PERSONAL DECLARATION

I hereby declare that all information given above is true, complete and correct to the best of my knowledge. I
understand that any misrepresentation or material omission made on the Volunteer form or other documents
requested by AirAsia (L) Foundation (the Foundation) renders any working arrangement liable to immediate
termination.

I further acknowledge that:
there will be no wages or allowances paid by AirAsia (L) Foundation for volunteering. I will, however, be
reimbursed for any expenses agreed in advance with AirAsia (L) Foundation.
Any and all voluntary services term may be terminated by myself or by AirAsia (L) Foundation by written
notice with immediate effect.
By volunteering, I agree to give full rights to AirAsia (L) Foundation to use any artwork, materials or
intellectual property produced during the whole volunteer duration (the Duration). Intellectual
property shall mean all intellectual property and proprietary rights including without limitation all rights
of inventorship and authorship, inventions, patents, patent applications, and know how for any product,
process, method, machine, manufacture, design, composition of matter, or any new or useful
improvement thereof, as well as copyrights, trademark, trade dress and service mark rights and all rights
in trade secrets, computer software, data and databases, and mask works.
I agree that I will treat and safeguard as private and strictly confidential in nature all information duly
acquired from, and/or disclosed by, the Foundation and/or in connection to my volunteer duration. I
further agree that such confidential information shall not be disclosed to any third party or be used for
whatever purpose without the prior written consent of the Foundation. Upon the termination and/or
expiry of the volunteer duration, as the case may be, I shall promptly return to the Foundation (without
the need for request) all information, documents and data in connection with the appointment herein
which have been provided to, or obtained by me and I shall not retain copies or record of the same in any
form. I shall thereafter not use, utilise or incorporate such information, document or data for any reasons
whatsoever and this shall apply during and after the period of voluntary service term.
AirAsia (L) Foundation is under no obligation to provide me with any payment or any guarantee of paid
employment upon completion of my assignment(s).
I undertake the whole Duration of the volunteer assignment to meet in good faith the goals and
objectives of AirAsia (L) Foundation. I agree to release and discharge AirAsia (L) Foundation for
responsibility over any and all personal injuries, damages, losses, causes of action, liabilities or
unaccounted for expenses that may arise during the Duration of voluntary service. Furthermore, I shall
indemnify and save harmless the Foundation against and from:-
a. any act, omission, breach, non-observance or non-performance by you of any of this
provisions of this letter which results in the Foundation incurring any liability whatsoever;
b. any claim, damage, loss or expenses due to or resulting from any negligence, wrongful act or
breach of duty on your part; and
any and all claims and proceedings for or on account of infringement of any protected
intellectual property rights related to or in any way connected with any reports, electronic
and digital reports and data bases, computer storage discs, computer printouts, estimates
and other relevant documents or information compiled or prepared by you in relation to or in
connection with the appointment herein and against all damages, costs, charges and
expenses whatsoever in respect thereof or in relation thereto.
Nothing contained herein shall be construed to imply a partnership, joint venture, principal-agent or an
employer-employee relationship between the parties herein and neither party shall have any right,
power or authority to create any obligation, express or implied on behalf of the other.
This form and all terms and conditions herein and all disputes and differences arising out of or in
connection with this letter shall be governed by and construed in accordance with the laws of Malaysia
and the courts of Malaysia shall have exclusive jurisdiction.
