In December of 2020, the National Commission on Military Aviation Safety published a report citing root causes to the thousands of military aviation mishaps.  The five-year study of this report recounts some of the leading causes which include inadequately trained aircrew and maintainers, aggressive operations tempo, overworked maintainers, and unpredictable budgets.
