Set For Health
Set For Health is a comprehensive protection plan that provides coverage for multiple critical illnesses
(42 major and 15 minor) and death until age 75, and returns all the premiums if Insured suffers no
major critical illness at the end of coverage period. Payment term options are 5, 10 or 20 years.

With up to 300% Sum Assured for major critical illnesses, 20% for a minor critical illness and another
100% for death, Set For Health offers up to 420% protection benefits to ensure the family and loved ones9
get sufficient amount of financial security if the unthinkable happens.

“Unrelated Major Critical Illnesses” means


1. Each of the three (3) Major Critical Illness claims must
belong to a different Major Critical Illness Group (see
groupings below), and
2. Diagnosis of subsequent critical illness should be at least
one (1) year from the date of diagnosis of the preceding 1

What we don’t cover (exclusions)


No Major Critical Illness and/or Minor Critical Illness Benefit claims will be paid if the
critical illness results directly or indirectly, wholly or partly, from any of the following
circumstances:
1. Attempted suicide or an intentional self-inflicted act by the Insured
2. Wilful participation in illegal and/or unlawful acts or omissions
3. Alcohol or drug abuse
4. HIV and/or any HIV-related illness including AIDS and/or any mutations,
derivations or variations thereof except HIV/AIDS due to Blood Transfusion and
Occupationally Acquired HIV/AIDS, or
5. War or any act of war (whether declared or not), civil or military insurrection
and civil commotion amounting to a popular uprising.
In the event of Insured’s death, no benefits will be paid in case of suicide within the
first two (2) years while Insured is sane. 1

Q1. Available payment options?


You may choose to pay for 5 years, 10 years, or 20 years. In all the payment options,
the coverage period is until age 75.
Q2. Eligible ages?
The age limits for each payment option were set so that you (Policyowner/Insured)
are able to finish paying for the premiums during your most productive years until
age 65 only.
For 5-pay option: 15 days to 60 years old
For 10-pay option: 15 days to 55 years old
For 20-pay option: 15 days to 45 years old
Q3. Minimum and maximum Sum Assured?
Minimum Sum Assured: PHP500,000
Maximum Sum Assured: PHP4,000,000 subject to applicable underwriting
guidelines on aggregation of Sum Assured for CI plans

Q5. What are the conditions for a valid critical illness claim?
1. Each of the three (3) Major Critical Illness claims must belong to a different
Major Critical Illness Group (see groupings in previous section).
2. Survival period is 14 days from diagnosis date before benefit becomes payable.
3. Claim shall be subject to the waiting period of 90 days from effective date or
date of last reinstatement.
4. Diagnosis of subsequent critical illness should be at least one (1) year from the
date of diagnosis of the preceding critical illness.
5. A 2nd benefit under Cancer Group may be claimed provided that Insured is
declared cancer-free for at least five (5) years prior to diagnosis of the 2nd
cancer.
6. No subsequent Major Critical Illness Benefit will be paid if a benefit due to
Terminal Illness or Loss of Independent Existence has been paid.

Q6. Is Healthy Life Benefit still payable at age 75 even if Insured


already claimed for a Minor Critical Illness benefit?
Yes. The only condition for Healthy Life Benefit to be payable at age 75 is that no
Major Critical Illness has been paid.

Q7. Are there policy dividends?


Set For Health does not have policy dividends, which are non-guaranteed benefits
that are offered in other insurance plans. Set For Health is all about guaranteed
financial protection for critical illnesses and death, plus a guaranteed reward for
keeping yourself healthy.

Q8. Are there cash values available for loan?


Yes. For as long as the Policy is not under RPU (see Q9 on next slide), the Policyowner
may file a cash loan against the Policy with applicable interest.

Q9. What are the available Non-Forfeiture Options (NFO) should


Policyowner decide to discontinue paying premiums?
The Policyowner may nominate any of the following NFOs:
NFO Brief Description
Reduced Paid-Up Policy continues to be effective but with a reduced
Insurance (RPU) Sum Assured
Automatic Premium Loan Policy remains inforce for as long as the cash values
(APL) are sufficient to pay the premiums falling due
Policy Surrender Policyowner receives cash value and Policy is
terminated

If no option is nominated, the RPU shall apply.

Q10. When are benefits not payable? (exclusions)


Benefit payable shall be based on the definitions and conditions of each critical
illness set out in the Policy Contract. In all cases, no Major Critical Illness and/or
Minor Critical Illness Benefit claims will be paid if the critical illness results directly or
