The following documents should be submitted directly to the respective college offering the
intended program.
1. Official transcript of academic record of undergraduate work (and graduate work, if any)
– one original and two photocopies
For evaluation purposes, photocopies of records may be accepted provided they are
properly authenticated (in the case of foreign applicants, by the Department of Education
or by duly designated authorities in the country of the applicant). Final admission will be
subject to verification of documents submitted against the original copies of credentials.
Certified English translations should also be submitted, where necessary.

1. Academic Information
1.2.1.1. High School ID card with photo or Equivalent valid ID with photo
1.2.1.2. Color passport-size photo taken within 6 months, in white background, with neutral
facial expression; must not be wearing eyeglasses or color contact lenses, and with
ears exposed if religious customs do not prohibit1.2.2. Proof of Citizenship as

2. The following applicants must secure clearance from the UST Office of the Registrar before paying
2.1 Out-of-school youth passers of the Philippine Educational Placement Test (PEPT) and
Alternative Learning System Accreditation and Equivalency (A LS A & E) - present the PEPT/ALS
A&E certificate
2.2 Applicants who are graduates of previous school years - present original Form 138 (Report Card)
2.3 Applicants from foreign schools - present Form 137 (Transcript of Records)
2.4 Foreigners graduating from local schools - present passport and Alien Certificate of Registration
6. Log-in your reference number, upload your ID picture, and then click save.
8. Request the Registrar’s Office of your school to accomplish the High School Grades Form. Make
sure that the school principal or registrar signs the High School Grades Form.
9. Complete your online registration to print the Application Form and Payment Slip. DO NOT
PROCEED TO THE BANK WITHOUT BRINGING THE PRINTOUT OF YOUR USTET PAYMENT
SLIP. Payments received by the bank by applicants who are not fully registered will not be
processed by UST.
10. Proceed to any Metrobank or BPI branch to pay, over the counter, the non-refundable application
fee indicated in the USTET Application payment slip. Fill out the Metrobank or BPI payment slip
