FINDINGS

RE: Allegations against Master Deputy Jeffery Van Camp

The recommendation of the Internal Affairs Unit in this complaint should be classified as:

1. 213.2 Neglect of Duty Offenses, (23) Failure to Follow General Orders, Directives, and
Sheriff's Orders: Members will adhere to General Orders, Sheriffs orders, policies, and directives
and will faithfully execute all duties and responsibilities of their assigned position.

• General Order 232 Public Records, 232.1 Duties and Responsibilities (4) Every member of
the Sheriffs Office is responsible for the security of records in their possession.

SUSTAINED

During his interview, Master Deputy Van Camp stated that he printed a copy of Escambia
County Sheriffs Office Offense Report ECSOl 1OFF030535. The evidence gathered during this
investigation clearly shows that the copy that Master Deputy Jeff Van Camp possessed was the only
copy of Escambia County Sheriff's Office Offense Report ECSOl 1OFF030535 that could have been
placed on the website LEOAFF AIRS.COM. The fact that this Offense Report was posted on this
website proves that Deputy Van Camp was not responsible for the security of the record that was in his
possession.

2. 213.3 Improper Conduct Offenses, (26) Dissemination of Information: Members will treat the
official business of the Sheriffs Office as confidential. Information regarding official business will
be disseminated only to those for whom it is intended and in accordance with established
procedures. Members may remove or copy official records or reports only in accordance with
established procedures.

SUSTAINED

During his interview, Master Deputy Van Camp stated that he printed a copy of Escambia
County Sheriff's Office Offense Report ECSOll OFF030535. The evidence gathered during this
investigation clearly shows that the copy that Master Deputy Jeff Van Camp possessed was the only
copy of Escambia County Sheriff's Office Offense Report ECS0110FF030535 that could have been
placed on the website LEOAFF AIRS.COM. The fact that this Offense Report was posted on this
website proves that Deputy Van Camp did not treat the official business of the Sheriff's Office as
confidential and that he failed to disseminate the Offense Report in accordance with established
procedures.

3. 213.3 Improper Conduct Offenses, (69) Unbecoming Conduct: Personnel shall conduct
themselves at all times, both on and off duty, in such a manner as to reflect favorably on the
Sheriffs Office. Conduct unbecoming shall include that which brings the Sheriffs Office into
disrepute or reflects discredit upon the personnel as a member of the Sheriffs Office, or that which
impairs the operation or efficiency of the Sheriffs Office or personnel.

SUSTAINED

Master Deputy Jeff Van Camp's conduct in this incident is questionable, brings the Escambia
County Sheriffs Office into disrepute, and reflects discredit upon the members of the Escambia County
Sheriffs Office.
In Master Deputy Jeff Van Camp's report, he wrote that he contacted by phone
and obtained a sworn statement from her. Deputy Van Camp had no means of verifying who he was
taking a sworn statement from. With the serious nature of the allegations made by Master
Deputy Van Camp should have taken steps to verify and obtain a written statement from her.
In William Clark's statement, he told Chieflnvestigator Barry Brooke that Master Deputy Van
Camp "seemed to have a problem with Tony Henderson and that it seemed to be important to Deputy
Van Camp that the harassing phone call be reported."
In Cheryl Gooden's statement, she alleged that David Alverson obtained this report from one of
his contacts and that the report was part of the campaign (Against James Owens) with David Alverson,
Tom McGuire, and Gene Polk.
In Master Deputy Jeff Van Camp's statement, he indicates that he printed a copy of Escambia
County Sheriffs Office Offense Report ECS0110FF030535 because, "I know situations like this
sometimes, Offense Reports disappear here at the Sheriffs Office. I printed it up for my own purpose."
Master Deputy Jeff Van Camp was unable to provide an example of an Offense Report disappearing.
These facts and the details included in this investigation bring into question, Master Deputy Jeff
Van Camp's motives for generating Escambia County Sheriffs Office Offense Report
ECSO 11 OFF030535 making it appear that the Offense Report was generated and released for political
reasons.

4. 213.3 Improper Conduct Offenses, (57)Untruthfulness in an Official Inquiry: Members will not
knowingly make false statements to a supervisor, an internal affairs investigator, or to any official of
a government agency during an official or administrative inquiry (see G.O. 224 - Investigations -
Employee Misconduct (Sworn) I G.O. 225 - Investigations - Internal Affairs).

During his interview, Master Deputy Van Camp stated that he printed a copy of Escambia
County Sheriffs Office Offense Report ECSOll OFF030535. The evidence gathered during this
investigation clearly shows that the copy that Master Deputy Jeff Van Camp possessed was the only
copy of Escambia County Sheriffs Office Offense Report ECSOI IOFF030535 that could have been
placed on the website LEOAFFAIRS.COM. During his interview, Master Deputy Jeff Van Camp was
adamant that he did not release the report, give the report to anyone, post the report on the internet, or
have any knowledge of anyone releasing the report or posting the report on the internet. The evidence
in this investigation overwhelmingly shows that Master Deputy Jeff Van Camp is the only person that
could have been involved in the release of the Offense Report. Master Deputy Jeff Van Camp's denial
of his involvement and or failure to disclose his involvement in the release of Escambia County
Sheriffs Office Offense Report ECSOl l OFF030535 prove that he was untruthful during his Internal
Affairs Interview.

The term sustained is defined in General Order 225 as meaning: A finding or conclusion that an
allegation is supported by a preponderance of evidence.

SUMMARY OF COMPLAINT:

On October 27, 2011, at approximately 0646 hours, a post was made to the website

LEOAFFAIRS.com under the heading "FEMALE LEO'S READ BEFORE ENTERING

PUBLIC DEFENDERS OFFICE." An un-redacted copy of Escambia County Sheriffs Office

Offense Report number ECS0110FF033535 was included in this post. On November 9, 2011,

Commander Darlene Dickey requested a review of this Offense Report to see if it could be

determined how the offense report was released. Based on my review of this incident, on

November 16, 2011, Sheriff David Morgan directed an Internal Investigation with Master

Deputy Jeffery Van Camp as the subject officer.

DETAILS OF INVESTIGATION:

On November 9, 2011, I printed a twenty page copy of the LEOAFFAIRS.COM Topic

"FEMALE LEO'S READ BEFORE ENTERING PUBLIC DEFENDERS OFFICE." I also

captured and printed a screenshot of Escambia County Sheriffs Office Offense Report

ECS0110FF030535 as it appeared on the website LEOAFFAIRS.COM. (See Tab "0")

A line by line inspection of Escambia County Sheriffs Office Offense Report

ECS0110FF030535 as it appears on LEOAFFAIRS.COM revealed the following facts:

• The Offense Report is printed in the new CTS America format. (CTS America Software
permits the printing of Offense Reports in two formats. The new format and the old
format which is referred to as the Legacy format.)

• The Location of Occurrence has been redacted from the Location of Occurrence Section
and the Narrative Section of the Offense Report. This information appears to have been
redacted using a black marker.

• The names of the people listed in the Persons Section, the "Reporting Person" and the
"Other Person" has been redacted from the Persons Section and Narrative Section of the
Offense Report. This information appears to have been redacted using a black marker.

• The name "Dr. Dave Miller" has been redacted from the Narrative Section of the Offense
Report. This information appears to have been redacted using a black marker.

• The name "Tony Henderson" has been redacted once from the Narrative Section of the
Offense report however the name still appears in this section. This information appears
to have been redacted using a black marker.

• Tony Henderson is the only name that is displayed in the Offense Report as it appears on
the website LEOAFF AIRS.COM.

• There is no supervisor approval in the Approving Supervisor Section of the Offense


Report.

I printed an Event Log for Escambia County Offense Report ECS0120FF030535. This

log documents events associated with this Offense Report when accessed through the CTS

America Offense Report Program, Case Management Program, and some accesses thru the

Master Name Index Program. A review of this Event Log shows the following: (See Tab "Q")

• Deputy Jeff Van Camp contacted Report Taker Lorie Trilone at 1530 hours on
10/21/2011 to initiate Offense Report ECSOl 1OFF030535.

• Report Taker Lorie Trilone worked on completing the Offense Report on 10/21/2011
between 1530 hours and 1611 hours.

• On 10/21/2011 at 1531 hours, Report Taker Darilyn Miller accessed Offense Report
ECS0110FF030535.

• On 10/21/2011 at 1555 hours, Detention Deputy Randall Bradshaw accessed Offense


Report ECS0110FF030535.

• On 10/21/2011 at 1613 hours, Sergeant Lisa Dixon accessed Offense Report


ECS0110FF030535.

• On 10/21/2011 at 1622 hours, Commander Ricky Shelby accessed Offense Report


ECS0110FF030535.

• On 10/21/2011 at 1655 hours, Sergeant Lisa Dixon accessed Offense Report

• On 10/21/2011 at 1802 hours and 1804 hours, Detention Sergeant Alan Miller accessed
Offense Report ECS0110FF030535.

• On 10/21/2011 between 2012 hours and 2020 hours, Senior Report Taker Lori Scott
accessed, updated, and finished Offense Report ECS0110FF030535. It Offense Report
was marked as finished by Senior Report Taker Lori Scott at 2020 hours.

• On 10/22/2011 at 0655 hours, Sergeant Lisa Dixon accessed and "Approved" Offense
Report ECS0110FF030535.

I noted the following significant facts from my review of the Offense Report that was

posted on the website LEOAFF AIRS.COM, a copy of the approved version of Offense Report

ECS0110FF030535 (See Tab "S"), and the Event Log associated with Offense Report

ECS0110FF030535:

• The Event Log did not log the printing of Offense Report ECS0110FF030535.

• The accesses by Sergeant Lisa Dixon are consistent with that of a supervisor attempting
to review and approve a report. Sergeant Lisa Dixon is Deputy Van Camp's Sergeant
and is responsible for approving his reports.

• Senior Report Taker Lori Scott appeared to make corrections to the Offense Narrative
Section of Offense Report ECS0110FF030535. In the event stamps on 10/21/2011
between 2012 hours and 2020 hours, Scott is logged twice viewing the narrative and then
updating the narrative. This action is consistent with corrections being made to the
narrative of Offense Rep011 ECS0110FF030535.

• A line by line comparison of the Offense Report that appeared on the website
LEOAFFAIRS.COM with a copy of the "Approved" Offense Report revealed no
changes.

• Based on the above facts I determined that the Offense Report that was posted on the
website LEOAFFAIRS.COM was printed between the 2020 hours on 10/21/2011 when
Senior Report Taker Lori Scott "Finished" the report and 0655 hours on 10/22/2011
when Sergeant Lisa Dixon "Approved" the report.

I researched CTS America's Case Management Program, Offense Report Program, and

Master Name Index Program to determine how an Offense Report could be printed but not show

up as a "Print" on the Event Log. Through this research I determined the following facts:

• When a report is printed in the new CTS America Offense Report format, no "Print" is
recorded in the Event Log for the report.

• When a report is printed in the new CTS America Offense Report format and that report
was accessed through either the Case Management Program or the Offense Report
Program, a "View" is logged in the Event Log for that Offense Report.

• When a report is printed in the CTS America Legacy Offense Report format from any of
the programs, it is logged a "Print" in the Event Log for the report.

• A person with access to the system could access a Master Name Index Record, then
access an Offense Report associated with that record, and then print that Offense Report
in the new CTS America Offense Report format without any event being recorded in the
Event Log for that Offense Report.

I obtained a copy of the Master Name Index Record Tables for both

(ECS011MNI012531) and William Douglas Clark (ECS011MNI008318). The Master Name

Index Record Table logs all accesses to a Master Name Index Record. and

William Clark are the only two persons associated with Offense Report ECS0110FF030535 and

therefore one of these Master Name Index Records had to have been accessed in order to print a

copy of Offense Report ECS0110FF030535 in the new CTS America format without registering

an event on the Event Log for that Offense Report. When I reviewed the accesses that were

made to these Master Name Index Records on 10/21/2011 and 10/22/2011, I learned the

following: (See Tab "P")

• On 10/21/2011 at 1232 hours and 1237 hours, Deputy Jeff Van Camp accessed the
Master Name Index Record of (ECS011MNI012531). This access
would have been made while Deputy Van Camp was on scene and receiving information
about this incident.

• On 10/21/2011 between 1532 hours and 1534 hours, Report Taker Lorie Trilone accesses
the Master Name Index Record of (ECS011MNI012531) eight times.
This access occurred while Deputy Van Camp was calling in the report to Trilone.

• On 10/21/2011 between 1531 hours and 1534 hours, Sergeant Lisa Dixon accesses the
Master Name Index Record of William Clark (ECS099MNI008318) four times. This
access occurred while Deputy Van Camp was calling in the report to Trilone.

• On 10/22/2011 at 0650 hours and 0652 hours, Deputy Jeff Van Camp accessed the
Master Name Index Record of (ECSO 11MNIO12531 ).

• There were no other accesses to the Master Name Index Record of


(ECS011MNI012531) of William Clark (ECS009MNI008318).

Based on the above facts, Deputy Van Camp's accesses of Master

Name Index Record on 10/22/2011 at 0650 hours and 0652 hours are the only accesses from

which Offense Report ECS0110FF030535 could have been printed without creating an event on

the Event Log for Offense Report ECS0110FF030535.

On November 16, 2011, Sheriff David Morgan ordered an Internal Investigation into the

release of Offense Report ECS0110FF030535. Deputy Jeff Van Camp is the subject of this

Internal Investigation. (See Tab "C")

I received a copy of an email exchange between Escambia County Sheriffs Office

Criminal Records Manager Sherry Walker and Pensacola News Journal Reporter Katherine

McFarland and a copy of Escambia County Sheriffs Office Offense Report ECS0110FF030535

and CAD Report ECSO 11 CAD230946 in redacted form as they were released to the Pensacola

News Journal. (See Tab "U") In the email exchange, McFarland indicates that she has a copy of

Escambia County Sheriffs Office Offense Report ECS0110FF030535 which contains part of

the narrative. McFarland indicates that parts of the copy that she received is blurry and requests

another copy of Escambia County Offense Report ECS0110FF030535 and a copy of the

associated CAD Notes. Walker replies to McFarland that she can send her a copy of the report

but tells McFarland that, "The narrative and other identifying info will have to be redacted as the

case is active."

I contacted Pensacola News Journal Reporter Katherine McFarland by phone and

requested an interview. McFarland declined my request for an interview.

On November 16, 2011, I printed out a copy of an article titled "She's not a stripper, pill-

head or deadbeat" that appeared on Ricksblog.biz. (See Tab "V") In this article,

and William Clark alleged that they did not file a report about Tony Henderson with the

Escambia County Sheriff's Office. Clark insisted that he only "shared in confidence," his

concerns over Tony Henderson calling with a "Deputy friend." indicated that

she talked over that phone with a Deputy who was a friend of Clark's and that she was never told

that she was making a formal statement or that her statement was part of an investigation.

also denied that Tony Henderson ever asked her to perform sexual favors (as reported in

Offense Report ECS0110FF030535).

I contacted William Clark by phone and requested and interview with him, Clark

declined my interview request.

I contacted by phone and requested and interview with her,

declined my interview request.

On November 16, 2011, I interviewed Escambia County Sheriffs Office Information

Systems Manager Sean Fretenborough, Escambia County Sheriffs Office Systems Analyst

Kevin Fralick, and CTS America Director of Database Administration Ulysses Hill. Sean

Fretenborough, Kevin Fralick, and Ulysses Hill were placed under oath prior to the interview.

All three reviewed and signed both the Perjury and Oath Acknowledgement and the

Confidentiality Notice. (See Tab "D")

I interviewed Fretenborough, Fralick, and Hill to have them review my research as they

are more familiar with the CTS America software. Through this interview I validated my

conclusion that Deputy Van Camp was the only person who could have printed Offense Report

ECS0110FF030535. I also learned that thru the "HostName" column of the Master Name

Index Table, I could identify the computer that Deputy Van Camp logged in to on 10/22/2012 at

0650 hours and 0652 hours. This computer is identified as ECS0-02546. Fretenborough

contacted me and advised that this computer is located in the Patrol Office next to a printer.

On November 16, 2011, I interviewed Escambia County Sheriffs Office 151 Lieutenant

Steve Hardy. Lieutenant Hardy was placed under oath prior to the interview. Lieutenant Hardy

reviewed and signed both the Perjury and Oath Acknowledgement and the Confidentiality

Notice. (See Tab "E")

On 10/24/2011 at 0829 hours, Lieutenant Hardy printed a copy of the Offense Report

ECS0110FF030535 in the CTS America Legacy Format. (See Tab "R") Lieutenant Hardy

printed the report after it was approved Sergeant Dixon. Lieutenant Hardy advised that he

printed the report because he had questions about the report. These questions were, why the

report was titled an information report instead of a criminal act and why Deputy Van Camp

didn't ask if she has any corroborating evidence such as text messages or voice mails.

On November 18, 2011, I interviewed Escambia County Sheriffs Office Report Taker

Lorie Trilone. Trilone was placed under oath prior to the interview. Trilone reviewed and

signed both the Perjury and Oath Acknowledgement and the Confidentiality Notice. (See Tab

"F")

Lorie Trilone was the report taker who Deputy Van Camp contacted to complete Offense

Report ECS0110FF030535. A copy of Deputy Van Camp's telephone call to Trilone is

included with this case file. (See Tab "Y") Trilone recalled taking the report but could not recall

any specific information about taking the report.

On November 18, 2011, I interviewed Escambia County Sheriffs Office Sergeant Lisa

Dixon. Sergeant Dixon was placed under oath prior to the interview. Sergeant Dixon reviewed

and signed both the Perjury and Oath Acknowledgement and the Confidentiality Notice. (See

Tab "G")

Sergeant Dixon is Deputy Jeff Van Camp's Sergeant and was the supervisor who was on

duty on October 21, 2011, when Deputy Van Camp completed Escambia County Sheriffs Office

Offense Report ECS0110FF030535. Sergeant Dixon accessed the Master Name Index Record

of William Clark (ECS099MNI008318) on 10/2112011 between 1531 hours and 1534 hours.

Sergeant Dixon advised that her access of William Clark's Master Name Index Record had

nothing to do with Offense Report ECS0110FF030535, but rather she was assisting Lieutenant

Joye with the investigation of Citizen Complaint number C2011-104 and that the access was

purely coincidental. Sergeant Dixon indicated that she accessed William Clark' s Master Name

Index Record to view Escambia County Sheriffs Office Offense Report ECS0110FF026928

which involved the theft of an iPhone from William Clark.

Sergeant Dixon advised that later in the day she accessed Escambia County Sheriffs

Office Offense Report ECS0110FF030535 because she was approving her shift's reports.

Sergeant Dixon advised that when she accessed the report that there were still errors in the report

and that it was "in gray" and not ready for approval. I checked the Event Log for Offense Report

ECS0110FF030535 and observed that Sergeant Dixon accessed the report at 1543 hours, 1613

hours, 1655 hours, and 1753 hours on 10/21/2011. (See Tab "Q")

Sergeant Dixon advised that after she became aware of this incident she sent an email to

Lieutenant Susie Joye, Lieutenant Steve Hardy, Colonel Butch Lewis, and Commander Ricky

Shelby to advise them of the incident. Sergeant Dixon advised that she did not send the normal

Unusual Incident email due to the sensitive nature of the incident. I asked Sergeant Dixon to

locate that email and forward me a copy, which she did. (See Tab "X") This email was sent on

October 21, 2011, at 1620 hours.

On October 21, 2011, at 1622 hours, Commander Ricky Shelby accessed Escambia

County Sheriffs Offense Report ECS0110FF030535. Commander Shelby's access would be

consistent with him reviewing the Offense Report. Sergeant Dixon was copied on an email that

Commander Shelby sent on October 21, 2012, at 1625 hours. In this email Commander Shelby

forwarded Sergeant Dixon's email to Captain Bruce Wood and Lieutenant Carl Jones advising

them that to review Escambia County Sheriffs Office Offense Report ECS0110FF030535.

Sergeant Dixon included Commander Shelby's email with hers. (See Tab "X")

I asked Sergeant Dixon to do a comparison of the approved version of Offense Report

ECS0110FF030535 with the version that appeared on the website LEOAFFAIRS.COM. Based

on this comparison, Sergeant Dixon concluded that they are identical.

On November 19, 2011, I interviewed Escambia County Sheriffs Office Senior Report

Taker Lori Scott. Scott was placed under oath prior to the interview. Scott reviewed and signed

both the Perjury and Oath Acknowledgement and the Confidentiality Notice. (See Tab "H")

On October 21, 2011, between 2012 hours and 2012 hours Lori Scott accessed Escambia

County Sheriffs Office Offense Report ECSOl 1OFF030535 causing six Event Stamps to appear

on the Event Log for this Offense Report. These Event Stamps were as follows: (See Tab "Q")

Lori Scott recalled Offense Report ECSOl 1OFF030535 but could not recall if she had to

make corrections to the report. Lori Scott indicated that it is common for her to have to make

corrections to Lorie Trilone' s reports.

On December 15, 2011, I interviewed Escambia County Sheriffs Office Commander

Rickey Shelby. Commander Shelby was placed under oath prior to the interview. Commander

Shelby reviewed and signed both the Perjury and Oath Acknowledgement and the

Confidentiality Notice. (See Tab "I")

On October 21 , 2011, at 1622 hours, Commander Shelby accessed Escambia County

Sheriffs Offense Report ECS0110FF030535. Commander Shelby stated that he received an

Unusual Incident email from Sergeant Dixon on October 21 , 2011 , at approximately 1620 hours.

The email was about Offense Report ECS0110FF030535. He read the email then reviewed the

Offense Report. Commander Shelby recalled that the report had not been approved and or

forwarded. Commander Shelby said that he then forwarded Sergeant Dixon's email to Captain

Bruce Wood and Lieutenant Carl Jones at 1625 hours, so that they could review the Offense

Report due to the serious nature of the allegations made against a public figure. Commander

Shelby brought a copy of his email with him to the interview. (See Tab "W")

On December 16, 2011, I mailed a letter to Tony Henderson Esquire requesting an

interview with him. (See Tab "T") After this letter was mailed, I was contacted by John Beroset

Esquire who advised that he was representing Tony Henderson. Beroset told me that Henderson

wished to provide me with information regarding this incident. Over the next seven month I

spoke with Beroset on numerous occasions. On each occasion, Beroset told me that Henderson

wanted to provide me with information regarding this incident, but he did not know when it

would occur. On July 5, 2012, I sent another letter to Tony Henderson, Esquire advising him

that ifl did not receive his information by July 20, 2012, I would not be able to include it as part

of my investigation. (See Tab "hh") I never received any information from Henderson.

On December 29, 2011 , I interviewed Escambia County Sheriffs Office Support

Services Manager Lynda Aiken. Lynda Aiken was placed under oath prior to the interview.

Lynda Aiken reviewed and signed both the Perjury and Oath Acknowledgement and the

Confidentiality Notice. (See Tab "J")

Lynda Aiken is the supervisor over Report Takers Lorie Trilone and Lori Scott and is

familiar with the CTS America Software. Prior to the interview, I asked Lynda Aiken to review

the event log for Escambia County Sheriffs Office Offense Report ECS0110FF030535. (See

Tab "Q") Based on Lynda Aiken's review of this Event Log, she concluded that Lori Scott

accessed Offense Report ECS0110FF030535 and made corrections to the Narrative Section of

the Offense Report. Lynda Aiken described Lori Scott as a very meticulous, good supervisor,

who frequently found errors (Grammar, Punctuation, and Spelling) and made corrections to

Offense Reports.

I asked Lynda Aiken to make comparison of the approved version of Offense Report

ECS0110FF030535 with the version that appeared on the website LEOAFFAIRS.COM. Based

on this comparison, Lynda Aiken concluded that they are identical.

On December 30, 2011, I interviewed Escambia County Sheriffs Office Detention

Sergeant Alan Miller. Sergeant Miller was placed under oath prior to the interview. Sergeant

Miller reviewed and signed both the Perjury and Oath Acknowledgement and the Confidentiality

Notice. (See Tab "K")

On October 21, 2011, Sergeant Miller accessed Escambia County Sheriffs Office

Offense Report ECS0110FF030535 at 2002 hour and 2004 hours. This access occurred prior to

any corrections that were made by Senior Report Taker Lori Scott and also prior to the Offense

Report being marked as "Finished" by Scott. Sergeant Miller advised that is common for him to

read Offense Reports during down time in Detention. Sergeant Miller indicated that he does this

because he does not write a lot of criminal reports as a supervisor in Detention and reading the

reports gives him good examples of what needs to be in them. Sergeant Miller advised that he

never prints or shares the information in the reports because that would be a violation of the

LAN User Agreement and a violation of Escambia County Sheriffs Office Policy.

On February 13, 2012, I sent Deputy Van Camp a letter notifying him of the internal

investigation and directing him to schedule an interview. (See Tab "L")

On February 24, 2012, I interviewed Escambia County Sheriffs Office Master Deputy

Jeffery Van Camp. Prior to the interview Deputy Van Camp was placed under oath. Deputy

Van Camp reviewed and signed the Employee Advice of Rights Form, the Perjury and Oath

Acknowledgement Form, and the Confidentiality Notice. Deputy Van Camp was accompanied

by his Fraternal Order of Police Representative, Steve McCann. McCann also signed the

Confidentiality Notice. Prior to the interview, Deputy Van Camp and Steve McCann reviewed

all of the evidence and statements that are associated with this investigation. Deputy Van

Camp's statement is thirty-five pages and should be reviewed in its entirety. (See Tab "M")

Deputy Van Camp stated that he was dispatched to Sonny Reid Bail Bonds but could not

recall where he was when dispatch contacted him. Deputy Van Camp stated, "I don't know. I

was in district one and ten, where I work." I asked Deputy Van Camp if he had any

conversations with William Clark about this incident prior to responding to Sonny Reid Bail

Bonds. Deputy Van Camp replied, "No." I clarified the question asking him if Clark spoke to

him on a separate occasion about the incident. Deputy Van Camp told me that Clark asked him

how to file a complaint on an attorney, but did not mention any names.

Once on scene, Deputy Van Camp advised that he spoke with William Clark about a

harassing phone call complaint with Tony Henderson of the Public Defender's Office being the

suspect. Deputy Van Camp advised that he then spoke with by phone and "she

basically told me what I have in my report."

Deputy Van Camp wrote in his report that he placed under oath. I

asked Deputy Van Camp if he knew prior to taking the report from her. Deputy Van

Camp advised that he did not know prior to taking the report. I then asked Deputy Van

Camp if he had any idea who he placed under oath. Deputy Van Camp replied, "Other than her

identifying herself to me over the telephone as ." I asked Deputy Van Camp if

he was aware that before you place someone under oath you have to positively identify who that

person is. Deputy Van Camp replied, "Well I tried to as best I could. She identified who she

was."

I asked Deputy Van Camp if he knew anything about the release of Escambia County

Sheriffs Office Offense Report ECS0110FF030535 or how it showed up on the website

LEOAFFAIRS.COM. Deputy Van Camp replied, "No sir." I then took time to explain the

investigation to Deputy Van Camp and explained how he became the subject of this

investigation. I then explained to Deputy Van Camp that he could be the only person who could

have printed and released the report. Deputy Van Camp told me that it is not uncommon for him

to print a report, but that he did not release it. I asked Deputy Van Camp if he printed a copy of

Offense Report ECS0110FF030535 . Deputy Van Camp replied, "Yes." Deputy Van Camp

could not recall when he printed the report but believed that he printed it in the Patrol Office. I

asked Deputy Van Camp if he still had the copy of the report that he printed. Deputy Van Camp

advised that he did not because he took everything out of his patrol vehicle and shredded it when

he went out on stress leave in December or January. During his interview, Deputy Van Camp

was adamant that he did not release the report, give the report to anyone, post the report on the

internet, or have any knowledge of anyone releasing the report or posting the report on the

internet. Later in the interview, Deputy Van Camp did agree that he had the only copy of the

report that could have appeared on the internet.

During his interview, Deputy Van Camp stated that it is not uncommon for him to print

reports. I asked Deputy Van Camp if this was when he was in Crime Stoppers or since he has

gone back to Patrol. Deputy Van Camp advised that he printed numerous reports while in Crime

Stoppers and now that he is back in Patrol he prints high profile cases or domestic violence case

because he likes to keep a record of those cases. I explained that those prints would be logged

and that since he has been back to Patrol he has only accessed ten Offense Reports. I then asked

how many of those reports he printed, Deputy Van Camp could not recall. Deputy Van Camp

advised that most of domestic violence he would print but he did not know if he printed any of

the ten reports that he accessed since he went back to patrol. I asked Deputy Van Camp why he

printed this particular report. Deputy Van Camp replied, "I know situations like this sometimes,

Offense Reports disappear here at the Sheriffs Office. I printed it up for my own purpose." I

asked Deputy Van Camp for examples. Deputy Van Camp replied, "That's what I've always

been told. I don't know that for a fact, that's what I've been told." I pointed out that he was

concerned about the report disappearing and that he shredded the report. Deputy Van Camp

responded that once it hit the newspapers there was no reason not to shred it. Deputy Van Camp

denied having anything to do with it appearing in the newspapers.

I asked Deputy Van Camp about his relationship with David Alverson. Deputy Van

Camp advised that he and David Alverson were good friends, that he worked with him at the

Pensacola Police Department, and that he has known Alverson for twenty-five plus years. I

asked Deputy Van Camp if Alverson has ever been in is patrol vehicle or if Alverson has access

to his patrol vehicle. Deputy Van Camp advised that Alverson had never been in his vehicle and

that he did not have access to his vehicle. I also asked Deputy Van Camp if Alverson could have

gotten he report out of his patrol vehicle. Deputy Van Camp replied, "No."

At the conclusion of Deputy Van Camp's interview, I advised him that he has the

opportunity to change or correct his testimony up until my report is final. Deputy Van Camp

indicated that he understood.

On February 29, 2012, Lieutenant Walter Matthews and I met with Assistant State

Attorney Greg Marcille and Chief Investigator Barry Brooke from the State Attorney's Office.

During this meeting we turned over the evidence associated with this investigation so that it

could be reviewed to determine if any criminal violations occurred. Deputy Van Camp's

statement and any information gleaned from Deputy Van Camp's statement were not turned over

to the State Attorney's Office.

On April 23 2012, I received an email from Escambia County Sheriff's Office General

Counsel Gerald Champagne. Attached to this email was a memorandum from State Attorney's

Office Chief Investigator Barry Brooke dated April 18, 2012. (See Tab "fP') In his

memorandum, Chief Investigator Brooke details his interviews with William Clark and

Chief Investigator Brooke reported that William Clark told him that Deputy Van Camp

sometimes used an extra office at Sonny Reid Bail Bonds and that on one occasion while at this

location, he complained to Deputy Van Camp about some harassing phone calls that his

girlfriend, , was receiving from her Public Defender, Tony Henderson. Chief

Investigator Brooke reported that Clark told him that Deputy Van Camp instructed him (Clark)

to call the Escambia County Sheriffs Office and request a Deputy to take a report on the

harassing phone calls and that he (Deputy Van Camp) would take the call. Chief Investigator

Brooke also reported that Clark told him that Deputy Van Camp seemed to have a problem with

Tony Henderson and that it seemed to be important to Deputy Van Camp that the harassing

phone calls be reported.

Chief Investigator Brooke reported that told him that she did not want

to be involved with an Escambia County Sheriffs Office or Florida Department of Law

Enforcement Investigation. Chief Investigator Brooke reported that told him that her

boyfriend, William Clark, contacted the Escambia County Sheriff's Office without her

permission. Chief Investigator Brooke reported that told him that when she talked to

Deputy Van Camp, he did not tell her that he was completing a report and that she thought that

she was speaking to him unofficially as a friend of Clark's.

On March 23, 2012, I printed out a copy of a picture showing David Alverson, Bruce

Miller, and an unknown person. The three were standing next to a large Bruce Miller for Public

Defender sign. (See Tab "gg") This picture was located on the internet.

On May 1, 2012, I interviewed Cheryl Gooden, Esquire. Gooden was placed under oath

pnor to the interview. Gooden reviewed and signed both the Perjury and Oath

Acknowledgement and the Confidentiality Notice. (See Tab "aa")

Cheryl Gooden is the ex-wife of David Alverson. Gooden and Alverson worked together

at the Public Defender's Office during the time that James Owens, Tony Henderson, and Kelly

Richards were running for the Office of Public Defender in 2007. Gooden stated that during this

time, David Alverson and two others went to Tallahassee and obtained a copy of a closed Florida

Department of Law Enforcement Investigation in which James Owens and Tony Henderson were

investigated for sexual battery. (Part of this report appeared on the website LEOAFFAIRS.COM

with Escambia County Sheriffs Office Offense Report ECS0110FF030535) Gooden described

David Alverson as being obsessed with James Owens and Tony Henderson saying, "He has been

doing everything to get some kind of dirt or concoct some kind of dirt on James Owens or Tony

Henderson to ruin them because he felt that they ruined his life." Gooden also alleged that David

Alverson has solicited people to make false allegations of a sexual nature against James Owens

and Tony Henderson. Gooden also alleged that David Alverson talked about sitting on top of a

building across from the Public Defender's Office with a rifle and taking James Owens out.

I showed Cheryl Gooden a copy of Escambia County Sheriffs Office Offense Report

ECSOl 1OFF030535 and asked her if she had ever seen it before. Gooden advised that she was

provided an un-redacted copy of the Offense Report by someone who obtained it from David

Alverson. Gooden indicated that it was her understanding that David Alverson obtained that

report through one of his contacts, either the person who he is dating now (Pensacola Police

Investigator Lisa Cutler) or someone else. Gooden also indicated that it was her understanding

that the report was part of the Campaign (Against James Owens) with David Alverson, Tom

McGuire, Gene Polk, and one other person.

On May 2, 2012, I received a copy of an all users email that Lieutenant Stull sent out to

members of the Pensacola Police Department on June 5, 2008. (See Tab "bb") I learned of this

email while coordinating the interview of Pensacola Police Department Investigator Lisa Cutler.

Lieutenant Stull indicated that a conversation that he had with David Alverson was the reason

the email was sent out. In his email, Lieutenant Stull reminded members of the Pensacola Police

Department about the upcoming elections and cautioned them about any request for information

that they receive. Lieutenant Stull also reminded members of the Pensacola Police Department

that accessing records from the Criminal Justice System Database is logged and the release of

those records is governed by strict guidelines and policies.

On May 9, 2012, I interviewed Pensacola Police Department Lieutenant Randy Stull.

Lieutenant Stull was placed under oath prior to the interview. Lieutenant Stull reviewed and

signed both the Perjury and Oath Acknowledgement and the Confidentiality Notice. (See Tab

"cc")

Lieutenant Randy Stull stated that on June 5, 2008, he was contacted by David Alverson

who was a former employee of the Pensacola Police Department. Lieutenant Stull indicated that

David Alverson asked him about an investigation involving James Owens where Owens had

been sexually harassing another former employee of the Pensacola Police Department.

Lieutenant Stull advised that he became suspicious of David Alverson when Alverson asked him

to "run some people for him." Lieutenant Stull advised that he believed Alverson was asking

him to run checks on people in the Criminal Justice Database. Lieutenant Stull advised that he

sent the email because he was concerned that Alverson would contact someone else in the

department and request the information.

On May 10, 2012, I interviewed Kelly Richards, Esquire. Richards was placed under

oath prior to the interview. Richards reviewed and signed both the Perjury and Oath

Acknowledgement and the Confidentiality Notice. (See Tab "dd")

I asked Kelly Richards what she knew about Escambia County Sheriff's Office Offense

Report ECS0110FF030535. Richards advised that she had heard about the report but that she

had never seen the report or had it in her possession.

On June 15, 2012, I interviewed Pensacola Police Department Investigator Lisa Cutler.

Investigator Cutler was placed under oath prior to the interview. Investigator Cutler reviewed

and signed both the Perjury and Oath Acknowledgement and the Confidentiality Notice. (See

Tab "ee")

Pensacola Police Department Investigator Cutler has been dating David Alverson since

October or November of 2011. Investigator Cutler stated that she has never seen a copy of

Escambia County Sheriffs Office Offense Report ECS0110FF030535 and that Alverson has

never discussed the report with her.

I contacted David Alverson by phone and requested an interview. Alverson referred me

to his Attorney, Patrice Cashwell. I contacted Cashwell and scheduled an interview with

Alverson on July 3, 2012. On July 2, 2012, Cashwell left a message on my voicemail advising

that Alverson was out of town on vacation and that we would have to reschedule. When I

attempted to reschedule the interview, Cashwell advised that Alverson would not appear without

a subpoena.

On July 17, 2012, I contacted Deputy Jeff Van Camp and advised him that this

investigation was coming to a conclusion. At this time, I advised Deputy Van Camp that he

could review the evidence that I have gathered since his interview.

On July 17, 2012, at approximately 1305 hours, Deputy Van Camp arrived at the Internal

Affairs Office with his Fraternal Order of Police Representative, Paul Johns. Deputy Van Camp

and Johns reviewed the additional evidence togeth~r.

After reviewing the evidence, Deputy Van Camp was provided an opportunity to amend

his statement. Deputy Van Camp declined.

FINDINGS:

The following findings are SUSTAINED regarding Master Deputy Jeffery Van Camp:

1. 213.2 Neglect of Duty Offenses, (23) Failure to Follow General Orders, Directives, and
Sheriff's Orders: Members will adhere to General Orders, Sheriffs orders, policies, and
directives and will faithfully execute all duties and responsibilities of their assigned position.

• General Order 232 Public Records, 232.1 Duties and Responsibilities (4) Every
member of the Sheriff's Office is responsible for the security of records in their
possession.

During his interview, Master Deputy Van Camp stated that he printed a copy of

Escambia County Sheriff's Office Offense Report ECS0110FF030535. The evidence gathered

during this investigation clearly shows that the copy that Master Deputy Jeff Van Camp

possessed was the only copy of Escambia County Sheriff's Office Offense Report

ECS0110FF030535 that could have been placed on the website LEOAFFAIRS.COM. The fact

that this Offense Report was posted on this website proves that Deputy Van Camp was not

responsible for the security of the record that was in his possession.

2. 213.3 Improper Conduct Offenses, (26) Dissemination of Information: Members will


treat the official business of the Sheriffs Office as confidential. Information regarding
official business will be disseminated only to those for whom it is intended and in accordance
with established procedures. Members may remove or copy official records or reports only
in accordance with established procedures.

During his interview, Master Deputy Van Camp stated that he printed a copy of

Escambia County Sheriff's Office Offense Report ECS0110FF030535. The evidence gathered

during this investigation clearly shows that the copy that Master Deputy Jeff Van Camp

possessed was the only copy of Escambia County Sheriff's Office Offense Report

ECSOI IOFF030535 that could have been placed on the website LEOAFFAIRS.COM. The fact

that this Offense Report was posted on this website proves that Deputy Van Camp did not treat

the official business of the Sheriffs Office as confidential and that he failed to disseminate the

Offense Report in accordance with established procedures.

3. 213.3 Improper Conduct Offenses, (69) Unbecoming Conduct: Personnel shall conduct
themselves at all times, both on and off duty, in such a manner as to reflect favorably on the
Sheriffs Office. Conduct unbecoming shall include that which brings the Sheriffs Office
into disrepute or reflects discredit upon the personnel as a member of the Sheriffs Office, or
that which impairs the operation or efficiency of the Sheriffs Office or personnel.

Master Deputy Jeff Van Camp's conduct in this incident is questionable, brings the

Escambia County Sheriffs Office into disrepute, and reflects discredit upon the members of the

Escambia County Sheriffs Office.

In Master Deputy Jeff Van Camp's report, he wrote that he contacted

by phone and obtained a sworn statement from her. Deputy Van Camp had no means of

verifying who he was taking a sworn statement from. With the serious nature of the allegations

made by Master Deputy Van Camp should have taken steps to verify and

obtain a written statement from her.

In William Clark's statement, he told Chief Investigator Barry Brooke that Master

Deputy Van Camp "seemed to have a problem with Tony Henderson and that it seemed to be

important to Deputy Van Camp that the harassing phone call be reported."

In Cheryl Gooden's statement, she alleged that David Alverson obtained this report from

one of his contacts and that the report was part of the campaign (Against James Owens) with

David Alverson, Tom McGuire, and Gene Polk.

In Master Deputy Jeff Van Camp's statement, he indicates that he printed a copy of

Escambia County Sheriffs Office Offense Report ECS0110FF030535 because, "I know

situations like this sometimes, Offense Reports disappear here at the Sheriffs Office. I printed it

up for my own purpose." Master Deputy Jeff Van Camp was unable to provide an example of an

Offense Report disappearing.

These facts and the details included in this investigation bring into question, Master

Deputy Jeff Van Camp's motives for generating Escambia County Sheriffs Office Offense

Report ECS0110FF030535 making it appear that the Offense Report was generated and

released for political reasons.

4. 213.3 Improper Conduct Offenses, (57)Untruthfulness in an Official Inquiry: Members


will not knowingly make false statements to a supervisor, an internal affairs investigator, or
to any official of a government agency during an official or administrative inquiry (see G.O.
224 - Investigations - Employee Misconduct (Sworn) I G.O. 225 - Investigations - Internal
Affairs).

During his interview, Master Deputy Van Camp stated that he printed a copy of

Escambia County Sheriffs Office Offense Report ECS0110FF030535. The evidence gathered

during this investigation clearly shows that the copy that Master Deputy Jeff Van Camp

possessed was the only copy of Escambia County Sheriffs Office Offense Report

ECS0110FF030535 that could have been placed on the website LEOAFFAIRS.COM. During

his interview, Master Deputy Jeff Van Camp was adamant that he did not release the report, give

the report to anyone, post the report on the internet, or have any knowledge of anyone releasing

the report or posting the report on the internet. The evidence in this investigation

overwhelmingly shows that Master Deputy Jeff Van Camp is the only person that could have

been involved in the release of the Offense Report. Master Deputy Jeff Van Camp's denial of

his involvement and or failure to disclose his involvement in the release of Escambia County

Sheriffs Office Offense Report ECS0110FF030535 prove that he was untruthful during his

Internal Affairs Interview.
