JOINT AFFIDAVIT OF COHABITATION


(As required by Article 34 of the Family Code
Of the Philippines as Amended by Executive
Order No. 277, July 17, 1987)

That we, BANJO A. ACALA and JOEVELYN R. FONTEJON, both of legal


age, Filipinos, single, and residents of barangay Junob, Dumaguete City,Negros Oriental,
after having been duly sworn to in accordance with law, do hereby depose and manifest:

That sometime in January 2004 we met each other in Pinili Street, Dumaguete
City, Negros Oriental because I, Joevelyn Fontejon was selling different kinds of goods
in the said Street, while I, Banjo A. Acala was working in the nearby store owned by
Mario Ong; and then we became intimate friends;

That moved by our mutual love and understanding, in the part of the year 2005,
we have decided to live together as husband and wife in Junob. Dumaguete City, Negros
Oriental until the present;

That during our cohabitation we were blessed with three children, namely:

1. JOANA F. ACALA, who was born on April 2, 2005;


2. JB F. ACALA, who was born in April 11, , 2008; and,
3. JEROME F. ACALA, who was born on April 14, 2012.

That we have been living together as husband and wife for more than Five (5)
years already, or for Nine (9) years, without the benefits of being legally married and it
is now our desire to marry each other for our own good and welfare of our children;

That there is no legal impediment to our marriage as we both possess the legal
capacity to marry each other;

That we executed this affidavit freely and voluntarily just to attest to the
truthfulness and veracity of our statements herein;

IN WITNESS WHEREOF, we have hereunto affixed our signatures this ___ day
