Cape Fear Valley Hospice provides services to family members love ones whom
are living with terminal illness, and quality of life is what matters most. This
company beliefs are that all chapters of life, even the final stage of life can be
well lived. The staff with Cape Fear Valley Hospice are compassionate
professionals whom are here to help your love one to live comfortably and with
dignity when there is no possible cure. Our top priority is to provide optimal
physical, social, emotional and spiritual needs of each patient and family .

Strengths
The hospice team works as a well oil

machine to provide excellent service to

Weaknesses
A major weakness I have observed is that even

though every hospice team member advocates for


the patient and family members needs. Not every
team member is aware of community resources
that are available in the patients surrounding
community.

Every team member is vital towards providing

excellent end of life care. The team consists of


multiple RN, Chaplains, office administrators but
only BSW working in the field, whom is the main
person the team members go to for information on
resources.

IDENTIFYING THE PROBLEM


The BSW job is to provide information that assists
the patient and family members with their medical
needs and other related stressors that affects the
patients peace of mind.
Problem

Some patients are on fix income, therefore the needs


for community resources to fulfill gaps in patients
needs. Example: paying rent, goods, water bills and
etc. The knowledge of these resources are not on
hand for some of the patients area.

I have observed that the Hospice Social worker only has


information on Cumberland County resources and has to
obtain other county resources through research which,
is time consuming.

PROJECT PROPOSAL TO CAPE


FEAR VALLEY HOSPICE
I would like to start a book of resources
that the BSW and other interdisciplinary
hospice team members could utilize when
patients need additional resources. These
resources would assist patients and family
members with service gaps, such as
paying rent or money for food. Every team
member would be provided with a
resource book because he or she may be
more informed on resources provided in
their patients community.

This resource provides: victim advocacy, 24-hour crisis line, court

advocacy, domestic violence order and police report, transportation,


confidentiality address program, emergency legal aid referral,
referral/information, hospital accompaniment, support group, children
support group, emergency shelter and housing, emergency finance,
clothing closet, food bank, and victim compensation.
