APPLE INFRINGEMENT QUESTIONNAIRE

If you received a cease-and-desist letter from a law firm such as Kilpatrick

Townsend, or other intellectual property law firm alleging a violation of Apple’s
intellectual property rights (ex. trademark violations for importing infringing
goods). Call us to discuss your case or send us this form completed. In some cases,
mere cooperation will help get your case closed, whereas, in other cases you may
be facing a potential federal copyright litigation case or ITC 337 proceeding or other
legal action. If you are sued, and there is no response filed, a default judgment can
be filed against you which can be as high as the law allows. Don’t go it alone.

Please answer the following legibly and with as much detail as you can provide, and
we can provide a quote to represent you or your company:

1. Your company full legal name and address (or person if not a company)
