1. BHMG has an actual and physical office, Beverly Hills Advanced Surgery Institute,
Inc. known as BHASI in Beverly Hills, California and we have been in that community
for close to 12 years doing business as a Multi-Specialty Surgery Center. We are
licensed by the California Department of Health Services and well as Medicare
Certified under the U.S. Federal Government. BHASI also has a branch office in
Pasadena, California known as Pasadena Advanced Surgery Institute, Inc.

2. We are the largest U.S. based, managed and operated Multi-Specialty Ambulatory
Surgery Center in Asia.

3. We are not an affiliate or franchise but an actual branch office. All BHASI corporate
proprietary policies, procedures, processes and systems have been incorporated into
BHMG.

4. We offer and array of outpatient surgical services to include: Cosmetic, Plastic and
Reconstructive Surgery, Cosmetic Laser and Dermatology Procedures, General and
Vascular Surgery, Gastroenterology, Gynecology Surgery, Otolaryngology (ENT)
Surgery, Podiatry, Orthopedic and Sports Medicine, Pain Management, Urology and
Men’s Health.

5. BHASI is a professionally managed U.S. corporation and BHMG is managed with the
same professional expertise.

6. The President , CEO and owner of BHMG is also the President, CEO and owner of
BHASI.

7. Maria M. Rebujio is BHASI’s and BHMG’s President and CEO and has over 25 years
experience managing Multi-Specialty Surgery Centers.

8. Under the Governing Body and BHMG’s Medical Director are a staff of 80 of the country’s top
Board Certified Surgeons and Physicians in ten different specialties and 16 Board Certified
Dermatologists.

9. Each and every surgeon and physician at BHMG is credentialed by BHMG’s

governing board to guarantee that all our surgeons and physicians are board
certified and distinguished specialists in their particular fields.

10. All of BHMG’s nursing staff have additional certification in Advanced Cardiac Life
Support i.e. ACLS certified.

11. BHMG does not have Cosmetic Surgeons, but Board Certified Plastic and
Reconstructive Surgeons who can perform aesthetic or cosmetic surgery.

12. Board Certification requirements entails that the physician has passed both the
rigorous verbal and oral examinations in their particular field, have been in practice
for the required amount of time and lastly have been invited to be a member of their
particular board association i.e. Philippine Board of Plastic & Reconstructive Surgery
or Philippine Board of Dermatology.

13. We do not have Cosmetologists at BHMG. Our Dermatology patients are treated by
only Board Certified Dermatologists themselves.

14. BHMG is licensed by the Philippine Department of Health Services and meets all
U.S. and Joint Commission International (JCI) Guidelines for Ambulatory Surgery
Centers. BHMG’s goal is to be the first JCI certified ambulatory surgery center in
Asia.

15. BHMG has been approved by the Philippine Government for Medical Tourism.

16. We have four U.S. Department of Health standard operating rooms with an 8-bed
recovery suite, the largest of its kind in Manila for a licensed Ambulatory Surgery
