Our Admission Committees are interested in finding out more about you and your involvement in activities in addition to your school work. The information you provide will be kept confidential and will be used by our Admission Committees in making individual admission decisions. For September 2007 admissions please

complete and submit your Admission Information Form no later than Thursday, March 1, 2007.

Instructions
This form is intended for applicants to the Faculties of Applied Health Sciences, Arts (except for the Computing and Financial Management and the Independent Studies programs - see note below), Environmental Studies and Science and to the School of Architecture (Faculty of Engineering). You need to complete only one form regardless of how many programs you have applied to within these Faculties. Note: If you have applied to Computing and Financial Management and another Faculty of Arts program you will need to submit this form and the Faculty of Mathematics form. If you have applied to Independent Studies you do not need to complete an Admission Information Form. If you require the form for the Faculties of Mathematics (including Computing and Financial Management), Engineering, the Software Engineering program or for the School of Pharmacy, please go to: www.findoutmore.uwaterloo.ca/thenextstep/applicants/aif.htm or call 519-888-4567 ext. 33106. Use one of two ways to respond to the questions: You may print responses on the Admission Information Form in the space provided, or if you prefer, word process/type your responses on 8.5" x 11" paper. Include your full name and UW ID number at the top of each page, double space your answers, and then staple the completed pages to your Admission Information Form. For Questions 2, 3, 4, and 6, please limit your answers to your five most recent and/or significant examples.
