Personal Lock guideline

 1. Only AES-VCM personnel allowed to perform personal

lockout.

 2. Simple maintenance job with a single point of isolation.

 3. Shall be written in JESA & Permit to Work. Must be approved by
CBO in duty shift.

 4. For electrical equipment, =< 400 volts. NOT apply for isolation point
required RACK IN/RACK OUT.
