This is the beginning of your professional network.  Your work activities and your reactions to them; how your work activities affect your thoughts about your major and/or possible career; practicum rewards and frustrations; other thoughts/feelings (5-7 sentences per week).  In either instance, make a point of taking photographs, collecting training materials, and/or gathering other materials related to your practicum. Eventually, you will construct a "Practicum Portfolio" and create a professional poster to explain your practicum; the materials you collect will help you prepare for success with both. Progress Report #1  After one-third of your practicum is complete, please complete progress report #1. This is an opportunity to reflect on what's been happening so far.  Explain...  What is going well in your practicum?  What is not going as well as you'd hoped?  Include a strategy for attempting to deal with the needs for improvement, e.g., arrange a meeting with your site supervisor, list topics you would like to discuss with him/her or me, change your work schedule, etc. Progress Report #2 (includes a self-evaluation)  After two-thirds of your practicum is complete, please complete both a progress report selfevaluation.  This exercise will help you learn to think about yourself as an employee. Are you meeting the goals your supervisor has set for you? Are you meeting the goals you have set for yourself? This is an opportunity to make adjustments before your supervisor completes your formal evaluation.  Use the Supervisor Evaluation Form to rate yourself. Explain your self-ratings fully. Be sure to give yourself credit for what you do well - as well as being honest about what you need to do better. Supervisor’s Evaluation Form  You complete the same exact form for Progress Report #2 that your supervisor uses to evaluate you. The Learning Contract acknowledges your supervisor’s agreement to fill out this form (available under ‘Practicum Materials’ on ELMS) upon the completion of your practicum. You should provide your supervisor with an electronic copy of the form as you near the end of your practicum and ask her/him to email it to etesupervisoreval@gmail.com. This form is NOT to be submitted by the student.
