GUIDELINES

ABOUT THE AWARD

The Order of Laureano Gonzales Award shall be bestowed to

residents and registered voters of Mandaluyong City who have
achieved levels of excellence in the field of education, business, arts,
humanities, literature, culture, sciences, or information technology and
brought honors to themselves and the city. This Award is a fitting
tribute to Laureano Gonzales, one of the Katipunan Triumvirate, who
spearheaded the uprising of the brave men of Mandaluyong (then
called San Felipe Neri) on August 29, 1896.

The City Government of Mandaluyong believes that by

recognizing outstanding qualities and contributions of its citizens, all
residents shall be inspired to achieve excellence in certain endeavors
as well.

WHO ARE ELIGIBLE TO JOIN?

Nominated citizen must meet the following requirements:

1. A bonafide resident and registered voter of Mandaluyong City for at

least one (1) year.

2. Has not been found guilty of any administrative, civil, or criminal

offense and has no pending administrative, civil, or criminal case at
the time of nomination.

WHERE TO GET NOMINATION FORMS

Nomination Forms are available at the Search Secretariat,

Office of the City Mayor, Mandaluyong City or at your nearest barangay
halls. Forms can also be downloaded at www.mandaluyong.gov.ph

NOMINATION DOCUMENTS FOR SUBMISSION

Only the nomination folder containing the following documents

shall be submitted to the Search Secretariat, Office of the City Mayor,
Mandaluyong City:

1. Properly accomplished nomination form. Information required

should be complete as this will be the same form that will be
provided to the members of the Search Committee for Evaluation.
Authenticity of this document should be certified by the nominee,
endorser, and nominator.

2. Latest statement of assets and liabilities.

3. Certification issued by the nominee that he/she has not been found
guilty of any administrative case, civil, or criminal case nor has any
pending case against him/her at the time of nomination.

4. Detailed information on dismissed court cases, if any.

5. BIR Tax Clearance.

6. Photocopies of nominee’s clippings, news items, pictures and other

documents to support the nomination.

7. Certificates and all other documents required in each item of the

nomination form.

THE SEARCH PROCESS

Who can Nominate

 Any person in good standing can nominate in only one (1)

category.

Accomplishment of the Nomination Form

 The accomplished nomination form together with the

required attachments should be sent directly to the Secretariat

Committee. Nominees must submit three (3) photocopies and
four (4) 2 x 2 colored photos. Nominees are also advised to keep
a copy for their file/reference.

 All nomination forms must be officially signed by the

endorser, the nominee, and the Barangay Chairman.

Evaluation of the Nomination Forms by the Search

Secretariat

 The Search Secretariat evaluates the nomination forms based

on the eligibility criteria and determines if the documents
submitted is complete.

 The Search Committee confirms the completeness of the

documents and distribute the same to the Board of Judges by
category.

Evaluation of the Candidates by the Board of Judges

(BoJ)

 The BoJ for each category evaluates the nomination forms and
attachments based on agreed criteria.

 Based on cut-off score set by the Search Committee, the Judges

determine the second-round qualifiers.

 Based on a cut-off score set by the Search Committee, semi-

finalists are then determined. A thorough background check on
the character of each semi-finalist is undertaken.

 The semi-finalists are interviewed by the BoJ to determine the

finalists.

 The BoJ once again reviews the nomination forms and

attachments and conducts interviews of all finalists.

 The BoJ recommends winners for approval by the Search

Committee and confirmation by the City Mayor.

 The BoJ may elect not to give out award in any category if
the candidate falls short of the standards set by the Search
Committee. The decision of the Board of Judges is final. The

Board reserves the right to withdraw any award at any time as
necessary.

Proclamation of Winners and Recognition Program

The winners are proclaimed and given due recognition through

the following:

 Media Day

 Awarding Ceremonies at the Tatlong Bayani Monument

PRIZES

Awardees will receive a trophy and a plaque of recognition.

Nominators of awardees and the head of agency shall receive
certificates of recognition.

 Read and follow instructions carefully.

 Please do not skip any portion of the form. Be as complete and

as thorough as possible.

 Attach all the requirements.

IMPORTANT DATES TO REMEMBER

June 15, 2006 - Deadline for submission of original copies of

nomination forms
together with attachments to the Search
Secretariat.

June – July 2006 – Judging

August 25 – 29, 2006 – Recognition Program (required for

winners)
