, has to be admitted along with the sick
mother, for the latter’s treatment only no Hospital stoppage charge
will be recovered.

2. 20% of the Hospital stoppages will reckoned as diet charges for the
purpose of reimbursement.

CHAPTER VI: HOSPITAL STORES, EQUIPMENT AND DIET
The Hospitals and clinics (both Factory Health Clinic and Estate
Health Clinic) are to be equipped as per the norms and scale of
authorisation decided and finalised by the Ordnance Factory Board
from time to time. Such norm and scale of authorisation will be
subject to revision periodically, to enable the Hospitals/clinics cope up
with the changing requirements.
The Hospitals and clinics (both Factory Health Clinic and Estate
Health Clinic) are authorised to procure their requirement of Medical
stores & equipments including hygiene chemicals as per norms, scale
of authorisation, arrangement and guidelines as decided and finalised
by the Ordnance Factory Board from time to time. Such norm and
scale of authorisation will be subject to revision periodically, to enable
the Hospitals/clinics cope up with the changing requirements.

4. Procurement of instruments, equipments, appliances etc. not covered
under the guidelines:
The Hospitals are fully dieted Hospital and all persons who are
entitled for admission will be provided with Hospital diets. Only
persons admitted to Hospital are entitled to Hospital diets. These diets
will be issued until the person is discharged.

EXPLANATORY INSTRUCTIONS ON ORDNANCE FACTORY MEDICAL REGULATIONS:
Apart from what has been stipulated in the Regulations, following are clarified:
1. To avail health care from OF Hospitals and health clinics under various provisions of OFMR, employees of
Ordnance Factories, allied establishments and other establishments and their dependent family members
will have to produce proof of identity and proof of entitlement.
2. All the entitled employees and entitled family members will be issued proof of identity and entitlement for
obtaining treatment from the Hospital such as:
1) Individual photo identity card
2) Individual Health Record Book with Photo
3) Any other document as decided to be acceptable by the treating Authority
3. In the absence of such identity data of employee, online information available from administration section
of Factory may be used for this purpose.
4. When a patient already admitted to OF Hospital becomes non-entitled under OFMR due to transfer to
another station, superannuation from service, dismissal, compulsory or voluntary retirement of the
employee, 3 months period completed after death of employee, the patient will be treated as non-entitled
patient on payment of requisite Hospital Stoppage Charges, from the date he/she becomes non-entitled.
B. Chapter-I (3) (ii) (A): The civilian employees working in Ordnance and Ordnance Equipment Factories,
their Allied Establishments residing within a radius of 8 Kilometres from the main gate of the Factory
which is to be reckoned by normal route by road, and their families will be governed under Ordnance
Factory Medical Regulations (OFMR) for the purpose of health care.
D. Chapter-II: For illness and injuries related to their Factory occupation, apart from provisions under
health care of entitled employees, the employee may also be referred to any private Hospital recognised
under either CGHS or CS (MA) Rules for treatment, if the Medical Officer-in-charge of the Factory
Hospital deems such referral to be essential in the best interest of the patient’s health. Such referrals to
private Hospitals outside the state will require the sanction of Director of Health Services, Ordnance
Factory Board, on the recommendation of Medical Officer-in-charge of the Factory Hospital.
E. Chapter-III (7): Entitled employees and their entitled family members, when the required facilities for
their health care are not available at Hospitals and clinics, will obtain such facilities under CS (MA) Rules
on the advice of MO attending the patient. In such cases provisions/orders under CS (MA) Rules will be
applicable to them. The Medical Officer-in-charge of the Hospital, may, at his discretion refer the patient
to any other Factory Hospital with requisite facilities for that particular treatment. In addition, under the
above circumstances, entitled employees will also have the choice to avail healthcare from nearest Service
Hospital on payment of necessary charges.
In order to alleviate the inconveniences of the Employees in incurring expenditure towards treatment, individual Units
may decide to institute a system to give an undertaking to those Hospitals in the same station, treatment at which are
ordinarily permitted by the HOD/HOO, to pay the charges as per the rates fixed by the Govt. under the relevant Rules or
the actual expenditure incurred, whichever is less to the Private Recognised Hospital. The specific treatment/ diagnostic
facility permitted, specified charges for the same should also be indicated in such case. Finance/LAO should invariably
be consulted prior to furnishing such undertaking. It should be made clear that any expenditure beyond the rates fixed
by the Govt. under the relevant Rules or any treatment given other than the one for which undertaking was given will be
borne by the patient. Where in emergency etc specific treatment cannot be determined such undertaking be given based
on package based estimate or item-wise estimate provided by the Hospital.

b. Referral to nearest Military Hospital: Such referral can be made only on the basis of a request from the
patient subject to the condition that:
i. The expenditure incurred shall be borne by the patient and can be claimed for reimbursement under
CS (MA) Rules.
ii. Hospital Stoppage charges payable in case of indoor admission shall be admissible for reimbursement
and 20% of such charges will be reckoned as diet charges.
iii. TA/DA shall be governed under the provisions of OFMR (17)
c. Referral to another OF Hospital: Such referral to OF Hospital either in the same or another station can
be made by the MO I/C Hospital only for specific facility in the Hospital subject to the condition that:
i. Requisite facility is available in that OF Hospital
ii. The receiving Hospital should confirm availability of facility, suitability of patient to undergo the
procedure, appointment for the procedure etc before the patient is referred.
iii. Such referral to OF Hospitals outside the State will require sanction of DHS/OFB, preferably before
the referral is made. However, in exceptional circumstances, post-facto sanction may be sought after
clearly indicating the reason for not seeking prior sanction.
iv. TA/DA shall be governed under the provisions of OFMR (17)
4. Validity of Non-availability Certificate/referral:
i. The period of validity of Non-availability certificate/referral varies due to different circumstances of
non-availability of required facility and need to be stated clearly. Few circumstances are given below
for compliance:
ii. If the patient has not used Non-availability certificate/referral within the period of validity but still
needs the facility which is not available in the Ordnance Factory Hospital will be given fresh Non-
availability certificate/referral or the same Non-availability certificate/referral can be revalidated for a
further period.
iii. The period of validity of Non-availability certificate/referral is further subject to known events which
would render the patient non-entitled such as superannuation.
iv. A system to indicate the validity period in the referral may be instituted by each OF Hospital.
F. Chapter-III (8): During leave or temporary duty at an outstation the employees will obtain health care
from Factory Hospital/Health Clinics in the outstation. In case there is no Health Clinic/Factory Hospital
in the outstation or where Health Clinic/Factory Hospital exists but is situated beyond 8 kilometers from
the place of stay of the employee at the outstation, CS (MA) Rules will be applicable to them.

G. Chapter-III (9): The family members of an entitled employee, who are staying at a place other than the
duty station of the employee for whatsoever reason, either temporarily or permanently, and who fulfill the
conditions for ‘family’ under CS (MA) Rules while staying at a place other than the duty station of the
employee, will receive health care from Estate Health Clinic /Factory Hospital in the station. In case there
is no Estate Health Clinic or Factory Hospital or where Estate Health Clinic/Factory Hospital exists but is
situated beyond 8 kilometers from the place of stay at outstation, CS (MA) Rules will be applicable to them
for obtaining healthcare at outstation.
H. Chapter-III (10): When treatment obtained by entitled employees and their entitled family members in
case of Medical emergencies is not authorised under OFMR, CS (MA) Rules will be applicable to them for
obtaining reimbursement of the expenses towards such emergency treatment.

consciousness, convulsive feature, Urine output, feature of shock, Extent of external wound, Extent of active
bleeding, Extent of Chest pain or pain in other parts of the body etc.)

I. Chapter-III (13): Employees and entitled family members governed under OFMR, desiring to obtain
health care under indigenous system of medicine, may be permitted to obtain the same as per CS (MA)
Rules. Permission from the Medical Officer-in-charge of the Ordnance Factory Hospital has to be obtained
so that treatment for the same ailment is not taken simultaneously in more than one system of medicine.
J. Chapter-III (14): The Medical Officer-in-charge of the Factory Hospital may call a local qualified
Specialist Private Medical Practioner or a Central/State Govt. Specialist Medical Officer to attend out-
patients and in-patients in the Factory Hospital. The consultation fee for out-patient and in-patient
consultation will be paid at the rate fixed by the Govt. for such purpose from time to time.
5. Engagement of more than one Specialist in one discipline for periodical out-patient consultation is not
ordinarily permitted. Such engagement shall require approval of DHS which shall be accorded on the merit
of the case and on the basis of information/justification furnished by the OF Hospital in favour of utilising
the service of more than one Specialist in one discipline.
6. For in-door consultation/procedure/treatment, services of Specialist of any discipline may be utilised
according to necessity.
7. Inclusion of other disciplines shall require approval of DHS which shall be accorded on the merit of the
case and on the basis of information/justification furnished by the OF Hospital in favour of utilising the
service of Specialists in other disciplines.

8. Engagement of more than one Specialist in one discipline is permitted, even if a Specialist of the same
discipline is available in the Hospital, if the MO I/C is convinced about such necessity and such
engagement can be sufficiently justified.
K. Chapter-IV (18): Civilian employees working in Ordnance and Ordnance Equipment Factories, their
Allied Establishments who are governed under CS (MA) Rules, may be given treatment from the Factory
Hospital and Clinic as out patient for minor ailments while on duty. They may also be admitted to Factory
Hospital as non-entitled patients for emergencies while on duty and other illness. Such admissions will be
regulated in the same manner and subject to the same conditions as for other non-entitled personnel.
L. Chapter-IV (21): Treatment of employees of Defence Accounts Department & Kendriya Vidyalaya
Sangathan: Those employees of the above establishments residing within a distance of 8 kilometers of the
Factory main gate and their entitled family members are also entitled for out-patient health care from
Factory Hospitals and Health Clinics.
M. Chapter-IV (22): Contract Labourers: The labourers employed at the Factory by the contractors, may
be given treatment in case of emergencies and minor ailments as out-patients as given to the non-entitled
persons, at the Factory Hospital/ Health Clinics to the extent the facilities are available. In case of injuries
related to their work at the Factory, they will be entitled to free treatment at the Factory Hospital/Health
Clinic to the extent facilities are available.

3. Contractors and their staff are not entitled to free Medical attendance and treatment facilities. They and
their family members may be treated in Hospitals and health clinics as non-entitled patients and charged
accordingly.
4. In case of injuries related to their work at the Factory, the facilities would include free Ambulance transport
to any other Hospital at a reasonable distance chosen by the contractor or the patient in consultation with
the Hospital.
N. Chapter-IV (23): Apprentices: The apprentices undergoing training at the Factory, during their training
period, will be entitled for treatment as given to industrial employees according to the provisions of the
scheme for training of apprentices in Ordnance and Ordnance Equipment Factories under the apprentices
Act, 1961 as amended from time to time. For injuries arising out of and in the course of training as an
apprentice, the apprentice will be entitled to healthcare as given to the employees in such conditions.
O. Chapter-IV (24): Medical Officer-in-charge of the Factory Hospital may permit admission of non-entitled
persons to the Hospital, subject to availability of beds and payment of the prescribed Hospital stoppages.
Medical facilities will be provided only to the extent available in the Hospital. Charges of facilities availed
from other sources in the absence of requisite facilities in the Factory Hospital, including purchase of
drugs normally not available in the Hospital will be borne by the patient. If the non-entitled person
admitted to Factory Hospital as above is a non-entitled relative of an entitled employee, the Hospital
stoppage charges will be borne by the employee. In the other cases, Hospital stoppage charges will be
recovered direct from the non-entitled patient or the person on whom the patient is dependent.

P. Chapter-V (25): The Factories will ensure round the clock availability of Ambulance Vehicles at the
Hospitals and Health Clinics as per statutory requirement under the Factories Act as well as functional
requirement of the Factory/Hospital.
Q. Chapter-V (26) (A) for entitled patients: Whenever the patient’s condition warrants provision of
Ambulance Vehicle in the interest of the health of the patient, for journey to and/ or from Factory Hospital
or any Hospital/Diagnostic Centre authorised for the treatment of the patient, Ambulance Vehicle will be
provided. Under the above circumstances, if the Ambulance Vehicle cannot be provided for any reason and
a certificate to that effect is issued by MO attending the patient, Ambulance charges incurred would be
reimbursed to the extent admissible under CS (MA) Rules.
R. Chapter-V (26) (B) for entitled patients: Movement of Ambulance Vehicle is not usually permitted
outside the limits of the Station/City/Municipality. Such movement may be permitted in exceptional
circumstances when the required facilities are not available in the station and there is no other suitable
mode of transport that can transfer the patient to the destination Hospital with required facilities
without endangering the life of the patient or aggravating the health of the patient. Such movement will
be subject to the approval of the Medical Officer-in-charge of the Hospital.
T. Chapter-V (26) (B) for non-entitled patients: In other circumstances, use of Ambulance Vehicle within
the limits of the station/City/Municipality can be provided to a non-entitled person subject to the
availability of Ambulance Vehicle and approval of the Medical Officer-in-charge. Such use of Ambulance
Vehicles should not be permitted compromising the availability of Ambulance Vehicle for the entitled
patients. Ambulance Charges at the rate of Rs 8/- per kilometre should be recovered from the patient.
U. Chapter-V (26) for Paramedical Personnel: Paramedical personnel is permitted to use the Ambulance
Vehicles for the purpose of attending emergent cases during off hours, for journey from their residence to
Hospital and vice versa, when ordered by MO attending the patient.
V. Chapter-V (27) (A): Hospital stoppage charges will be recovered at prescribed rates in respect of patients
admitted to Factory Hospital.
iii. At the time of discharge, dues, if any, would be collected and refunds, if any, would be made.
iv. For unplanned discharges/transfers, the refunds, if any, would be made on the next working day.

2. Collection of payments by Hospital:
a. The MO I/C shall be responsible for instituting a system and detail necessary manpower for collection
of cash.
b. Hospital office will be responsible for collecting all cash payments made in Hospital including HSR of
in-patients, out patients, any other charges.
c. Arrangement for collection of cash outside working hours of the Hospital office will also be made by
the MO I/C Hospital.
d. Payment can be received only in cash unless specified otherwise. Payment by any other means will not
be accepted.
e. The collected cash shall be dealt as per the provisions of cash handling instruction and relevant
guidelines issued from time to time.
f. The revenue so collected shall be deposited in the consolidated fund of the Govt. of India.
3. Accounting & Audit: Introduction of above procedures will lead to generation of substantial cash
transactions which will be dealt with by the Of Hospitals. In this context, following procedures are
enunciated:
a. All cash receipts in connection with other transactions will be dealt by OF Hospitals. MO I/C will be
responsible for collection of cash and proper accounting thereof. For managing the cash handling at the
OF Hospitals, MO I/C may detail suitable staff at hospital offices. In this regard, the under mentioned
instruction shall be followed:
1) For this purpose, hospital offices will maintain a Cash book in appropriate format. At the end of
each working day balance held in Hospital cash book will be denuded by transferring the sum to
the Main Cash Book maintained at Factory Cash Office.
2) Cash receipts generated by OF Hospitals shall be credited to code head Misc. Receipts–802/01,
UCC–‘08’.
3) In respect of advances accepted by the Hospital, there may be cases of refund. In all such cases,
based on appropriate clearance issued by MO I/C Hospital, claims for refund will be admitted by
the Factory Cash office.
b. At the end of each month, MO I/C shall personally verify the entries in the Hospital Cash Book and
satisfy himself that all the sums received during the month have appropriately been credited to the
Consolidated Fund of India. MO I/C will also record the result of such inspection.
c. The above documents will be brought under the audit ambit of Local Audit. The criteria prescribed for
audit of cash books shall apply in respect of these documents, viz. one month detailed audit, half yearly
etc.
d. The whole system may be instituted in active consultation and co-ordination with the cash office of the
connected Factory.
W. Chapter-V (27) (E): In case of admission for accepting small family norms like vasectomy/tubectomy/
IUCD and management of any post acceptance complications, as a part of National Health Programme, no
Hospital stoppage will be recovered.
X. Chapter-V (28): The recovery rates in respect of Hospital Stoppage, Ambulance Charges etc. shall be
revised periodically, preferably each year, through a duly constituted Board of Officers including a
Finance representative.
The rate of Hospital stoppage for the above categories shall be as under, subject to other terms and
conditions mentioned in OFMR:
Gazetted Officers Rs.10/- per day
Employees other than Gazetted Officers Rs.5/- per day
b) Non-entitled patients:
10. Category-I:
i. Family members of employees who are staying at a place other than the duty station of the
employee (9)
ii. Employees working in Ordnance Factories/Allied Establishments dependent on the Hospital but
governed under CS (MA) Rules (18)

iii. Employees of Defence Accounts Department and Kendriya Vidyalalya Sanghatan attached to
Ordnance Factories & Allied Establishments dependent on the Hospital and their entitled family
members. (21)
The rate of Hospital stoppage for the persons mentioned under Category-I shall be as under, subject
to other terms and conditions mentioned in OFMR:
11. Category-II:
i. Contract Labourers, not their family members, working in Ordnance Factories/ Allied
Establishments dependent on the Hospital for illness/injuries other than for injuries related to their
work at the factories.
ii. People, not their family members, living or working in the OF Estate in connection with work
related with but not directly involving Govt. service like servants, canteen staff, WWA staff etc.
iii. Non-entitled relatives of entitled employees like married daughter, son, grand children,
grandparents, in-laws etc.
The rate of Hospital stoppage for the persons mentioned under Category-II shall be as under, subject
to other terms and conditions mentioned in OFMR:
50% of the Hospital stoppage chargeable for patients under Category-III below, except that
medicines which will be charged at MRP (not 50% of MRP)
12. Category-III:
i. All other non-entitled patients, not falling under any of the above mentioned categories, shall be
charged at the following rates, subject to other terms and conditions mentioned in OFMR:
a) Where no CGHS rate is available, the same should be decided by MO I/C Hospital based on the
recommendations of a Standing Committee of the Hospital based on actuals and other parameters.
However, OFB may fix common rates for such items.
b) If accommodation is provided in Special Ward/Officers’ Ward the Hospital stoppage rates will be
higher than the above rates by 25%.
c) No separate diet charge shall be levied.
d) Medicines shall be charged at Maximum Retail Price (MRP), if the same is not included in the
package

visit will apply, provided the patient brings the OPD slip issued on the first occasion.
5. Medicines shall be charged at Maximum Retail Price (MRP)
6. Issue of Medical Certificates (of any type) will be charged @Rs.30/- in addition to item wise charges
for consultation, investigations etc.

Y. Chapter-VI (1 to 7): Hospital stores, equipment and diet
Issues not covered under this policy may be dealt with according to guidelines or instructions issued by OFB
from time to time regarding material management and store management.
1. Consumables: Includes items which are consumed during their use (e.g., drugs, stationery, lab
reagents, sanitary items like soap) and disposables.
2. Expendable Stores: Includes non-consumable items which are not durable or are fragile in nature,
require to be replaced frequently and are of low value. The following items are examples of
expendable Stores:
3. Non-Expendable Stores: These are relatively high value items and are durable with significant life
period. These include high value instruments, Equipment, furniture, etc
b. Hospital Store Department: Ordnance Factory Hospitals will have two store departments:
a) Medical Store (MS): MS deals with all Medical Stores of the Hospital.
b) General Store (GS): GS deals with all General Stores of the Hospital.
i. Medical Stores (MS): Medical Stores will consist of Medical Equipment, Stores.
1. Consumable
a) Drugs, vaccines etc
b) Disposables

c. Custody of Store: A Medical Officer will be in-charge of each store - MS and GS. The person holds
administrative responsibility of the store. Pharmacists are responsible for the Medical store and
designated staffs are responsible for the General Store. They will be personally responsible for the Stores
under them. The responsibility of individual Stores in MS and GS will be allotted to the individual
pharmacists and staff working in MS and GS by MO I/C of the store concerned, in consultation with MO
I/C Hospital.
d. Authorisation of Hospital Stores: The Hospitals can draw Medical and general Stores and Equipment
according to the scales including
a) Medical Stores
Scale of Medical Stores for Ordnance Factory Hospitals
Formulary for Ordnance Factory Hospitals
b) General Stores
Scale of Hospital General Stores for Ordnance Factory Hospitals
Scale of diets and extras for Ordnance Factory Hospitals
Scale of General Furniture for Ordnance Factory Hospitals
Scale of Information Technology items for Ordnance Factory Hospitals
The above scales are for guidance purpose. The procurement will be as per actual requirement. Further,
according to local requirement, procurement of items can also be done beyond the scales for items not
included in the scales or items included in the scales in quantities more than that of the scales. In case of
such procurement beyond the scales, the necessity must be recorded and the CFA should be satisfied
regarding the genuineness of such requirement.

e. Functions of Hospital Stores Departments: The MS and GS have the following inter-related
responsibilities:

1. Procurement: The general guidelines and policies issued by Government and OFB from time to time for
procurement of Stores should be followed.
a) Planning/Forecasting of Requirement
a. Consumable and Expendable Stores: Stores will be classified into two categories by the individual
Hospital, as indicated below:
b) Drawing up Specifications: Technical specification for the goods to be procured should be drawn up
with care. The technical specifications should be complete, clear, unambiguous, transparent and
consistent with the actual requirement.
c) Methods of procurement: The methods of procurement to be followed are:
1. Rate contract
2. Open Tender
3. Limited Tender
4. Single tender
5. Cash purchase
6. Purchase through Local Purchase Committee
7. Through PAC
d) Procurement system
1. Central procurement by OFB - For items selected by OFB for centralised procurement
2. Central procurement through DGS&D
3. Factory common procurement – GS items of common nature such as stationery, sanitary items can
be indented from store of the Factory concerned.
4. Local purchase of Hospital specific Stores – Local purchase of the following Hospital Stores can be
resorted to through OTE/LTE/CP
a) Items other than those reserved for centralised procurement by OFB.
b) Items reserved for central procurement but urgently required or supply not received due to any
reason.
Procurement action through Tender will be done through procurement section of the Factory. Cash
purchase will be arranged by MS/GS. The procurement proposal will be approved by MO I/C Hospital.

e) Follow Up with procurement agency:
1. The procurement proposal will be approved by appropriate authority and forwarded to appropriate
procurement agency.
2. The proposal will be followed up with the procurement agency to ensure early completion of the
process, placement of Supply Orders and prompt deliveries.
2. Inventory control: The objective of inventory control is to minimise the investment and expenditure on
inventories, the inventory carrying costs, losses due to expiry, surplus and obsolescence and to avoid stock
outs of vital and essential items. Inventory control aims to ensure that inventory holding is minimum
consistent with effective and efficient functioning and is utilised within life period.
a) The Hospitals will exercise inventory control through standard techniques such as:
1. Standardisation and Codification of inventory items
2. Determination of lead time, lead time usage, safety stock, re-order points and economical order
quantities
3. Inventory analysis such as ABC/ VED
4. Review and control of non-moving/slow-moving/surplus items
5. Budgetary control
b) Standardization & Codification: Each item proposed to be stocked in MS/GS will have an item code
based on proper specification. A master file of all codes allotted centrally or locally will be maintained
by all Hospitals. The logic for codification is given below:
1. Store Management:
a) Receipt: Received materials include those
− supplied by the suppliers
− received from Factory Stores
− received from sister Factories/Hospitals
− returned to Stores by the user units of the Hospital
• General Stores common with Factory will be routed through Factory Stores. Medical Stores and
Hospital specific General Stores from suppliers will be received in MS/GS directly.
• At the time of the delivery of the Stores, they will be accepted on “said to contain” basis and the
final acceptance is subject to detailed inspection of the Stores and testing of the Stores.
b) Inspection: All Equipment and Stores received directly from suppliers will be inspected as early as
possible to ensure that the materials received and accepted are of the ordered quantity, quality, price, and
meet the terms and conditions of the supply order. MO I/C Hospital will be the inspecting authority. The
MO I/C Hospital will decide the modalities of inspection and nominate inspecting Officer/s. If the goods
fail to meet the specifications given in the contract, the same will be rejected. The supplier will be asked
to replace the goods or rectify the defects within the specified time limits.
c) i) Accounting of Stores: Inspection passed received material will be taken on charge of MS/GS in the
relevant stock register. Different stock ledgers of quantitative, unpriced type will be maintained in respect
of different types of Stores.
ii) Accounting of Ration Stores: Rations will be issued from the ration Stores on the basis of the ‘Daily
Diet Requisitions’ received from the ward. The details of diets ordered will be maintained in the ward in
‘Monthly diet sheet’. The details of diets and extras issued will be maintained by GS in ‘Monthly
summary of diets and extras’. The monthly issues will be charged off from “Ration Stores Register”.

d) Proper and Safe Storage:
a. Storage of Equipment and Stores should be carefully planned to ensure that materials are stored
properly and safely so that they are not wasted due to deterioration, expiry, theft/pilferage, fire etc
under proper environmental conditions. The MO I/C Hospital will decide the requirements to meet
proper and safe storage as above.
b. The store procedures and storage arrangement should ensure that items with a definite life period are
consumed within their life period, batches with earlier expiry date are issued first, and any surplus that
cannot be consumed within their life period are detected and disposed off at the earliest.
c. Poisons, dangerous drugs, narcotic and addictive drugs as per the list decided by MO I/C Hospital will
be kept separately under careful supervision and accounting as per legal requirements as applicable.
e) Issue of Stores:
a. Consumable Stores: Consumable Stores will be issued by MS/GS against the indents raised by the
various departments of the Hospital like pharmacies, ward, laboratory, operation theatre, out-patient
department etc in accordance with the time schedule and procedure laid down by the MO I/C of the
Hospital from time to time.
b. Expendable Stores: Expendable Stores will generally be issued on demand note raised by the
departments and will be charged off from the stock register.
c. Non- Expendable Stores: The non-expendable items will only be distributed to various user
departments and will not be struck off from charge. The distribution will be shown on the register and
will be supported by distribution vouchers. The item will be written off from the stock register only
under the following circumstances:
f) Requisitions for Local Purchase: The various departments of the Hospital will place their requisitions to
MS/GS for procurement Equipments, Stores which are required to be purchased locally for their
departments. MS/GS will process these requisitions in accordance with the instructions issued by the MO
I/C of the Hospital
g) Stock Verification: The objective of stock verification is to verify the quantity and physical condition of
the Stores. Responsibility and Frequency of Verification are given below:
MO I/C Hospital may order additional periodical, random and surprise verification of Stores.
2) Maintenance and Repairs of Equipment: For repairs the Hospital Equipment will be sent to
authorized or suitable service centre. Maintenance and repair of specific Hospital Equipment should be done
by manufacturer/supplier/reputed agencies through Annual Maintenance Contracts. Calibration of Equipment
should be done periodically as decided by MO I/C through designated/ reputed agencies.
3) Life Time of Medical Equipment: The normal life of Equipment differs from item to item. The
duration for which an Equipment can be used without any major repair, is considered to be the normal life of
the sophisticated Medical Equipment, as such Medical Equipment do not work efficiently after major repairs.
Further, rapid advances in Medical Equipment result in older models being replaced by newer ones and
difficulty in getting older Equipment serviced. The normal life suggested for different Medical Equipment is
given below:

The life of any other item, not covered by the above list, shall be decided by the MO I/C Hospital
a. Survey and Opinion by a Board of Officers: CFA has the power to declare an item surplus or
obsolete or unserviceable. The reason for declaring the item surplus or obsolete or unserviceable
should be recorded. CFA may at his discretion constitute a committee at appropriate level to declare
items surplus or obsolete or unserviceable.
b. Disposal: The Stores for disposal are divided into the following categories:
Category Includes
Expired items Drugs, disposables, lab chemicals etc
Surplus Stores Consumable Stores that cannot be consumed within their life period. Non-expendable
items that are in working order but are not required for use in any department of the
Hospital and stock lying in Stores for more than three years shall be deemed as surplus
unless there is sufficient reason to treat them otherwise
Unserviceable Stores Non-expendable items which are not in working order, outlived its normal span of life
and are beyond economic repair
Obsolete Stores Non-expendable items in working order but cannot be put into use effectively being
outdated due to change in technology/design
Empties Empty containers, crates, bottles, plastic jars, drums etc
Scrap Process waste, unserviceable items declared as scrap, broken & any other item not
covered above but has got resale value
Infected articles Articles, which have been used by persons suffering from infectious diseases and
which cannot be properly disinfected.
1. Disposal of Expired items: Expired consumable items not exceeding Rs 5,000 in each case will be
charged off from the stock register after sanction for the same by MO I/C Hospital and when certified
not due to negligence. For items exceeding Rs 5,000 value per item, loss statement will be prepared
and sanction CFA will be obtained. Then, the expired items will be disposed off or destroyed
immediately by adapting suitable modes so as to avoid health hazards.
2. Disposal of Surplus Stores:
1) Consumable and expendable Stores:
a. Such surplus Stores will be intimated to the other OF Hospitals more than 6 months in advance
of the date of expiry of the item. The items will be issued free of cost to the Hospitals who
require the same.
b. If no OF Hospital requires the item, or if the life period of the item is less than 06 months, the
same will be circulated to local Central/State Government/Local body/not for profit
organisations in that order of preference and issued free of cost as per their requirement.
c. If the surplus Stores cannot be utilized within their life period despite above efforts, they will be
written off as per the existing procedure under the sanction of CFA.
2) Non-expendable items:
a. The non-expendable items declared surplus by CFA will be circulated to other OF Hospitals and
will be issued to them free of cost.
b. General items required by other sections of the Factory can be transferred to them.
3) Disposal of Obsolete Stores: Disposal of Stores declared obsolete will be carried out as decided
by CFA.

4) Disposal of Unserviceable Stores: Disposal of unserviceable Stores will be carried out as decided
by CFA or as per the recommendations of board of survey approved by CFA.
5) Disposal of empties, scrap: Those items having salvage commercial value should be returned to
Factory Stores for further action
6) Disposal of Infected articles: Articles, which have been used by persons suffering from infectious
diseases and which cannot be properly disinfected, will be destroyed under the orders of CFA and
these orders will be the authority for their writing off from stock registers.
All receipts and issue vouchers will be posted to the stock registers at the earliest. Items of local purchase of
Medical stores and equipment will be maintained in the same ledgers. The MOs I/C MS/GS will ensure that
accounts of the store are maintained up to date.

b) Adjustment of accounts–When unserviceability is due to fair, wear and tear the stocks in question will
be written off on Expense Voucher on the authority of the approved board proceedings. In case the
change in condition is due to causes other than fair, wear and tear, the write off will be supported by a
loss statement approved by the CFA.
c) Destruction of unserviceable/condemned stores–Where the board has recommended destruction/
mutilation, the article will be destroyed in the presence of a MO detailed by MO I/C Hospital and the
following certificate will be recorded by the MO concerned on the approved board proceedings/
sanctioned loss statement(s) as the case may be: - “I certify that the above mentioned articles have been
destroyed/mutilated this day in my presence beyond the possibility of further use”.
d) Disposal of stores surveyed by the board will normally be carried out as recommended by the approved
proceedings-The scrap resultant from destruction of condemned stores that have a salvage value and
those recommended to be passed on to salvage unmutilated will be sent to the Factory Store.
e) Replacement action can be initiated.
5. Defective drugs:
a) If any “Toxic reaction” or physical alteration or any doubt about the potency of a drug occurs
suggesting the possibility of a faulty batch of drugs, a report will be submitted by MO I/C of the
department concerned to the MO I/C of MS with copy to MO I/C Hospital giving the following details:
a) Name
b) Batch number and dates of manufacture and or expiry.
c) Manufacture’s name.
d) Reasons for considering the drug to be toxic.
e) Total quantity held in stock.
b) On the orders of MO I/C, MS will suspend the use all the stock of the particular item available in the
Hospital immediately and call back the stocks to MS. The stock so returned and any stock available in
MS will be segregated from other regular stock and taken in charge of suspended stock register and will
be stored separately.
c) A sealed sample of the suspended stock will be sent for analysis to appropriate drug testing laboratory.
On receipt of the test report from the laboratory, final disposal instructions will be issued with regard to
such stores.
6. Dispatch of Medical equipment and stores:
a) The stores for dispatch will be carefully packed under the supervision of an Officer in such way as to
preclude any possibility of damage in transit, and commensurate with the nature and value of the stores
involved.
b) The description of the quantity of store shown in the packing note will tally with the actual contents.
c) The packages containing fragile equipment will be clearly marked “FRAGILE WITH CARE.”
Inflammable articles will be packed and dispatched in accordance with instructions of the Railways/
transporters.
d) Stores will be dispatched by the cheapest route and mode of carriage meeting the requirements of
urgency of dispatch and nature of store involved.
e) Articles of value such as X-ray apparatus, microscopes and other expensive, fragile or delicate scientific
instruments, apparatus/ stores will be dispatched under escort.
f) Bulk stores will be despatched through Factory stores whereas small stores despatched through post
may be directly despatched by Hospital through usual procedure.
7. Controlled Stores:
a) Issue of certain consumable and expendable stores may be controlled by the MO I/C Hospital with a
view to conserve their stock or curtail their expenditure.
b) Such items are called “Controlled stores”.
8. Loan of equipment:
a) Medical equipment required by a OF Hospital for a short period, usually not exceeding one year may be
obtained on loan from another OF Hospital.
b) Articles so obtained, will be taken on charge and accounted for in the ledger in the usual manner and
will be returned without delay when they are no longer required and in any case, before the expiry of the
sanctioned loan period.
c) The issue/receipt vouchers and the ledger entries of both Hospitals pertaining to such articles will be
marked with the note “On Loan”
9. Loss Statement:
a) The loss statement is prepared under the following circumstances to regularise loss of stores.

b) The loss statement is prepared accompanied by required documents. After vetting by the local accounts
Officer for vetting, the loss statement is sanctioned by CFA. After sanction is received the items are
charged off though I.V.
10. Discrepancies in Stores:
a) Discrepancy of stores can arise/noted in the following circumstance.
i) During stock verification of Store.
ii) At the time of receiving the stores
a. The loss of store in transit from the consigners noted while taking delivery from transporter.
b. Packing mistakes by supplier noted while opening the consignment
iii) Due to theft
b) When discrepancy is noted action is taken as per the prescribed procedure.
11. Regularisation of Breakage:
a) Breakage of store in transit detected on receipt.
b) Breakage of material in use while handling: These are to be recorded in the breakage register where
specific reason for the breakage should be noted by the staff and the MO I/C Department concerned
should recommend whether the breakage is chargeable to state or chargeable to individual concerned.
The breakage is to be regularised through Loss statement.
12. Hospital store – Important documents:

Z. Chapter-VI (8): The scale of authorisation of Ambulance Vehicles and any other patient carrying Vehicles,
their specification, method of procurement, periodicity of replacement, authority and responsibility
connected with repair and maintenance etc. will be governed by the directives issued by Ordnance Factory
Board from time to time.

ii. The Ambulance Vehicle may be positioned at the FHC or at any other place depending on the
expanse, nature of work and requirement of the Factory.
iii. The number of Ambulance Vehicle available inside the Factory premises may also vary and will
depend on the expanse, nature of work and requirement of the Factory.
iv. The occupier of the Factory may decide such issues regarding positioning and number of
Ambulance Vehicles in consultation with the MO I/C Hospital.
v. A defined policy may be made by each Factory in this regard.
vi. The MO I/C of FHC or the Medical Officer posted at the FHC will have the overall responsibility
and control of the Ambulance Vehicles inside the Factory premises.
vii. The departmental Ambulance Vehicles will preferably be used for services within the Factory
premises.
b. Few instances are given below, suggestive of the circumstances, in which the Patient Carrying
Vehicles may be utilised:
i. For transportation of patients advised to attend a referral Hospital and in such case a number of
patients may be taken together for attending the Hospital(s) under supervision of a staff from the
Hospital. They may be brought back in the same manner.
ii. For providing transport service to patients, according to a scheduled time table, to and from an
Estate located at a distance from the Hospital.
iii. For providing transport service to patients to and from the Ordnance Factory Hospital to one or
more referral Hospital(s), according to a scheduled time table.
iv. For carrying Medical and/or Paramedical personnel for professional work/duty to a distant place
but under the administrative and professional responsibility of the Hospital. Examples are:
vaccination in Estate, Health check-up in a locality, sanitary round in a locality/area, inspection
round to a health unit, collection of Hospital requirements etc.
c. Ambulance Vehicle and Patient Carrying Vehicle shall be utilised only for purposes for which they are
provided.

ix. Establishment of procurement system:
25. Procurement system may be streamlined in accordance with OFMR to ensure availability of
required stores to the Hospital in time and in accordance with their requirement.
26. System for taking anticipated actions are required to be established to minimise delay is
availability of stores to the Hospital.
27. The Hospital may be consulted for this purpose.
x. Verification of Emergency treatment claims:
28. A system for verification of claims for Medical reimbursement in case of emergency may be
established according to instruction.
29. This will ensure consistency and transparency.
30. The Hospital may be consulted for this purpose.
xi. Guidance to Hospital in all aspects of store management:
31. Necessary help and guidance may be extended to the Hospital in all aspects of store management.

2. PURCHASE PROCEDURE FOR MEDICINES & RELATED ITEMS:
A) The procurement of medical stores items shall be done through the following systems:
i) Procurement through Centrally concluded Rate Contract: Items shall be procured on the basis of
rate contract concluded centrally by OFB or by any other authorised agency. The following modalities
may be followed:
1. A list of approved vendors shall be prepared on the basis of appropriate vendor selection method.
2. Contracts shall be concluded with the manufacturers/authorised distributors with the provision of
staggered delivery to the unit Hospitals as per the requirement of such units.
3. Initiation and processing of procurement action at OFB shall be done by Medical Section in
consultation/association with MM & Finance Division.
4. The rate contract documents shall be circulated to all concerned.
5. MO I/C Hospital will be ‘Direct Demanding Officer (DDO)’ for the purpose of the contracts.
ii) Procurement of medical stores items may also be made locally by entering into parallel Contracts with
provision for staggered supply through:
a. Local Contract for bulk supply: This method shall be used for procurement of items in bulk on a
monthly/quarterly basis. This method will be operated in respect of items where requirements can
be estimated beforehand with reference to some past trends.
1. Separate contracts may be concluded for different groups of medical stores such as medicines,
surgical items, laboratory items etc.
2. Item wise bulk purchase through Tender enquiry may also be resorted to meet the requirement.
b. Local Contract to meet emergent daily requirement: In view of the peculiarity and emergency
nature of the requirement, each Hospital may enter into a parallel running contract for procurement
of medical stores items on day to day basis to avoid loss of time involved in processing the
procurement.

10. The TPC may carry out the vendor selection process as technical assessment exercise or another
committee may be entrusted with the job. In that case, for carrying out the verification process a three
member committee comprising of (i) One Medical Officer, (ii) One Officer from procurement Section of
the connected Factory and (iii) One Officer from Associated Finance will be constituted by GM in
consultation with MO I/C Hospital.
11. Attempt should be made to enlist dealers/retailers located proximally, i.e. within the city/within 5-10 km
radius etc. The exact distance shall be defined by the Hospital. This clause may be done away with in case
of bulk supply contract.
12. Advertisement calling for requests for vendor enlistment to be published in reputed local newspapers (to
include at least one major vernacular newspaper).
13. The supply of medical stores items will be on credit and payment will be made on monthly/quarterly
basis.
14. Provisions of EMD, SD, LD and Risk purchase etc., as per standard provision, will be incorporated in the
tender document.
15. At least two vendors will be selected, simultaneously, as far as possible.
16. Subject to fulfillment of other conditions, Net Discount on Maximum Retail Price (MRP) shall be the
basis for determining the L1 bidder. Subject to the acceptance of discount rate offered by L1 rates, L2
vendor will be selected.
17. Each of the selected vendors will be loaded @ prefixed ratio (60:40 or 70:30) of the Annual requirement,
subject to the condition that L1 shall have major portion of the supply. Distribution ratio should be
specified in tender document accordingly.
18. In the tendering process, officers designated as per provisions of OFB orders on the subject, will
discharge the functions of CFA & Chairman of TPC as per prevailing TPC structure.
19. Subject to annual renewals, enlistment of a vendor shall remain valid for three years.
iii) In addition to the above tendering procedure, in cases of emergent requirement, OF Hospitals
may procure medical stores items through spot purchase (cash purchase) or local purchase
committee routes.

3. COMPUTATION OF NOTIONAL INCOME:
The value of service shall be calculated for all the components of healthcare services rendered to each patient of
all categories (entitled and non-entitled), non-patient care services and miscellaneous services rendered by the
Hospital. The sum total of such value shall constitute the `notional income’.

c) Standard Diets:
1. Ordinary diet (OD): It is the default diet for patients. Other diets are prescribed on specific

2. Fluid diet (FD): Fluid diet is advised in conditions like Fever, Loose motions, Vomiting, Post-
operative cases, etc where patient cannot take Ordinary Diet and when patient prefers it to
ONV/OV.
3. Convalescent diet (CD): Convalescent diet is advised for patients suffering from chronic
debilitating diseases such as prolonged fever, severe malnutrition, Tb, burns and as transition diet
from FD to OD.

1. No Diet (N.D): The following categories of patients will be placed on ‘No Diet’ (ND) category:
Those who do not require any diet - patients on `Nil Oral’ order
Those who require diet but cannot be placed on any full or part standard diet. Ex. Infants – they
will be placed on ND and subsisted on extras.
2. Extras: Extras are not permissible with standard diets. Extras will be permitted only for those on
No diet, if the medical officer-in-charge considers it essential to meet the special dietary needs of
the patients. In such cases, the individual components of standard diet can be prescribed as ‘extras’
in quantities not exceeding the maximum of the scale of the particular item in the scales.
3. Not in scale items: For special cases, when on ‘No Diet’, quantities in excess of those specified
may be issued if considered necessary by the Medical Officer, who may also demand any ration or
substitute not included in the scale. However, such not in scale issues should be rare, supported by
reasons recorded in writing and shall require sanction of MO I/C Hospital.
e) Modified Diets:
1. Vegetarian Diet: The vegetarian substitutes from the substitutes list against non-vegetarian items
such as eggs, meat will be provided.
2. Modified diet with specific restrictions: The standard diets take care of general nutritional
requirements, specific disease condition of the patient may call for restrictions of specific
components of the diet. Such restrictions may be ordered by the Medical Officer. Examples are
restrictions of sugar for diabetics, salt for hypertensives, calories for obese, cholesterol in
Hyperlipidaemia, proteins in chronic renal failure, Potassium in renal failure etc.
f) Age Criteria:
0–4 N.D – extras to be ordered
5 – 10 Half adult scale
Above 10 Full adult scale.
The age groups indicate the beginning of the year of age to the completion of year of age.

g) Eligibility:
i. Only persons admitted to hospital are entitled to hospital diets.
ii. On day of admission, persons admitted before 1.00 pm will be eligible for one of the Standard diets and
persons admitted after 1.00 pm will be placed on a 1/2 standard diet (1/2 FD, 1/2 OD, 1/2 CD).
iii. On day of discharge, persons discharged after 12.00 pm will be eligible for one of the Standard diets
and persons discharged before 12.00 pm will be placed on a 1/2 standard diet (1/2 FD, 1/2 OD, 1/2 CD).
iv. Short admission (< 24 hrs) patients will be placed on ½ or full standard diet or ND depending on the
duration of admission.
v. No outside food is normally permitted.
vi. Sick attendants admitted to hospital will be issued OD.
h) Ordering of diet:
i. MO will order the diet for the patients taking into consideration nutritional needs of the patients, their
disease condition, age and diet preference.
ii. The diet ordered and served should be health promoting and disease preventing in nature which should
serve as a model for the patient to follow at home.
iii. The senior nurses of the wards are responsible giving specific diet requirements of individual patients
taking into consideration the specific diet orders of the Medical Officer and the patient preferences
within those orders. Preferences such as vegetarian diet, rice or roti, use of sugar substitute etc may be
made by the patient.
i) Miscellaneous: The MO I/C hospital will ensure that diets are varied as much as possible and that food is
served in an appetizing and attractive manner. The MO I/C hospital will decide the timings for serving
meals so as to suit local requirements.
j) Scale of rations:
i. Diets are prepared from rations issued as per authorized scales. The scale of rations for OD and FD is
given as Annexure. For CD the scale of rations will be same as OD and 300 ml of milk and one egg are
authorized additionally.
ii. The scale is arranged in major food groups. A list of alternative food items are given against food
groups. A list of substitutes is also given in the scale to provide variety, cater to regional variations in
food habits, and to meet the requirements of different types of patients.
iii. The Medical Officer I/C will decide the alternatives and substitutes to be used locally and their
quantities within the scales as per local requirements.
iv. The given quantity of each food group is for the standard item or alternatives. The authorized quantity
will be the total of various alternatives, and substitutes as per the equivalent quantities.
v. The quantities given are maximum permissible, which may not be required especially in larger
hospitals. Authorized quantity will not be issued routinely. Actual issue and drawl of rations will be
authorised quantity or as per actual requirements whichever is less. The requirements will be based on
patient strength, past trends, patient preferences, diet restrictions etc to avoid wastage of food and
ensure economy.
k) Scale of rations for Night duty MOs and staff: Medical Officers and staff on night duty will be
authorized for the following items per person on night duty:
1. Tea 5 grams or soluble coffee powder 3 grams.
2. Sugar 20 grams
3. Milk fresh 60 millilitres.
l) Scale for breast fed babies: Infant food or milk powder will be issued to breast fed babies whose
mothers are admitted into the hospital as patients and are advised on medical grounds not to suckle their
babies.
