Internship Weekly Report Format

NOTE: Each week you will do 2 (two) reports, One for me and One for the agency. Send the one you do for me via email and have the agency sign the other. Keep that one and return to me at the end of the semester along with the rest of your material. Weekly reports should be submitted on a specific day of the week. That day is on, or before, each Sunday at 11:59 PM EST. This weekly report should be composed and written in Microsoft word and sent to the University supervisor by way of an attachment to e-mail. The format of your document title should look like this: WR1JohnS.doc. The title of your report in the subject line should be as follows: WR 1. This is what I want to see in your personal electronic folder that I keep for each student. The next weekly report should be titled as WR 2, and so on and so on until you reach weekly report number 15 (WR 15) for the fall and spring semesters and weekly report number 12 (WR 12) for the summer semester. Below is an example of your weekly report. Please note the following parts of the report: Personal information-WR1 along with your first name and last initial. Example: ( WR1LarryB.doc). If you do this, it will save the document just the way I want it to be saved. I also want in this part your name, semester, course and the calendar date for the week. In the second part of this form I want the number of hours, date and the day of the week in bold. Then come down one space and indent and then start describing what you did that day. Remember you will have a space in the last part of the form to give your feedback and reactions to the week. Please note that you also must total the number of hours for the week and semester in this section of the form. You should have your supervisor sign this part. IMPORTANT-Keep these and turn in at the end of the semester. In the last section of this form, I want you to give your feedback and reactions of your weeks experience with the facility or site where you are working. This is a communication between you and me, so that potential problems with your site or site supervisor can be discussed and resolved. SEE THE FOLLOWING EXAMPLE:
