WAIVER OF RIGHTS

We, GENEROSO L. CODINO, widower, Filipino and of legal age and FRANK-
LEO E. CODINO, married, Filipino and of legal age, residents of Lower Cabatangan,
after having been duly sworn in accordance with law depose and say:

1. That we are the legal heirs of LUDIVINA E. CODINO, who died on September
13, 2018 at Lower Cabatangan, Zamboanga City;

2. That during her lifetime, she was an active and a bona fide holder of a St. Peter
Life Plan with Contract No. _________________;

3. That we do hereby waive, relinquish, and renounce andy and all of our rights and
interests, whatsover, over the said life insurance in favor of MERCY-LUD C.
NAVASCA, also a legal heir and daughter of the deceased;
