A letter or certificate in writing from UNIRAZAK, shall be conclusive evidence of the amount owing to UNIRAZAK by the Student as at the date of such letter or certificate. 2. ANY notice including writ of summon issued by UNIRAZAK in relation to the above matter may be given to me by UNIRAZAK in writing by either by personal service or registered post at my last known address given in which such notice and/or writ of summon, are deemed to be served on me immediately after service or within 3 days after posting. 3. We acknowledge that we earn a combine monthly income of not less than RM1,000 and the term of this guarantee and/or indemnity have been explained to me and we hereby agree to be bound by it at our own free will. 4. We do solemnly and sincerely declare that I/We are not an undischarged bankrupt as defined under the Bankruptcy Act, 1967. 5. I/We agree and confirm that the information given in this registration form is true and correct and I/We understand that it is being used to determine my/our credit responsibility. I/We hereby authorize UNIRAZAK and its officer or third party agencies appointed by UNIRAZAK to obtain any information UNIRAZAK may require for these purpose from other sources including consumer report, credit reporting agency, or reference schemes and each such source is hereby authorized to provide you with such information and permit UNIRAZAK to disclose or release data/information relating to my/our company/business/individual to credit bureau or central bureau. Notes: 1. 2. Only those with a monthly income of not less than RM1,000 are eligible to become Guarantor. Any one of the following shall be eligible to be a Witness to this document: Commissioner for Oaths / Head of Village / Tuai Rumah / Ketua Kaum or Mukim Bertauliah / Lawyers / Judges / Government Officers at Management or Professional level (formerly Class A) / Government School Headmasters / Jaksa Pendamai / State Government EXCO Members. Please also ensure that you get your witness to stamp their designation at the witness column of LGI. Students are required to, either have this document stamped at the nearest Stamp Duty Office, or purchase a duty stamp (setem hasil) for RM10 at any post office and submit it together with the LGI for our onward endorsement at the stamp duty office. Please take note that, we will not receive cash for the stamp duty. The completed document together with a certified copy of Students NRIC and Guarantors NRIC, as well as proof of income document (e.g. Pay slip, Borang J, Borang EA or Employers Certification Letter on salary), must be submitted to the Payment Counter during registration into the University. Students who fail to submit these documents during registration shall not be allowed to register into the University.
