Health Perception and Health Management Pattern Before Hospitalization During Hospitalization
Now that she was diagnosed to have heart problem patient X is now more concern about her health status. now she see the importance of having. She thinks that her illness is hereditary

Interpretation
There is a change in health perception and health management pattern due to her health problem, the patient now realized that her health status is not in good term. She see that importance of seeking medical consultation when health problem occur

Patient X view herself as a healthy person. She thinks she is physically and mentally healthy. To make her physically fit she join the aerobics club in her community. Patient X is not used to seek medical advice when she had health problem. She refuses to check her blood pressure before.

Nutrition and Metabolic Pattern

Before Hospitalization
Patient has a good appetite, she eat at least 3-5 times a day. She usually eat large amount of rice every meal, she also love to eat preserve viand like tocino, skinless & longanissa. She usually had morning and afternoon snack ate her office, their usual snack were pancit, Banana Cue or Turon. She also love to drink soft drinks, according to her she could drink 1L of soft drinks in every meal.

During Hospitalization
When the patient was admitted to the hospital her appetite decreases, she became conscious on her diet. She restricted herself to fatty and salty foods. She also restricted herself to drink soft drinks.

Interpretation
A change in Nutrition and Metabolic Pattern due to her case and from the advise of her doctor. She became more concern on her diet, she knows that a change on her diet is a must.

During Hospitalization Patient X stay in the hospital changes her activity. She must stay relax and stress free. Every afternoon.y Elimination Pattern Before Hospitalization Patient X defecates at least once a day and urinates 2-4 times. she walks from her office to her home. She knows that she must avoid strenuous activity that could raise her blood pressure. she also attends her Aerobics Class in their Plaza. Her daily activity will change a lot. y Activity Exercise Pattern Before Hospitalization Patient X is an outgoing and active person. She is an active member of a group in their barabgay. Aside from her role of being a mother and doing the household chores she also has office works that occupied her time. do the household chores go to the office. she doesn t have problem in urinating. also she in not comfortable to use the comfort room of the hospital. Every Wednesday morning. she experienced problem in defecating. She perspire evenly except when she do some activities During Hospitalization When the patient was admitted to the hospital.exercise pattern. She experienced a problem on defecating due to environmental factors. Interpretation There is a change on Patient X activity-exercise pattern due to her hypertension and disease. and have her daily activity. She could not do hard and strenuous activities that could raise her blood pressure. She doesn t have problem on defecating & urinating. Changes in her urination occur because of the medication that was given to her. She can t do her mother role to her children. . though an increase in her usual urination happen because of her medication Interpretation There is a change in the Patient X elimination pattern. She used to join group activities in their place.

y Sleep-Rest Pattern Before Hospitalization Patient X enough sleep. Interpretation There is a change in sleep-rest pattern due to her stay in the hospital. She feels slight discomfort in her stay in the hospital. During Hospitalization Patient X doesn t have hearing problem and does not wear eyeglasses. Interpretation There is a slight change on her cognitive pattern due to her hospitalization. y Cognitive-Perceptual Pattern Before Hospitalization Patient X doesn t have hearing difficulty. since her medication is given almost every hour. During Hospitalization Patient X stays in the hospital changes her sleeps and rest pattern. She hasn t had enough sleep. but she can stay long lying on bed. She never forgets to take a rest every day and every time she do some activities that made her feel tired. She is not comfortable in sleeping in other place expect their home. she cannot sleep lying on bed for a long time. she goes to bed at 9pm and wake up in the morning at 5. She does not used to stay up late in the evening. She can also learn and understand things easily. The patient can feel more comfortable on lying on bed for a long time. she feel no difficulty on breathing. She is able to learn and understand things easily. Days prior to confinement the patient feels discomfort and problem on sleeping. She experienced difficulty of breathing sometimes. . She also experienced headache. She doesn t wear eye glasses.

Her niece and nephew where close to her. She can t perform her role as a mother. Her friends were so concern on her status. she has normal menstrual cycle. her children was not allowed to go to the hospital. She is a moth of 3 children. Her friend gives her support.. She thinks that she may not be able to perform her role as a mother to her children and friend to her friends because of her illness. She doesn t use contraceptive. She is a working mom but she never forgets her role as a mother. trying to cheer her up. y Sexuality Reproductive Pattern Before Hospitalization Patient X was a mother of 3 children. She never had problem during pregnancy except for her high blood pressure During Hospitalization Patient X doesn t use contraceptive. Interpretation There is a change in the patient role and relationship pattern due to her hospitalization and diagnosed disease. Interpretation There is no change in her sexuality and reproductive pattern. she is a full time mother and housewife to her family. During Hospitalization Patient misses her children. . She has her menstruation every month.y Roles and Relationship Pattern Before Hospitalization Patient X belongs to an extended family. She makes it sure that that she have time for her family. her friends and officemates where close to her. Patient has a good relationship towards others.

she used to laugh on her problems. She believe that her health problem is just an obstacle and a test to her. she do her best to cope up to her health problem. She never forgets to pray. y Values and Belief Before Hospitalization Patient X has a stronghold and believes to God. if they have problem in their family they discuss it together.y Coping-stress tolerance pattern. To cope up with her problem she also never forget to pray. Interpretation There is a little change to her coping-stress pattern. Before Hospitalization Patient X is a jolly person. During Hospitalization Patient still believe to God. . She never let her problem ruin her day. Interpretation There is no change in her Values and belief Pattern. She always goes to church every Sunday together with her family. During Hospitalization Even though Patient X was diagnosed to have heart problem she still can laugh and smile. She never forgets to pray.
