SELF PERCEPTION/ SELF CONCEPT The client sees himself a good father and provider to his family.

At 77, he is a semi retired farmer from Ilocos. Hiring workers to till his farm, he still continues to oversee his farm. Out of five children, four were able to graduate and are mostly medical professionals. Four months ago, the client exhibited coughing and as such, he attempted to self medicate. Eventually a week prior to hospital admission, the client experienced difficulty in breathing together with coughing with whitish phlegm. On August 30th, he was admitted at the SBS Hospital in the Marikina area, where he was temporarily staying with relatives. Later on, he was referred to the DLS Hospital for treatment of his hernia and Community Acquired Pneumonia, which was diagnosed soon after. On September 12th, the client was operated on for his hernia. We can surmise that the client possesses a sense of confidence and fulfillment as a person from his ability to overcome personal hardships despite his chronic ailments including type 2 diabetes and hypertension. From the interview with the nephew, it was mentioned that the client is optimistic that he will be able to overcome his ailment and fully recover from his operation in the same manner that he was able to overcome an appendectomy about four years ago. There was no indication of sadness and depression over his situation. He mentioned that the client continues to find inspiration from his family. Clearly the client has a positive outlook in life.

ENVIRONMENTAL HISTORY The client is a semi retired farmer living in Ilocos. The fact that he was able to provide for the education of his children who are mostly in the medical profession (i.e. nurse, dentist, midwife) indicates that the clients living standard is at least comfortable. He and his family live near their farm in a small house made of light materials (i.e. wood, hollow blocks, cement), with electricity and running water, adequate lighting and adequate ventilation. Being in a farm in Ilocos, it can be assumed that the client lives in an environment with fresh air, adequate protection from the weather, in an area that is not densely populated, but possibly with questionable hygiene due to the prevalence of animals. When the client decided to go to Manila to pursue medical attention for his ailment, he lived with relatives in Lilac St., Concepcion Dos, Marikina City. This area is known for having a middle class standard of living.
