Health Perception Pattern

2. Nutritional/ Metabolic Pattern

BEFORE HOSPITALIZATIO N The patient believes that a healthy lifestyle must be maintained. According to her relatives, she maintains her health with a balance of exercise and eating a balance of many foods. The relatives have also told us that she is a social drinker, and she believes that a couple of drinks cant harm her. Anytime she feels that she is going to be sick, she consults with her doctor. Normal lang naman pagkain nyan sa pang arawaraw, pero mas gusto nya gulay. Ayaw nya sa isda. As verbalized by the Relatives. She eats 3-4 times a day And usually just consumes 1 cup of rice and one serving of meat and vegetables. She rarely eats desserts. She drinks 7-9 glasses of water per day.

DURING/AFTER HOSPITALIZATIO N The patient is cooperative and believes that she will be healed soon. Although she is unhealthy during her stay, she does not consider it as a serious health condition. According to her relatives, this is related to how she copes with stress, she just smiles and says Gagaling din naman ako.

ANALYSIS The patients positive attitude towards health tells us that she has a good perception regarding health. She is also cooperative with regards to the health care services provided to her.

On Osturized Feeding due to dysphagia: 1600kcal divided into 6 equal feedings of 1:0.5 dilution. Intake/Output: March 4 2011 am and pm: 1300cc March 5 2011 am: 1920cc

The change in her nutritional/metaboli c pattern is due to the dysphagia the patient is experiencing. The diet estimated is given by the physician. We believe that this is in-order to maintain her nutritional needs at a normal level. Her large intake of fluids is related to her diet.

3. Elimination Pattern

Bowel Movement: 1 per day, medium in amount, semiformed. Voiding Pattern: Hindi ko alam kung ilan beses sya umihi eh, naka diaper kasi sya. As verbalized by the caregiver.

[Bowel Movement] 1BM, medium in amount, brown, semi-formed. [Voiding Pattern] Output march 4 2011 am and pm: 2280cc Output as of march 5 am: 1100cc On average: 140.1cc per hour this indicates polyuria.

There has been no change in her bowel elimination pattern. The frequency of her BM and other characteristics of her stool was noted to be similar and unchanged. Although, the voiding elimination changes cannot be estimated. At present, the client is manifesting polyuria, which may be resulting from her increased fluid intake from sources such as IVF and OF. The ActivityExercise pattern of the patient is hindered by her being unambulatory. This may be due to muscle weakness in her lower limbs which is one of the initial symptoms of the disease.

4. Activity-Exercise Pattern

The patients daily activities include walking and aerobic exercises.

5. Roles/Relationship Pattern

She is a proud mother of 5 girls and 3 boys. She also keeps the peace when there are miscommunication s around the household.

The patients activity and exercise is reduced due to her hospitalization, she has difficulty moving her legs which led her to be un-ambulatory. However, the nurses assist her in her passive range of motion exercise and frequently turn her to positions. Her family and friends visit her during visiting hours and this makes her happy. They cheer her up and always hope that she gets better soon. It is likely that it would be difficult for her

SelfPerception/Selfconcept Pattern The patient is 67 years old and she perceives herself as a good mother and grandmother. She has no body image disturbances as told to us by her relatives.

Sana gumaling na ako, mahal na ata ng babayaran. As verbalized by the patient. This could indicate the patient is experiencing mild anxiety due to the expenses of her hospitalization. This affects how she views herself as a person.

The patients mild anxiety could be attributed to her feeling like she is being a slight burden to her family. Her anxiety is related to her feeling of being a burden to her family and this affects how she perceives herself as a person. Mild anxiety is a form of an anxiety that is not severe enough to be considered as a disorder.

7. Coping/StressTolerance Pattern

The patients main stressor is family problems. She copes by watching Umaga kay ganda(An ABS-CBN morning show) and watching the teleseryes during the evening.

Her stressor during hospitalization is the hospitalization itself. There is no presence of a television her room in the ICU. According to her relatives, she likes

The patients Coping-Stress pattern before hospitalization seems healthy. Although, due to the absence of a television in her room, she just deals with stress with a simple foot massage.

8. Value-Belief Pattern

Pumupunta sya sa simbahan(Catholic) mga 3 beses sa isang lingo, at nagdadasal yan bago kumain at bago matulog. Talagang may tiwala siya sa Diyos natin. As verbalized by the Relatives. Magalang sya, magaling sa tao, mabait at bihira magsinungaling. As verbalized by the relatives. Her values include the traditional Filipino values of Po and Opo, pakikipag kapwa tao and Katapatan.

She is unable to go to church due to her hospitalization. Malakas parin naman paniniwala nyan sa dyos. As verbalized by the relatives. She still believes in the importance of being respectful, being kind, and honesty. Wala namang nagbago sa values nya. Hindi naman naapektuhan ng hospitalization nya ngayon. As verbalized by the relatives.

The patient retains her Filipino values and her beliefs remain unchanged despite of her present condition.

9. Sleep/Rest pattern

She sleeps with an average of 6-9 hours a day. She wakes up at 5am and sleeps at 9pm.

She sleeps on an average of 4 hours and is randomly awakened.

The patient shows slight sleep/rest pattern disturbance; this maybe attributed to her hospital stay, but is not enough to indicate that she has an abnormal sleep-

rest pattern.

10. Cognitive/ Perceptual Pattern

She is understandable and reads the newspaper every day. She has no known mental disabilities. She has normal vision, taste and hearing for her age.

No Change

The patient shows a healthy Cognitive/Perceptu al pattern. She can still express herself and perceives things logically.

11. Sexuality Reproductive pattern

The Patient is 77 years old and has undergone menopause. She is widowed therefore her sexuality pattern is no longer active.
