JUDICIAL AFFIDAVIT OF WITNESS

I, RAIDA SALEM PASANDALAN, 57 yrs. old, female, Filipino, housewife and

presently residing at Brgy. Bulod, Datu Paglas, Maguindanao. Under Oath state
and execute this Judicial Affidavit in English which is known to me and to serve
as my direct testimony in the above entitled case in the form of question and
answer as follows;

PRELIMINARY STATEMENT

That it was Police Corporal MORCHIE M MANGAKOY, Police Investigator of

Datu Paglas Police Station, who conducted examination to the undersigned
affiant at the Investigation room of Datu Paglas Police Station on February 1,
2021 at 1:30PM in relation to the shooting to death of my husband MARTI SAIK
PASANDALAN, on January 31, 2021 at about 9:00PM in Brgy Bulod, Datu Paglas,
Maguindanao;

That I, was appraised of my Constitutional rights and I, fully understand

and conscious that in answering questions asked on me, I did so under oath and
that I, may face criminal and or administrative liability for false testimony or
perjury;

1. Question : Why are you here at this office being investigated?

Answer : I, want to give statement on what I have seen prior the
shooting to death of my companion REYNALDO BALUYOT ESCOTO.

2. Question : What is your relationship with REYNALDO BALUYOT

ESCOTO?
Answer : None sir.

3. Question : Kindly narrate in details what have you seen prior the
shooting of REYNALDO ESCOTO on December 11, 2020?
Answer : On December 11, 2020 at about 10:40AM, I saw DENCIO
MAITUM on standby with his open pipe Black Bajaj motorcycle at the road
intersection going to Sitio Lanao Na Bae and the Provincial Road going to
Columbio, Sultan Kudarat, with a visible pistol tuck-in on his waist, left and
 right. Seemingly, he is waiting someone in the said place. I, signal him
with my horn, and passed by at him with my habal habal motorcycle to
fetch my passenger at Mayo, Sultan Kudarat. And when, I return back, I
saw many people in the place, and someone told me that the passengers
of Green Tricycle from Isulan, Sultan Kudarat were allegedly shot by
DENCIO MAITUM.

4. Question : Do you know personally DENCIO MAITUM?

Answer : Yes sir.

5. Question : How did you know DENCIO MAITUM?

Answer : I, know him personally, since, before he is also a habal habal
driver, and sometimes on the midyear of 2017, he left after he was
suspected in the two (2) shooting incident of a Christian vendor happened
in Brgy Katil, Datu Paglas, Maguindanao.

6. Question : Do you have any personal or family grudge with DENCIO

MAITUM?
Answer : None sir.

7. Question : In your own opinion, what is the motive of DENCIO MAITUM

in shooting them?
ANSWER : I, do not know sir, but gossip circulating that the victims
were allegedly a government asset.

8. Question : For the meantime, I don’t have further questions to ask, do

you have any statement to add or remove it from your statement?
Answer : I don’t have for this moment sir.

9. Question : Are you willing to sign this Affidavit consisting of three (3)
pages to certify that all your statements are made true?
Answer : Yes sir

witness- affiant AHMAD GARCIA BAGUINDO at Datu Paglas Police Station, Brgy
Poblacion, Datu Paglas, Maguindanao;

That I have faithfully recorded and translated into English language the
questions asked on him and the corresponding answers that he gave in response
to the question asked;

Neither or nor any other person coached this witness-affiant regarding the
answers given by him;
