Highlights: Reasonable increases and we are moving vision back to VSP
2012 Open Enrollment Timeline:

It is time for open enrollment for the 2012 plan year. Our focus on wellness seems to be paying off with renewals well below industry standards. Health insurance contributions are increasing approximately 5% and dental insurance contributions are increasing about 1%. The new monthly contributions are at the bottom of this page. Based on your feedback, we will be switching our vision coverage from United Healthcare back to VSP, which was the company we used when our health insurance was with Guardian. VSP’s vision includes a broader network of private practitioners that seem to be more suited to our staff’s vision care needs. Other than the shift in vision coverage, there will be no significant changes to our medical, prescription and dental plan designs. This is the one time of the year when you can add and remove family members without a qualifying event. You can also change between the Choice and Choice Plus plan. Take a careful look at the checklist on page two. Please refer to the 2012 Open Enrollment Timeline on the left for a list of open enrollment meetings and due dates. You will receive an appointment for each meeting. You are welcome to bring your spouse to the meetings if you’d like. Staff nearing retirement are welcome to attend the retiree meeting to learn about how our health plan integrates with Medicare and other issues specific to retirees. If you want to make a change to your health, dental, or life insurance, please complete an insurance change form. You must re-enroll in flexible spending account each year and submit new forms if you want to participate. All the forms can be found on mybenergy (login info below) and are available in HR. Everything must be submitted to HR no later than November 30. Please stop by and talk to any one of us in HR if you have any questions regarding your benefits.

Flexible Spending Accounts

Complete a Flexible Spending Account Enrollment Form. Note: You must re-enroll each year to continue participation.

Life Insurance — ASHA provides a group benefit of two times salary to all eligible staff.

*You can purchase additional coverage with a guaranteed issue amount of up to $40,000. Any amount exceeding $40,000 and any increase in spouse life will be subject to Evidence of Insurability. If you answered YES to any of the above questions, you need to complete the Enrollment Form in the 2012 Prudential Enrollment Guide and/or the Notice of Change in Dependents’ Status. Please complete the Beneficiary Designation Form if you need to update your beneficiary information.

Long Term Disability Insurance — ASHA provides a group benefit of 60% of salary to all eligible staff. The benefit is limited after age 67.

Long Term Care Insurance — ASHA provides a small benefit to eligible staff with more than 10 years of service.
