    1. A. For HIGH SCHOOL GRADUATING STUDENTS a photocopy of Form137 duly signed by the Principal/Registrar in blue ink with the school seal affixed thereat. B. For HIGH SCHOOL GRADUATES or PEPT QUALIFIERS - original and photocopy of high school card. C. For TRANSFEREES - endorsement from the WMSU Dean of Admission. 2. Two identical copies of 2 x 2 photo taken recently with NAME TAG. 3. Correctly accomplished WMSUCET application form. 4. Non-refundable test fee of Two Hundred Pesos (P200.00).

    No Requirements, No Application Form No Proxy Application
