Licensed Clinical Social Worker

The Licensed Clinical Social Worker position is for the new Transition of Care (TOC) Program, Older Adult Services. This will be a full time (with benefits), non-exempt position, reporting to the Director of Older Adult Services. GENERAL: The Licensed Clinical Social Worker provides intensive outpatient case management for Mills Peninsula Hospital patients that require post-discharge transitional care services. Conducts assessment and develops treatment and care plans or the target population. QUALFICATIONS: Required 1. Licensed Clinical Social Worker. 2. Licensed for at least two years. 3. Two years clinical supervisory experience. 4. Experience working with older adults. 5. Knowledge of Social Services availability in San Mateo County. 6. Ability to take initiative with minimal direction, work offsite, and work in close collaboration with other team members and vendors. 7. Strong organizational and verbal and written communication skills. 8. Computer efficiency in MSOffice, internet usage and database; experience. 9. Operate own vehicle on regular basis to visit clients and attend meetings. 10. Ability to work occasional nights or weekend days. Preferred Bilingual Spanish/English or English/Mandarin or English/Cantonese. Two years previous care management experience with older adults in a medical setting. RESPONSIBILITIES include, but are not limited to: 1. Coordinate care of large caseload of high-risk discharged patients from Mill Peninsula Hospital. 2. In collaboration with TOC RN, determine level of care; conduct assessments; create care plans; determines lead clinician based on clients needs, clinical or social. 3. Provide assessments and interventions in the community (home visits, counseling, coordination for vendor services focusing on psycho-social needs). 4. Make referrals to vendor services and appropriate community resources. 5. Conduct referral follow-ups to assure client needs are met. 6. Maintain database (enter client information, plan of are, services and outcomes). 7. In coordination with TOC RN, manage, coordinate and communicate clients services within TOC team.

8. Coordinate discharge from Transitional Program and handoffs to TOC Wellness Coaches and other community providers. 9. Provide clinical lead responsibilities for TOC MSW and TOC Wellness Coaches. 10. Prepare timely reports. 11. Attend and participate in appropriate TOC meetings.
