AFFIDAVIT OF DISCREPANCY

I, MARICRIS R. GENERAL, of legal age, married, Filipino and a resident of Purok 2,

Kahayag, Bacolod, Lanao del Norte, Philippines, having been duly sworn in accordance with
law, hereby depose and state:

That in my Certificate of Live Birth as recorded in the Office of the Civil Registrar of
Bacolod, Lanao del Norte, I was born on January 21, 1992, at Liangan East, Bacolod, Lanao del
Norte;

On the other hand, in my marriage certificate it is indicated that I was born on January
19, 1992;

That the date of birth appearing in my Birth Certificate and in my Marriage Certificate
refer to one and the same person, that the reason of such discrepancy is due to clerical
inadvertence in my marriage certificate;

That I am executing this Affidavit of Discrepancy to attest to the truth of the foregoing
facts and for the purpose of Change/Correction of entries in my marriage certificate;

Further, I am declaring that from this date hereon I would be using my true and correct
birth date in all my papers, records and other documents.
