UP Requirements for Submission

The following documents should be submitted directly to the respective college offering the
intended program.

1. Official transcript of academic record of undergraduate work (and graduate work, if any)
– one original and two photocopies

For evaluation purposes, photocopies of records may be accepted provided they are
properly authenticated (in the case of foreign applicants, by the Department of Education
or by duly designated authorities in the country of the applicant). Final admission will be
subject to verification of documents submitted against the original copies of credentials.
Certified English translations should also be submitted, where necessary.

Applications accompanied by photocopies of academic records not properly authenticated


will not be processed.

2. Two written recommendations from former professors or experts in the field;


3. Duly accomplished application for admission form, including two (2) passport-size
photographs;
4. Marriage contract (for married women applicants);
5. Certification of English/Filipino language proficiency;
6. A non-refundable application fee (minimum of P100 for Filipino citizens, P200 for resident
aliens, and US$25 for non-resident aliens, depending on the college).

DLSU:
1. Academic Information

facial expression; must not be wearing eyeglasses or color contact lenses, and with

Click the SUBMIT Button1.6. Pay the non-refundable Admission Processing Fee The fees are
PhP600.00 for Filipino citizens (including those who are dual citizens) or PhP2,500.00 for
non-Filipino citizens. The processing fee for graduates of Philippine Science / Public High
Schools is waived. The modes of payment include:

UST

1. Only applicants who are currently enrolled in Grade 12 may apply.

2. The following applicants must secure clearance from the UST Office of the Registrar before paying

the application fee:

2.1 Out-of-school youth passers of the Philippine Educational Placement Test (PEPT) and

Alternative Learning System Accreditation and Equivalency (A LS A & E) - present the PEPT/ALS

A&E certificate

2.2 Applicants who are graduates of previous school years - present original Form 138 (Report Card)
2.3 Applicants from foreign schools - present Form 137 (Transcript of Records)

2.4 Foreigners graduating from local schools - present passport and Alien Certificate of Registration

3. Applicants must apply online.

4. Click Register and select the application for COLLEGE.

5. Fill out the Pre-registration form to get your reference number.

6. Log-in your reference number, upload your ID picture, and then click save.

7. Print the Senior High School Grades Form.

8. Request the Registrar’s Office of your school to accomplish the High School Grades Form. Make

sure that the school principal or registrar signs the High School Grades Form.

Afterwards, encode your grades in your application portal.

9. Complete your online registration to print the Application Form and Payment Slip. DO NOT

PROCEED TO THE BANK WITHOUT BRINGING THE PRINTOUT OF YOUR USTET PAYMENT

SLIP. Payments received by the bank by applicants who are not fully registered will not be

processed by UST.

10. Proceed to any Metrobank or BPI branch to pay, over the counter, the non-refundable application

fee indicated in the USTET Application payment slip. Fill out the Metrobank or BPI payment slip

with the following information: (Present to Metrobank or BPI the USTET Application payment slip

with reference number for verification purposes)
