Revisar la relacin ventilacin - perfusin, en funcin de:
Posiciones corporales Velocidad de flujo inspiratorio Patrones musculares respiratorios
Incorporar el concepto de la mejora de la relacin V/Q como objetivo primordial de la Kinesiterapia Respiratoria.

AMIS. CINCO POSTURAS Y DOS EJES Expansin, ventilacin, perfusin y V/Q Sujetos: 20 adultos sanos. Inhalacin de gases inertes radioactivos (K81 y K85), detectados durante volumen corriente con una cmara gamma y lavado en la espiracin. Posturas: sentado erecto, supino, prono, decbito lateral derecho e izquierdo. Ejes: vertical y horizontal
Parmetros:
Expansin Pulmonar (volumen), es un ndice entre la capacidad funcional residual regional y la capacidad pulmonar total regional (CFRr/CPTr). Ventilacin Pulmonar: expresada como ventilacin regional por unidad de volumen alveolar (V/VA). Perfusin Pulmonar: expresada como perfusin regional por unidad de volumen alveolar (Q/VA) Relacin Ventilacin Perfusin (V/Q)
El contenido alveolar de gas preinspiratorio es uniforme en todo el pulmn en esta postura, probablemente la gradiente de presin pleural debe estar reducida en posicin prono.

COMPORTAMIENTO DE VOLUMEN, VENTILACIN, PERFUSIN Y RELACIN V/Q EJE VERTICAL DECUBITOS LATERALES Las gradientes de expansin superior/inferior fueron casi las mismas, pero el pulmn inferior tiene un menor volumen, posiblemente por el peso del mediastino y la transmisin de la presin abdominal sobre los lbulos caudales inferiores. En posicin decbito lateral el pulmn inferior ventil el doble que el pulmn superior. En posicin decbito lateral la perfusin para el pulmn dependiente (apoyado) fue mayor que para el pulmn no dependiente.

FLUJOS INSPIRATORIOS Y DISTRIBUCIN DE LA VENTILACIN ALVEOLAR Cuando se inspira a partir de Capacidad Funcional Residual (CFR) a velocidad de flujo baja, la ventilacin se distribuye principalmente en las bases pulmonares. La inspiracin rpida alter ligeramente esta distribucin, con una cada en las regiones basales y un alza en las regiones apicales.
