RE: ABSENCE OF GEN. INFO. SHEET (G.I.S.)
I, MARIVELE ANTONETTE HERNANDEZ MARAYAG, of legal age,
Filipino, single, and resident of B10 L34 La Vecina [Camella] at Dos
Rios Subdivision, Brgy. Pittland, Cabuyao City, Laguna after having
been sworn to in accordance with law, do hereby depose and say:
That I am a duly elected Member of the Board of Directors of La
Vecina at Dos Rios Homeowners Association, Inc., (LVDRHOA for
brevity) a duly registered non-stock non-profit corporation with office
address at Clubhouse, La Vecina at Dos Rios Subdivision, Brgy.
Pittland, Cabuyao City, Laguna,

That I became a member of the said board of directors by virtue

of my election to the said board on 20 August 2022;
That at the moment, our association LVDRHOA cannot provide
and cannot submit a General Information Sheet (GIS) to Department
of Human Settlements and Urban Development (DHSUD) due to a
refusal by Vista Land and Lifescapes Inc., the subdivision developer,
to update the said GIS and provide an updated copy of the same;
That I am executing this affidavit to attest to the truth of these
statements and for any other lawful purposes this may serve.
