OUR LADY OF THE LAKES SPRING ANTIQUE & CRAFT SHOW SHOW DATE: SATURDAY, OCTOBER 20, 2012

SET-UP TIME: 10 AM to Noon SHOW TIME: Noon to 5 PM SHOW DATE: SUNDAY, OCTOBER 21, 2012 SET-UP TIME: 7 AM to 9AM SHOW TIME: 9 AM to 2 PM EXHIBITORS LEASE AGREEMENT The undersigned hereby applies for a space at the Our Lady of the Lakes Church Spring Antique & Craft Show, scheduled for Saturday, October 20, 2012 and or Sunday, October 21, 2012 and agrees to the rental fee per space to the manager(s). All fees will be in cash, money order or check upon execution of this Agreement. If accepted, the undersigned hereby agrees to abide by the following conditions: 1) Our Lady of the Lakes and their representatives shall not be responsible for any injury, loss or legal action sustained by the exhibitor, their employees, goods, or property, or the public, from any cause whatsoever while the premises are being occupied under this Agreement. 2) The exhibitor will be assigned table(s) only upon receipt of your payment. Mail reservation and payment must be received no later than October 12, 2012. 3) After unloading, all exhibitors vehicles and trailers can be moved to the side of the Hall or Rectory to allow ample parking for visitors and shoppers. 4) Please give 48 hours notice if you are not going to occupy your table/space in the show. 5) Make checks/money orders payable to: Our Lady of the Lakes Church and mailed to the address listed below. Please include a selfaddressed stamped envelope for confirmation. 6) Confirmation of acceptance will be sent to exhibitors upon receipt.
