Mission Statement

To improve the effectiveness and efficiency of care and the


quality of life of people receiving long-term services and
supports by fostering person- and family-centered quality
measurement and advancing innovative best practices.

LTQA STRATEGIC PRIORITIES

WHILE THE MISSION OF THE LONG TERM QUALITY ALLIANCE (LTQA) IS TO


ACCELERATE QUALITY IMPROVEMENT AMONG PEOPLE OF ALL AGES WHO RECEIVE
LONG-TERM SERVICES AND SUPPORTS (LTSS) IN ALL SETTINGS, THE LTQA BOARD
IDENTIFIED THE NEED TO ADDRESS A COMMON CHALLENGE CONFRONTING A
SPECIFIC POPULATION SEGMENT IN ORDER TO PRODUCE DEMONSTRABLE
IMPROVEMENT IN QUALITY IN THE SHORT-TERM. THUS, BOARD MEMBERS HAVE
AGREED THAT THE LTQA’S STRATEGIC PRIORITIES FOR THE NEXT TWO YEARS ARE
TO:

a. Advance the use of adult-specific key person- and family-centered quality


indicators who require long-term services and supports (LTSS). These
indictors focus on improvements in care transitions and health related
quality of life, and reductions in potentially avoidable hospitalizations,
re-hospitalizations and total health care costs.
b. Disseminate widely and promote adoption person- and family-centered,
effective transitional care practices that affect aforementioned outcomes;
and,
c. Engage and secure “buy in” for policies and mechanisms that advance
high quality, person- and family-centered transitional care.

To assure effective person- and family caregiver- centered care
transitions, improve health related quality of life, and decrease
potentially avoidable hospitalizations, re-hospitalizations and total
health care costs among adult recipients of long-term services and
supports (LTSS), the following goals, strategies and deliverables have
been identified.

Goal 1. Achieve agreement among stakeholders on key definitions of
effective person- and family-centered transitional care as well as a
framework for the acceleration of quality improvement, the
improvement of outcomes and the reduction of costs.

Strategies

1.A.1. Collect and assess universally held definitions of quality,


person and family-centered a) long-term services and supports and b)
transitional care across consumers, settings and types of services.

1.A.2. Create a framework for the acceleration of quality


improvement, improvement of outcomes and reduction of costs.

1.A.3. Design and implement processes to obtain consensus among


Alliance members on key definitions and framework for the acceleration of
quality improvement.

Scope of Work – TBD

Deliverable - Widespread dissemination of LTQA endorsed definitions and


quality framework.

Timeline - Endorsement of definitions and quality framework by LTQA


Board and Members by January, 2011.

Goal 2. Provide actionable input to the Department of Health and
Human Services (including the Centers for Medicare and Medicaid
Services (CMS) and the Administration on Aging (AoA) and others
related to specific regulatory provisions in the Affordable Care Act to
promote quality, person-and family-centered transitional care.

Strategies

2.A.1. Identify all relevant provisions in Affordable Care Act.

2.A.2. Develop and implement strategies (e.g., recommend LTQA


representatives for key positions, proposed regulatory language) to shape
regulations and critical decisions that can advance strategic priorities.

Scope of Work -TBD

Deliverable - Implementation of strategies that shape regulations and


critical decisions.

Timeline – a) Identification of key provisions – October 2010

b) Implementation of strategy - Ongoing

Goal 3. Advance meaningful measurement to accelerate quality
improvement.

Strategies

3.A.1. Identify a set of key measures suggested by the Department of


Health and Human Services (including CMS, AoA and other federal
agencies) including those endorsed by NQF that pertain to LTQA’s
strategic priorities for the implementation of health care reform (e.g.
measures related to quality improvement, payment reform, public
reporting, etc.).

3.A.2. Identify gaps in measures and foster development of such


measures.

3.A.3. Have a set of available measures adopted by LTQA Board and


Members.

3.A.4. Implement strategies to promote use.

3.A.5. Use measures to evaluate community initiatives focused on


LTQA strategic priorities.

Deliverables – Assessment of the effectiveness of available measures in


accelerating and accessing changes in performance in quality.

Timeline – a) 3.A.1 and 3.A.2 – February 2011

b) 3.A.3 and 3.A.4 – Ongoing

Goal 4. Achieve adoption of effective transitional care practices that
integrate long-term services and supports and health services and
contribute to demonstrable improvements in quality.

Strategies

4.A.1. Obtain LTQA adoption of a set of principles to guide the


design, implementation, and evaluation of effective person- and family-
centered transitions.

4.A.2. Disseminate the LTQA statement of principles widely.

4.A.3. Identify effective transitional care practices that foster long-


term services and supports and health care services integration.

4.A.4. Pilot test the implementation of effective transitional care


practices in collaboration with community partners.

4.A.5. Identify lessons learned and recommend strategies to scale


robust transitional care practices based on pilot findings.

1. Widespread dissemination of LTQA endorsed principles for the


facilitation of effective care transitions.
2. Lessons learned and recommendations for scaling effective
transitional care practices.

Goal 5. Promote engagement and “buy in” for policies and mechanisms
that advance high quality, person- and family-centered transitional care.

Strategies

5.A.1. Develop and implement an internal communication plan to


promote “buy-in” and engagement of LTQA Board, workgroups and
members towards the achievement of the LTQA’s strategic priorities

5.A.2. Develop and implement an external communication plan to


engage a wide range of stakeholders (e.g., LTSS consumers, policy makers,
public) to build support for policies and mechanisms that promote effective
person- and family-centered transitional care.

5.A.3. Create and pilot a recognition program to acknowledge


organizations that have achieved excellence in the advancement of LTQA’s
strategic priorities.

Deliverables -

1. Implementation of master internal and external “messaging


plans” via a variety of communication tools (e.g., websites, social
networking, story bank of “best practices”, campaign).
2. Implementation of recognition program that acknowledges
organizations that have demonstrated excellence in the achievement
of LTQA’s strategic priorities.

Timeline -
a. Initial internal “messaging plan” developed and implemented –
November, 2010 and ongoing

b. Initial external “messaging plans” developed and implemented
–February, 2011 and ongoing
