I, the undersigned, do hereby, for myself and all who may hereafter claim through or for me, waive and release Arizona Asian American Association ,the above named agency from all claims, rights, and causes of action accruing in my favor as a result of personal injury, loss of life, or loss of property against The Arizona Asian American Association, the above named agency, and their representatives while participating in the activities related to 16th Arizona Asian Festival, the above stated project. I understand normal risk associated with the activity described above and I agree to participate in the activity. I further agree with them that no suit or action at law shall be instituted for the above reasons by others or me in my behalf. VOLUNTEER APPLICATION FORM

General Information:
The primary goal with Arizona Asian Festival is to encourage the development and interest of children’s creative skills while bringing the community together in Unity, an effort to promote Diversity through cultural awareness and support the work of Arizona Asian American Association.

Volunteer Requirements
The whole family can volunteer. While some areas have minimum age requirements, youth are welcome with family and friends. • • Volunteers must be able to meet the physical requirements of any job duty they sign up for. Volunteers must sign the volunteer release form. Any volunteers under the age of 18 must volunteer with a parent/guardian/leader/teacher/etc. Any volunteers under the age of 18 must have a signed parental waiver in advance. Volunteers must be able to stay for the entire volunteer shift that they sign up for.

Volunteer Descriptions
1. Avenue of cultures • Help ethnic director as needed 2. Charity Fundraising • Help the charity committee with their charity fundraising. Help co-chair as needed 3. VOLUNTEER APPLICATION FORM

• Help attendees with games, prepare area, keep equipment ready, help games co-chair as needed • Help sell raffle tickets to raise money 4. Runner/ Vendor and General Assistant • Keep volunteers supplied with drinking water, supply booths with supplies and run errands, volunteers must have a lot of energy • Provide assistance to any help they need • General assistance as needed by chairperson 5. Set up/ Decoration • Help with setting up booths, hanging signs, taping down table covers, clean up, must be able to lift 50 pounds and stand for extended periods of time. • Put up decorations, banners, etc. 6. Clean up/ Tear Down • Take trash to the dumpster, check the bathrooms, throw away trash • Remove signs, put away table and chairs, remove banners, booth, empty trash, sweep area 7. Booth/ Tent assistant (Info and water booth) • Provide patrons with information, hand out program books, assist attendees, redemption booth for passport completion, job is sometimes busy but always in the shade, work closely with the committee chairperson • Sell water to people, ice and dispense drinks, inventory soda and water, count and turn in money at end of the day, work with booth captain and chairperson 8. Peformances assistant (Street and Stage) • Keep performers on schedule, decorate stage and keep props in order, check in performers, must be able to stand for extended periods of time, provide water to performers and volunteers, assist in clean up of the stage and dressing room areas • Help with parade, coordinate with performance stage to have performers entertain the streets, make sure performers are spread out evenly 9. Survey • Prepare survey materials, conduct survey during festival, collate and summarize survey statistics 10. Photography • Take pictures of people, performers, participants 11. Traffic Patrol • Provide traffic direction and safe passage for both pedestrians and motorists traveling to and from the festival.
