Family Goal:

Family will go on a community outing (i.e. stroller strides, zoo, park) every week.

Strategies supporting family goal:

Mom and Dad will decide which time of the week, what day of the week work’s best outside of their work schedule in order to fulfil family goal.
Access community resources – EI team will work to support family by encouraging them to search various community activities around them.
Transportation – Ensure that family has the proper transportation in order to travel to various places each week.
Community involvement – Invite friends and other family members to participate during specific outings such as going to the zoo.

My family goal is structured to support and enhance a strong family dynamic by creating a more family/community based atmosphere. My goal is structured to
encourage community involvement for purposes of navigating the many opportunities/ activities the community offers, that will generally bring the family
together and create future supports.
