Comparing the certificates

Here are key differences between the long-form certificate released Wednesday and the short-form certificate released in 2008:

Name of hospital

The long form lists the name of the hospital.


The long form is called Certificate of Live Birth. The short form is called Certification of Live Birth.

Other information

The long form includes the mother’s address, the ages of both parents and the parents’ birthplaces.


The long form has signatures at the bottom from the attending physician, the local registrar and the mother.
