The various FORMS that need to be maintained in order to fulfill the conditions of
THE FACTORIES ACT, 1948.

EACH FORM CONTAINS CERTAIN DETAILS THAT NEED TO BE FOLLOWED TO THE LETTER.

Measures to effectively dispose of wastes and effluents. ventilation) and also the position of the machinery within the plant. regarding the arrangements made for the disposal of Trade waste and Industrial effluents.  Be submitted in triplicate.Form 1  contains flowchart of the manufacturing process at its various stages.  Be accompanied along with a certificate from the TNPCB.  Presents blueprints of the Factory. drains. fumes and gas.

FORM 2 DESCRIPTION: For the Application for the registration and grant/renewal of License. .

 Satisfied.  Every license/ renewal of license granted shall remain in force up to 31st December of the same year for which the license was granted/renewed. .  Every application received by DCIF be registered in Form 3. Chief Inspector of Factories (DCIF)  Be submitted in triplicate.Form 2  Be accompanied by a treasury receipt or a crossed demand draft drawn from a Nationalized bank in favor of the Dy.  Occupier may remit the license/ initial license or renewal of license (yearly) in favor of doing so for a lump sum period of 5 consecutive years . the DCIF will grant license in Form 4.  The licensing fee is to be paid by October 31st of the same year.

if paid after the expiry of the license. a duplicate license can be obtained by paying an amount of Rs. Fines range from ‡ ‡ ‡ 10% of the Fee amount along with the Fee. if paid in the month of DECEMBER. 20% of the Fee amount along with the Fee. 50/- . if paid in the month of NOVEMBER. NOTE: If the original license is lost or has been destroyed.PENALTY In the event that the fee is not paid on time then a fine shall be imposed along with the fee. 30% of the Fee amount along with the Fee.

FORM 3 : Registering of Factories FORM 3A : Notice for change of Manager FORM 4 : Registration and License to work .

FORM 5 DESCRIPTION : Certificate of Fitness .

issues the Certificate of Fitness in FORM 27.  The counterfoils of all issued certificates be kept with the Certified Surgeon. During visit. .  The records of all examinations done during visit be kept with the Factory Manager.  The foil and counterfoil be filled.  The Certified Surgeon shall visit the Factory.  Contain the left thumbprint of the person in whose name it is being issued.  The results of the tests entered by the Certified Surgeon in the HEALTH REGISTER in FORM 17.FORM 5  Be issued by the Certified Surgeon.

varnishing and painting carried out be entered in FORM 7 .FORM 6 : HUMIDITY REGISTER RECORD OF LIME WASHING AND PAINTING FORM 7 : Record of dates on which whitewashing.

FORM 8 DESCRIPTION : REPORT OF EXAMINATION OF PRESSURE VESSEL OR PLANT .

 Internal Test can be replaced by conducting a Hydrostatic Test Once in 2 years.  Conduct Internal Test Once every 12 months. period of External Test extended .  EXAMINATION REPORT sent to Inspector within 7 days of it being held .  Incase of hard to open Pressure Vessel.  Conduct External Test Once every 6 months.once every 4 years.FORM 8  Every Pressure Vessel in service be thoroughly examined by Notified person.
