AFFIDAVIT OF NON OPERATION

I, ERIC S. BANTOG, of legal age, Filipino and with residential address at #15 San
Vicente Road, Brgy. San Vicente, City of San Pedro, Laguna, after having been duly sworn to in
accordance with law, hereby depose and say that;

1. I am the owner of BANTOG CONSTRUCTION located at #15 San Vicente Road,

Brgy. San Vicente, City of San Pedro, Laguna;

2. The said business had no operation since December, 2021 up to present due to
bankruptcy;

3. I am executing this affidavit in order to attest to the truth of the foregoing statements,
to inform the authorities concerned and for all legal intents and purposes this may
serve best;
